Definition :-
Abdominal ultrasound is an imaging procedure used to examine the internal organs of the abdomen, including the liver, gallbladder, spleen, pancreas, and kidneys. The blood vessels that lead to some of these organs can also be looked at with ultrasound.

It uses reflected sound waves to produce a picture of the organs and other structures in the upper abdomen. Occasionally a specialized ultrasound is ordered for a detailed evaluation of a specific organ, such as a kidney ultrasound.

An abdominal ultrasound can evaluate the:
*Abdominal aorta, which is the large blood vessel (artery) that passes down the back of the chest and abdomen. The aorta supplies blood to the lower part of the body and the legs.

*Liver, which is a large dome-shaped organ that lies under the rib cage on the right side of the abdomen. The liver produces bile (a substance that helps digest fat), stores sugars, and breaks down many of the body’s waste products.

*Gallbladder, which is a saclike organ beneath the liver. The gallbladder stores bile. When food is eaten, the gallbladder contracts, sending bile into the intestines to help in digesting food and absorbing fat-soluble vitamins.

*Spleen, which is the soft, round organ that helps fight infection and filters old red blood cells. The spleen is located to the left of the stomach, just behind the lower left ribs.

*Pancreas, which is the gland located in the upper abdomen that produces enzymes that help digest food. The digestive enzymes are then released into the intestines. The pancreas also releases insulin into the bloodstream; insulin helps the body utilize sugars for energy.
*Kidneys, which are the pair of bean-shaped organs located behind the upper abdominal cavity. The kidneys remove wastes from the blood and produce urine.

A pelvic ultrasound evaluates the structures and organs in the lower abdominal area (pelvis).

Why It Is Required to be Done:-
The specific reason for the test will depend on your symptoms. Abdominal ultrasound is mostly  done to:

*Determine the cause of abdominal pain.

*Detect, measure, or monitor an aneurysm in the aorta. An aneurysm may cause a large, pulsing lump in the abdomen.

*Evaluate the size, shape, and position of the liver. An ultrasound may be done to evaluate jaundice and other problems of the liver, including liver masses, cirrhosis, fat deposits in the liver (called fatty liver), or abnormal liver function tests.

*Detect gallstones, inflammation of the gallbladder (cholecystitis), or blocked bile ducts. See an illustration of a gallstone.

*Detect kidney stones.

*Determine the size of an enlarged spleen and look for damage or disease.

*Detect problems with the pancreas, such as pancreatitis or pancreatic cancer.

*Determine the cause of blocked urine flow in a kidney. A kidney ultrasound may also be done to determine the size of the kidneys, detect kidney masses, detect fluid surrounding the kidneys, investigate causes for recurring urinary tract infections, or evaluate the condition of transplanted kidneys.

*Determine whether a mass in any of the abdominal organs (such as the liver) is a solid tumor or a simple fluid-filled cyst.

*Determine the condition of the abdominal organs after an accident or abdominal injury and look for blood in the abdominal cavity. However, computed tomography (CT) scanning is more commonly used for this purpose because it is more precise than abdominal ultrasound.

*Guide the placement of a needle or other instrument during a biopsy.

*Detect fluid buildup in the abdominal cavity (ascites). An ultrasound also may be done to guide the needle during a procedure to remove fluid from the abdominal cavity (paracentesis).
How the Test is Performed :-
This test is done by a doctor who specializes in performing and interpreting imaging tests (radiologist) or by an ultrasound technologist (sonographer) who is supervised by a radiologist. It is done in an ultrasound room in a hospital or doctor’s office.

You will need to remove any jewelry that might interfere with the ultrasound scan. You will need to take off all or most of your clothes, depending on which area is examined (you may be allowed to keep on your underwear if it does not interfere with the test). You will be given a cloth or paper covering to use during the test.

An ultrasound machine creates images that allow various organs in the body to be examined. The machine sends out high-frequency sound waves, which reflect off body structures to create a picture. A computer receives these reflected waves and uses them to create a picture. Unlike with x-rays or CT scans, there is no ionizing radiation exposure with this test.

You will be lying down for the procedure. A clear, water-based conducting gel is applied to the skin over the abdomen. This helps with the transmission of the sound waves. A handheld probe called a transducer is then moved over the abdomen.

You may be asked to change position so that the health care provider can examine different areas. You may also be asked to hold your breath for short periods of time during the examination.

Abdominal ultrasound usually takes 30 to 60 minutes. You may be asked to wait until the radiologist has reviewed the information. The radiologist may want to do additional ultrasound views of some areas of your abdomen.

