Palmar hyperhidrosis

Description:
Palmer hyperhidrosis is profuse perspiration (excessive sweating) of the palms.It is one form of focal hyperhidrosis, meaning profuse perspiration affecting one area of the body. Sweaty palms may be accompanied by profuse perspiration of the feet, forehead, ckeeks, armpits (axillae) or be part of general hyperhidrosis (profuse perspiration throughout the body). Hyperhidrosis refers to profuse perspiration beyond the body’s thermoregulatory (temperature control) needs.

Palmer  hyperhidrosis is a common condition in which the eccrine (sweat) glands of the palms and soles secrete inappropriately large quantities of sweat. The condition may become socially and professionally debilitating. The condition usually is idiopathic  and  it begins in childhood and frequently runs in families.

Symptoms:
The intensity of symptoms may vary among sufferers and trigger factors should be carefully noted. Common symptoms  are :

*Perspiration of the hands can vary from mild clamminess to severe perspiration resulting in dripping sweat.
*Temperature differences of palmar surface compared to surface temperature of other parts of the body may be noted.
*Sloughing (peeling) of skin may be noted in profuse perspiration.
*Episodes of profuse perspiration may be followed by periods of extreme dryness on the palmar surface.
*Hyperhidrosis often starts in puberty, and family history is often reported.

The secondary effects of palmar hyperhidrosis can result in both psychosocial effects as well as difficulty in undertaking certain tasks or handling equipment. Sufferers of palmar hyperhidrosis are often reluctant to partake in socially expected actions like shaking hands or touching loved ones. The embarrassment of dealing with this condition can affect the level of interactivity in both social and work situations. Difficulties with holding objects, gripping equipment or soiling electronic devices like keyboards may affect functioning at work. Daily activities such as writing with a pen or counting cash notes is often difficult.

Causes:
Hyperhidrosis is either primary focal or secondary generalized.

1. Primary Palmar  Hyperhidrosis

Focal palmar hyperhidrosis is usually localized and is referred to as primary (essential, idiopathic), meaning no obvious cause, except strong family predisposition can be found (4,5), and affected persons are otherwise healthy . Sweating on other locations as feet, armpits and face may appear. Primary palmar hyperhidrosis is caused by overactivity of the sympathetic nervous system, primarily triggered by emotional causes including anxiety, nervousness, anger and fear .

There may be a significant reduction in perspiration during sleep or sedation.

2. Secondary Palmar Hyperhidrosis

In secondary palmar hyperhidrosis hands sweat due to an obvious underlying disorder like:

*Infections including local infections, tuberculosis and tinea ugunium.
*Neurological disorders like peripheral autonomic neuropathy
*Frostbite
*Arteriovenous Fistulas
*Acromegaly
*Acrodynia
*Complex Regional Pain Syndromes
*Pachyonychia Congenita
*Primary Hypertrophic osteoarthropathy
*Dyskeratosis Congenita
*Blue rubber-bleb nevus
*Glomus tumor

*Secondary palmar hyperhidrosis as part of generalized hyperhidrosis due to  several  hormonal causes (diabetes, hyperthyroidism, thyrotoxicosis, menstruation, menopause), metabolic disorders, malignant disease (lymphoma, pheochromocitoma), autoimmune disorders (rheumatoid arthritis, systemic lupus erythrematosus), drugs like hypertensive drugs and certain classes of antidepressants (list of medications causing hyperhidrosis), chronic use of alcohol, Parkinson’s disease, neurological disorders (toxic neuropathy), homocystinuria, plasma cell disorders. How Sweat Glands Work:
In eccrine glands, the major substance enabling impulse conduction is acetylcholine, and in apocrine glands, they are catecholamines.

Body temperature is controlled by the thermoregulatory center in the hypothalamus and this is influenced not only by  by core body temperature but also by hormones, pyrogens, exercise and emotions.

Diagnosis:
The first step in diagnosing  the  Palmar  hyperhidrosis is to differentiate between generalized and focal hyperhidrosis.

A thorough case taking and medical history is usually sufficient to diagnose palmar hyperhidrosis and any trigger factors (scheduled drugs, narcotics, chronic alcoholism).