How To Prepare For the Test:-
Tell your doctor if you have had a barium enema or a series of upper GI (gastrointestinal) tests within the past 2 days. Barium that remains in the intestines can interfere with the ultrasound test.

Preparation for the procedure depends on the nature of the problem and your age. Usually patients are asked to not eat or drink for several hours before the examination. Your health care provider will advise you about specific preparation.

For ultrasound of the liver, gallbladder, spleen, and pancreas, you may be asked to eat a fat-free meal on the evening before the test and then to avoid eating for 8 to 12 hours before the test.

For ultrasound of the kidneys, you may not need any special preparation. You may be asked to drink 4 to 6 glasses of liquid (usually juice or water) about an hour before the test to fill your bladder. You may be asked to avoid eating for 8 to 12 hours before the test to avoid gas buildup in the intestines. This could interfere with the evaluation of the kidneys, which lay behind the stomach and intestines.

For ultrasound of the aorta, you may need to avoid eating for 8 to 12 hours before the test.

How It Feels:-
There is little discomfort. The conducting gel may feel slightly cold and wet when it is applied to your stomach unless it is first warmed to body temperature. You will feel light pressure from the transducer as it passes over your abdomen. The ultrasound usually is not uncomfortable. However, if the test is being done to assess damage from a recent injury, the slight pressure of the transducer may be somewhat painful. You will not hear or feel the sound waves.

Risks Factors:
There is no documented risk. No ionizing radiation exposure is involved.

Results:-
An abdominal ultrasound uses reflected sound waves to produce a picture of the organs and other structures in the abdomen.

Abdominal ultrasound  Normal:
The size and shape of the abdominal organs appear normal. The liver, spleen, and pancreas appear normal in size and texture. No abnormal growths are seen. No fluid is found in the abdomen.

The diameter of the aorta is normal and no aneurysms are seen.

The thickness of the gallbladder wall is normal. The size of the bile ducts between the gallbladder and the small intestine is normal. No gallstones are seen.

The kidneys appear as sharply outlined bean-shaped organs. No kidney stones are seen. No blockage to the system draining the kidneys is present.

Abdominal ultrasound Abnormal:
An organ may appear abnormal because of inflammation, infection, or other diseases. An organ may be smaller than normal because of an old injury or past inflammation. An organ may be pushed out of its normal location because of an abnormal growth pressing against it. An abnormal growth (such as a tumor) may be seen in an organ. Fluid in the abdominal cavity (ascites) may be seen.

The aorta is enlarged, or an aneurysm is seen.

The liver may appear abnormal, which may indicate liver disease (such as cirrhosis or cancer).

The walls of the gallbladder may be thickened, or fluid may be present around the gallbladder, which may indicate inflammation. The bile ducts may be enlarged because of blockage (from a gallstone or an abnormal growth in the pancreas). Gallstones may be seen inside the gallbladder.

The kidneys may be enlarged because of urine that is not draining properly through the ureters. Kidney stones are seen within the kidneys (not all stones can be seen with ultrasound).

An area of infection (abscess) or a fluid-filled cyst may appear as a round, hollow structure inside an organ. The spleen may be ruptured (if an injury to the abdomen has occurred).

If Tea is Hot, Wait Four Minutes

Drinking very hot tea appears to increase the risk of oesophageal cancer, a new study has shown, prompting suggestions for a four-minute wait before swallows of freshly boiled tea.

The study from northern Iran, the largest so far to explore tea-drinking habits and oesophageal cancer, has corroborated earlier research from India, Singapore and South America that linked this cancer to hot beverages.

Researchers from the Tehran University of Medical Sciences studied tea-drinking habits and patterns of oesophageal cancer in Golestan province where black tea is popular. They found that people who consumed very hot tea (defined as 70°C or higher) had an eight-fold higher risk of oesophageal cancer than people who drank tepid tea (65°C or lower).

They found that drinking tea at temperatures between 65°C and 69°C — defined as simply hot — was associated with twice the risk of cancer of the oesophagus. Their research will appear in the British Medical Journal on Friday.

“It’s clear hot beverages are contributing to high levels of oesophageal cancer in this population, Paul Brennan, a research team member from the International Agency for Research in Cancer in Lyons, France, told The Telegraph.

“But other factors may be associated with oesophageal cancer in other populations,” said Brennan, head of genetic epidemiology unit at the IARC. “We need to investigate different factors in different regions or populations.”

The Iranian study also showed that waiting for tea to cool lowered the risk of the cancer. People who typically drank their tea within two minutes after it was poured had a five-fold higher risk than those who waited for four minutes or longer.

Although previous studies have pointed to the potential danger of hot beverages, Iranian digestive disorder specialist Reza Malekzadeh and his colleagues are among the first to investigate the link through rigorous temperature measurements.

Malekzadeh said the significance of the new research was in the use of statistical techniques to eliminate the effects of other risk factors that could also contribute to oesophageal cancer.

But doctors assert that there is no cause for alarm. “The public health message here is that people should wait four minutes before they begin sipping from a cup of hot tea,” Malekzadeh told The Telegraph.

Eight years ago, Rup Kumar Phukan and his colleagues at the Regional Medical Research Centre, Dibrugarh, Assam, had examined dietary habits in parts of northeastern India and shown that hot beverages and spicy food were linked to oesophageal cancer.

They had suggested that the long-term consumption of exceptionally hot food or beverages could cause chronic irritation and harm the lining of the oesophagus. “But chewing tobacco and smoking are also likely to be among the contributing factors in this region,” said a scientist at the Dibrugarh centre.

The Iranian study measured tea temperatures consumed by more than 48,500 people and studied tea-drinking habits of 300 patients with oesophageal cancer and 571 healthy people, emerging as the largest study on the topic.

Speculating on mechanisms to explain the link, the researchers have pointed out that chronic inflammation by high temperatures may stimulate the release of nitric oxide and reactive oxygen species — potentially harmful biomolecules.

Doctors caution that cancer is almost always a multi-factorial disease. The risk may be lowered or increased by several factors. Low consumption of fruits and vegetables, for instance, may increase the risk of cancer.

Cultural importance in the Indian subcontinent:
It is worshipped as a divine tree in the Indian subcontinent, particularly in Kashmir and Punjab villages, as the name deodar, a Sanskrit word, (Sanskrit: devdar), which means, “divine wood”. The first half of the word deva means the words divine, deity, deus, and Zeus and the second part connotes durum, druid, tree, and true. Several Hindu legends refer to this tree.

Forests full of Devadaru trees were the favorite abode or living place of ancient Indian sages and their families who were devoted to Hindu god Shiva for whom they performed very difficult tapasya (meditation) to please him.
It is the national tree of Pakistan.

Description:
It is a large evergreen coniferous tree reaching 40-50 m tall, exceptionally 60 m, with a trunk up to 3 m diameter. It has a conic crown with level branches and drooping branchlets.Dioecious trees are very rare.

The leaves are needle-like, mostly 2.5-5 cm long, occasionally up to 7 cm long, slender (1 mm thick), borne singly on long shoots, and in dense clusters of 20-30 on short shoots; they vary from bright green to glaucous blue-green in colour. The female cones are barrel-shaped, 7-13 cm long and 5-9 cm broad, and disintegrate when mature (in 12 months) to release the winged seeds. The male cones are 4-6 cm long, and shed their pollen in autumn.

Cultivation and uses
It is widely grown as an ornamental tree, much planted in parks and large gardens for its drooping foliage. General cultivation is limited to areas with mild winters, with trees frequently killed by temperatures below about ?25 °C, limiting it to hardiness zones 8 and warmer for reliable growth. It is commonly grown in western Europe (north to Scotland), in the Mediterranean region, around the Black Sea, in southern and central China, on the west coast of North America as far north as Vancouver, British Columbia, and in the southeastern United States from Texas to Virginia.The most cold-tolerant trees originate in the northwest of the species’ range in Kashmir and Paktia Province, Afghanistan. Selected cultivars from this region are hardy to zone 7 or even zone 6, tolerating temperatures down to about ?30 °C. Named cultivars from this region include ‘Eisregen’, ‘Eiswinter’, ‘Karl Fuchs‘, ‘Kashmir’, ‘Polar Winter’, and ‘Shalimar’. Of these, ‘Eisregen’, ‘Eiswinter’, ‘Karl Fuchs’, and ‘Polar Winter’ were selected in Germany from seed collected in Paktia; ‘Kashmir’ was a selection of the nursery trade, whereas ‘Shalimar’ originated from seeds collected in 1964 from Shalimar Gardens, India (in the Kashmir region) and propagated at the Arnold Arboretum.

Use in Construction material:
Deodar is in great demand as building material because of its durability, rot-resistant character and fine close grain, which is capable of taking high polish. It’s historical use to construct religious temples and as landscape around temples is well recorded. In India, during the British colonial period, deodar wood was used extensively for construction of barracks, public buildings, bridges, canals and railway cars.