Diagnostic criteria for primary focal (including palmar) hyperhidrosis  are:

*Bilateral and relatively symmetric sweating
*Frequency of at least 1 episode per week
*Impairment of daily activities
*Age at onset before 25 years
*Family history
*Cessation of sweating during sleep

Tests may include:
*Hematological studies may be necessary to identify thyroid disorders (thyroid function test for T3 and T4 as well as thyroid antibodies) and diabetes (fasting blood glucose or a glucose tolerance test).

*X-rays and MRI scans will assist for diagnosing tuberculosis, pneumonia and tumors.

*Superficial electroconductivity can be monitored as any hyperhidrosis reduces skin electrical resistance.

*Thermoregulatory sweat test uses moisture-sensitive indicator powder to monitor moisture. Changes in the color of the powder at room temperature will highlight areas of increased perspiration.

Treatment:
Conservative management should be coupled with prescribed treatment by the Doctor to reduce the symptoms.

*Counseling may be effective in managing primary palmar hyperhidrosis in cases of mental-emotional etiology.

*Trigger foods and aggravating factors should be noted if possible and relevant dietary changes should be implemented.

*Effective prevention of secondary palmar hyperhidrosis is difficult with conservative management and drug therapy or surgery may be required.

*Excessive physical activity and extremes of heat may be two trigger factors that should be avoided as far as possible.

*In cases of diabetes, a glucose controlled diet with low glycemic index may improve glucose tolerance which could assist with palmar hyperhidrosis.

*Abstinence from alcohol and narcotics is advisable if it is the causative factor for sweaty palms.

*Stimulants such as caffeine and nicotine may aggravate palmar hypehidrosis and should relevant dietary and lifestyle changes should be implemented.

*Anti-perspirant compounds like aluminum chloride can be applied on the palms to reduce moisture or palmar surfaces. Recent research on an aluminum sesquichlorohydrate foam has shown that it is effective in reducing sweat in palmar hyperhidrosis

Treatment remains a challenge: options include topical and systemic agents, iontophoresis, and botulinum toxin type A injections, with surgical sympathectomy as a last resort. None of the treatments is without limitations or associated complications. Topical aluminum chloride hexahydrate therapy and iontophoresis are simple, safe, and inexpensive therapies; however, continuous application is required because results are often short-lived, and they may be insufficient. Systemic agents such as anticholinergic drugs are tolerated poorly at the dosages required for efficacy and usually are not an option because of their associated toxicity. While botulinum toxin can be used in treatment-resistant cases, numerous painful injections are required, and effects are limited to a few months.

Standard therapeutic protocol may differ among cases of palmar hyperhidrosis depending on medical history and underlying pathology.

*Anticholinergic drugs have a direct effect on the sympathetic nervous system although there are numerous side effects.

*Treatment should be directed at contributing factors.

*Ionophoresis involves the use of electrotherapeutic measures to reduce the activity of sweat glands.

*Botulinum injections at the affected area may be useful for its anticholinergic effects.

*Surgery should be considered if drug therapy proves ineffective. Endoscopic transthoracic sympathectomy involves resection of the sympathetic nerve supply to the affected area. This prevents nerve stimulation of the sweat gland of the palms. However surgery has a host of complications including exacerbating the problem or increasing generalized hyperhidrosis.

Surgical sympathectomy should be reserved for the most severe cases and should be performed only after all other treatments have failed. Although the safety and reliability of treatments for palmoplantar hyperhidrosis have improved dramatically, side effects and compensatory sweating are still common, potentially severe problems.

Description:
Capparis zeylanica is a climbing shrub common in the forests of the Indian subcontinent and China.A rigid, climbing, much-branched shrub; young parts clothed with rufous tomentum. Leaves 2.5-7.5 cm long, elliptic, oblong, obtuse, acute or retuse; stipular spines hooked. Flowers supra-axillary, solitary or 2-3, one above the other in a vertical line, the upper the longest. Sepals 9 mm long, densely rufous-pubescent outside; petals twice as long as the sepals, densely villous. Fruit subglobose, 3.2 cm across. Methanolic extracts of the leaves have been shown to reduce diarrhea in mice. Many butterfly larva feed on its leaves.