The heartwood yields about 2.1 % of essential oil, consisting mainly of the sesquiterpene hydrocarbons a-himachalene 6-7%, p-himachalene around 91 % and other isomers including o-himachalene,2 with p-methyl acetophenone, p-methyl 3-tetrahydroacetophenone, atlantone and himachalo.

The petroleum ether extract of the bark oil yields saturated, straight chain and branched chain hydrocarbons (CI4-C2O)’

Flavonoids
Stem bark contains deodarin (3′,4′,5,7-tetrahydroxy-8-C-methyl dihydroflavonol), taxifolin and quercetin.

Medicinal Uses:
The curative properties of Deodar are well recorded in Indian Ayurvedic medicines, which are indicated below:-
Antidote; Astringent; Carminative; Diaphoretic; Diuretic; Skin; TB.

The inner wood is aromatic and used to make incense. Inner wood is distilled into essential oil. As insects avoid this tree, the essential oil is used as insect repellant on the feet of horses, cattle and camels. It also has antifungal properties and has some potential for control of fungal deterioration of spices during storage. The outer bark and stem are astringent. Its Biomedical actions are reported to be Carminative, antispasmodic, creates sweating, urination and is aromatic. Deodar’s Ayurvedic actions are reported to be a) increasing digestive function, b) removal of toxins from the bowel, c) alleviating coughing, d) cures skin disorders such as eczema and psoriasis. Cedar oil is often used for its aromatic properties, especially in aromatherapy and has characteristic woody odour which may change somewhat in the course of drying out. The crude oils are often yellowish or even darker in colour. Its applications cover soap perfumes, household sprays, floor polishes and insecticidesand also for microscope work as a clearing oil.

The heartwood is carminative, diaphoretic and diuretic. It is used in the treatment of fevers, flatulence, pulmonary and urinary disorders, rheumatism, piles, kidney stones, insomnia, diabetes etc. It has been used as an antidote to snake bites.

The plant yields a medicinal essential oil by distillation of the wood, it is used in the treatment of phthisis, bronchitis, blennorrhagia and skin eruptions.

The bark is astringent. It has proved useful in the treatment of fevers, diarrhoea and dysentery.

In Ayurvedic medicine the leaves are used in the treatment of tuberculosis.
Anticancer activity: The ethanolic extract was found to have cytotoxicity against human epidermal carcinoma of the nasopharynx in tissue culture.

Safety profile
A 15 % mixture of C. deodara oil in castor oil was subjected to acute toxicity tests in mice and found to be non-toxic. The formulation was non-irritant to the skin of rabbit and sheep and did not alter blood urea nitrogen and blood glucose levels. The LDso was 500 mgiii in adult albino mice. Applied topically to rabbits, it was found to have no adverse effects on skin or any other vital organ.

Blue Zones: Places on Earth Where People Live Longest

If you are looking for a Fountain of Youth, forget pills and diet supplements. Adventurer Dan Buettner has visited four spots on the globe where people live into their 90s and 100s and outlines how they add years of good life in his book, ‘The Blue Zones.’

The answer, Buettner says, includes smaller food portions, an active lifestyle and moderate drinking. “If someone tells you they have a pill or hormone (that extends life), you’re about to lose money,” Buettner says.

Buettner identifies four hotspots of longevity: the mountainous Barbagia region of Sardinia, an island off the coast of Italy; the Japanese island of Okinawa; a community of Seventh-day Adventists in Loma Linda, California, 60 miles east of Los Angeles; and the Nicoya Peninsula of Costa Rica, in Central America.

The term ‘Blue Zones’ takes its name from the blue ink Belgian demographer Michel Poulain used to circle an area of long-living Sardinians on a map.

What Buettner found in his seven years of research and travel were common denominators among the vigorous super-elderly – close family relationships, a sense of purpose, healthy eating habits. He distills them into what he calls the Power Nine.

“Picking half a dozen things off of this al a carte menu, and sticking to it, is probably worth eight to 10 (extra) years for the average American,” says Buettner, a tall 48-year-old who hopes to live until at least 100.

Buettner turned to probing the secrets of the longest-living cultures after leading three long-distance bicycle expeditions – from the tip of North America to the tip of South America; across the United States, Europe and the Soviet Union; and across Africa – in the 1980s and 1990s. He also used the internet to take classrooms on interactive quests to solve everything from the collapse of ancient Mayan civilization to human origins in Africa.