Flower: In axillary clusters; stamens cream when anthesis, red to purple in the evening. Flowering from February-April.

Fruit: An ovoid berry, pendulous, smooth, pustulate; blood red when ripe; seeds many. Fruiting April onwards.

Habitat : Physalis heterophylla  is native to North America, occurring primarily in the eastern United States and Canada. It is known to occur in all contiguous states except for Nevada and California. It is found mainly in habitats such as dry or mesic prairies, gravel hills and rises, sandy or rocky soils, and waste places such as roadsides.

Description:
Physalis heterophylla is a perennial, and is one of the taller-growing North American members of the genus, reaching a height up to 50cm. The leaves are alternate, with petioles up to 1.5cm, ovate in shape, usually cordate at the base (this is especially true of mature leaves), 6-11 cm long at maturity. Each member of the Physalis genus has at least one characteristic that makes it easy to differentiate in the field. For P. heterophylla, the stems and leaves are glandularly pubescent, giving it the “clammy” feel from which its name is derived. The plant also has distinctive thick rhizomes that run horizontal to the stem. Some sources recognize four distinct subspecies based primarily on leaf variation:

*P. heterophylla var. heterophylla, with thin leaves that have dentate margins;

*P. heterophylla var. clavipes, with thick, conspicuously veined leaves and sparingly tooth-like protrusions on otherwise entire margins;

*P. heterophylla var. ambigua, with thick, conspicuously veined leaves and dentate margins;

*P. heterophylla var. nycangienea, with thin leaves that have sparingly tooth-like protrusions on otherwise entire margins.

The flowers are on simple inflorescences that emerge from leaf apexes. The petals are yellow on the exterior, and yellow on the interior with purple highlights emanating up each petal from the base. They are funnelform in shape, with five fused petals. There are five reticulated sepals, which enlarge after flowering to eventually protect the maturing frut. Stamens five, with yellow anthers and purple filaments. The flowers face downwards when open, and are about 2.5cm in diameter. The fruits are typical for the family (appearing like a tomatillo), and have a slightly bitter taste, though they are perfectly edible. The rest of the plant is poisonous.

Cultivation:
Succeeds in any well-drained soil in full sun or light shade. A polymorphic species.

Propagation:
Seed – sow March/April in a greenhouse only just covering the seed. Germination usually takes place quickly and freely. Prick out the seedlings into individual pots of fairly rich soil when they are large enough to handle and plant them out after the last expected frosts. Consider giving them some protection such as a cloche until they are growing away well. Diurnal temperature fluctuations assist germination. Division in spring. Very easy, larger divisions can be planted out direct into their permanent positions. We have found that it is better to pot up the smaller divisions and grow them on in light shade in a cold frame until they are well established before planting them out in late spring or early summer. Basal cuttings in early summer. Harvest the shoots with plenty of underground stem when they are about 8 – 10cm above the ground. Pot them up into individual pots and keep them in light shade in a cold frame or greenhouse until they are rooting well. Plant them out in the summer.

Edible Uses
Edible Parts: Fruit.

Fruit – raw or cooked. Quite nice raw though rather small, the fruit can also be made into pies, jams, sauces etc. Pectin or pectin-rich fruit should be added if the fruit is used in jams and preserves. The fruit can also be dried, ground into a meal and added to flour for making bread etc. The plant conveniently wraps up each fruit in its own ‘paper bag’ (botanically, the calyx) to protect it from pests and the elements. This calyx is toxic and should not be eaten.

Medicinal Uses:
Antitumor; Diuretic; Poultice.

The seed is considered to be beneficial in the treatment of difficult urination, fever, inflammation and various urinary disorders. A tea made from the leaves is used in the treatment of headaches and as a wash for burns and scalds. A poultice of the leaves and roots is applied to wounds. An infusion of the leaves and roots is used as a wash on scalds, burns and VD sores. Compounds in the plant are being investigated for antitumor activity.

Known  Hazards : All parts of the plant, except the fruit, are poisonous

Description:
It is a long-taprooted plant forming a basal patch of leaves, with each leaf made up of several rounded, lobular segments. It reaches a maximum height exceeding half a meter and its erect stem is topped with an umbel of carrotlike white flowers.

Named after Peter von Glehn, a Russian botanist (this plant also grows in Asia), this spreading perennial is confined to beaches and coastal dunes. Its small white flowers are arranged in compact umbels. Its pinnately compound leaves have fleshy leaflets with distinct veins, some with lobes. Its fruits (seen in the pictures) are borne in clusters, each with wing-like ribs. It has a long taproot. Look for it in the dunes of Netarts Spit.

Medicinal uses:
This supplement is used in traditional Chinese medicine as an expectorant and to treat bronchitis and whooping cough. Its mechanism of action is unknown, but animal models reveal analgesic properties. It is reported that glehnia root can hemolyze blood cells, stimulate myocardial contractility, and exert antibacterial effects. Various extracts from glehnia root display analgesic effects in a mouse study utilizing acetic acid-induced writhing tests. Concentrations of 10-50 mg/kg polyacetylene and 80-100 mg/kg coumarin fractions are necessary to elicit analgesia. The roots improve functioning of the liver and kidneys; treat lung diseases, coughs including hacking cough, fever, chest pain.  It is especially effective in treating joint pain and muscle pain, both of acute injuries and in chronic conditions like rheumatoid or osteo arthritis. It can be topically applied and taken internally.   In Japan, Hamaboufuu is an important plant in traditional folk medicine. One ancient use is as an annual tonic. On the day of the Japanese New Year, Japanese people drink a medicinal alcoholic beverage called Toso. The drink contains several medicinal herbs of which Hamaboufuu is one. Drinking it on the New Year’s day is said to insure health in the coming year. It is registered in the Japanese Herbal Medicines Codex.

Alangium Platanifolium

Description:
A decidious Shrub growing to 3m by 2m.  This outstanding understory shrub can become a small, wide-spreading tree of considerable beauty at maturity. The large (16-21cm long by 13-15cm wide), broadly ovate yellowish green leaves resemble those of the London plane (Platanus × hispanica). They are variable in shape but are generally tri-lobed, forward pointing toward the apex, with cordate bases. Often the acuminate lobes are twisted, adding to the distinctive foliar texture of this plant. In the spring the leaves unfold in the manner of hands in prayer and then turn a glorious yellow in the autumn. The flowers are white and appear in late June along the undersides of main horizontal branches often hidden by the verdant foliage. They are borne in 1- to 4-flowered cymes from the leaf axils of the previous year’s growth. Each flower consists of 6 petals narrowly strap-shaped and slightly twisted, forming a corolla tube at the base. Each petal reflexes to the midpoint to expose the bright yellow stamens and style. The pendulous, fleshy, egg-shaped fruit, coloured porcelain blue to dark violet, provides a stunning contrast to the golden fall leaf display.

It is hardy to zone 8. It is in flower from June to July. The flowers are hermaphrodite (have both male and female organs)
The plant prefers light (sandy), medium (loamy) and heavy (clay) soils and requires well-drained soil. The plant prefers acid, neutral and basic (alkaline) soils. It cannot grow in the shade. It requires moist soil.

Cultivation:
Succeeds in any moderately fertile well-drained soil[200]. Requires full sun and a sheltered position. A fairly hardy plant, but it does not succeed outdoors at Kew, the soft pithy shoots being cut back by winter cold. It grows well in Gloucestershire. This species is closely related to A. chinense.

Propagation:
Seed – we have no details for this species but suggest sowing the seed in a greenhouse in the spring. When large enough to handle, prick the seedlings out into individual pots and grow them on in the greenhouse for at least their first winter. Plant them out in early summer and consider giving them some protection from the cold for their first winter outdoors. Cuttings of half-ripe wood, July/August in sand in a frame.

Edible Parts: Leaves.

Young leaves – cooked.

Medicinal  Actions & Uses:-
Antirheumatic.

The root is used in the treatment of rheumatism and other bone diseases.