Buettner made his first expedition to Okinawa in 2000 and eventually wrote a National Geographic cover story, ‘The Secrets of Long Life,’ in November 2005.

Living long – even forever – is a human desire throughout history, says Robert Butler, president and CEO of the International Longevity Center- USA in New York. But Butler says he’s skeptical of claims of places of long-living people. “There’s always been these rumors but they’ve always turned out to be inaccurate,” said Butler.

 The problem with coffee is that it contains caffeine, a xanthine alkaloid that acts as a stimulant. But it is not just coffee that contains caffeine. The compound is also found in tea, carbonated beverages and chocolate.

Coffee contains 40 to 120 milligrams of caffeine per 120 ml, tea 15-30mgm/120ml, chocolate 3mg/30gm and cola drinks 20 to 50mgm/240ml. There can be an acute overdose of caffeine if more than three or four cups of brewed coffee, tea or cola drinks (providing 400 milligrams) are consumed in a short time. This causes caffeine intoxication with restlessness, nervousness, agitation, excitement, insomnia, flushing, urination, muscle twitching, rambling thought processes and speech, irritability and palpitations.

Caffeine ingestion can be fatal if more than 150 to 200 mgm per kilogram (80 to 100 cups of coffee for an average adult) is consumed within a short while. Overdose fatalities can occur in addicted people who snort pure caffeine powder. Caffeine stimulates the central nervous system. It makes the person alert with faster and clearer thought processes, improved focus, coordination, endurance and peak performance, especially in sports.

If large amounts are consumed over a prolonged period, caffeinism, addiction or dependency can occur. The person exhibits nervousness, irritability, anxiety, tremulousness, muscle twitching, insomnia, headaches, palpitations, peptic ulcers and gastroesophageal reflux disease (GERD). Tolerance develops quickly, especially among heavy coffee and cola drink consumers. Withdrawal is associated with symptoms such as headache, irritability and an inability to concentrate, which may last for days.

About 10 per cent of heavy coffee or tea drinkers develop symptoms that mimic organic mental disorders with anxiety, agitation and panic attacks. They may be misdiagnosed and unnecessarily medicated. Withholding caffeine would have actually cured them in a few days. Children should not be given tea or coffee, because caffeine stimulates their nervous system. They cannot tolerate more than 45 mgm of caffeine a day. Children who consume more than this become nervous, jittery, hyperactive, have difficulty concentrating and sleeping and have a rapid heart rate.

Parents sometimes do give their children cola drinks. Some of the orange or lime and lemony flavoured drinks also contain caffeine. The ingredients should be carefully checked on the beverage label before it is given to the child.

During the hot summer months, drinking caffeine-containing beverages to overcome thirst is actually counterproductive. Caffeine acts a mild diuretic (it increases urine production). Thirst is not assuaged and dehydration may be aggravated.

However, coffee and tea drinking is not entirely bad. Caffeine increases alertness and reduces drowsiness, two qualities essential for those on night shift work or driving a vehicle for a prolonged period of time.

Also, coffee enhances the action of many painkillers. The onset of action is faster as absorption is rapid and eventual relief is 40 per cent better. Many over-the-counter (OTC) headache, cold and flu drugs contain caffeine.

Coffee and tea drinking is also associated with a reduced risk of developing type 2 diabetes, gallstone disease, rheumatoid arthritis, alcoholic cirrhosis and Parkinson’s disease. Adults can safely consume two to three cups of coffee daily as it has no detrimental long-term health consequences and may even confer some health benefits.

Menopausal women are more sensitive to caffeine than their menstruating counterparts. It may produce jitteriness and interfere with sleep at lower levels. The traditional tea time consumption may need to be curtailed to ensure a good night’s rest.

Women have to be careful about their caffeine intake from all sources (not just tea and coffee) if they are pregnant or are planning to have a baby. The best time to stop is around a month before the pregnancy is planned. This abstinence should ideally be continued throughout the pregnancy. The caffeine is harmful because it stimulates the baby’s immature metabolism and stresses it. It is also suspected to decrease blood flow to the placenta. This in turn increases the risk of miscarriage and may eventually result in a small, underweight baby.

If you want to enjoy your cup of tea or coffee:-

• Start after you are an adult

• Restrict yourself to three or four cups a day

• Avoid additional caffeine in carbonated beverages

• Have your last cup at least six hours before your bedtime

• Avoid both tea and coffee during pregnancy.
