Sinusitis is an acute or chronic inflammation of the nasal sinuses-the hollow cavities found within the cheek bones and near the eyes. The inflammation is usually triggered by inadequate draining due to allergies, infections or structural problems of the nose such as narrow drainage passages or a deviated septum. Sinuses help warm, moisten and filter the air in the nasal cavity and also add resonance to certain sounds. . Normally, mucus collecting in the sinuses drains into the nasal passages. When you have a cold or allergy attack, your sinuses become inflamed and are unable to drain. This can lead to congestion and infection. Diagnosis of acute sinusitis usually is based on a physical examination and a discussion of your symptoms. Although colds are the most common cause of acute sinusitis, it is more likely that people with allergies will develop sinusitis. Allergies can trigger inflammation of the sinuses and nasal mucous linings. This inflammation prevents the sinus cavities from clearing out bacteria, and increases your chances of developing secondary bacterial sinusitis. If you test positive for allergies, your physician can prescribe appropriate medications to control your symptoms, thereby reducing the risk of developing an infection. People with sinus problems and allergies should avoid environmental irritants such as tobacco smoke and strong chemical odors, which may increase symptoms.

Structural problems in the nose—such as narrow drainage passages, tumors or polyps, or a deviated nasal septum (the bone and cartilage between the left and right sides of the nose)—may be another cause of sinusitis. Surgery is sometimes needed to correct these problems. Many patients with recurring or chronic sinusitis have more than one factor that predisposes them to infection. So, addressing only one factor may not be sufficient.

Diagnosis
Even if symptoms seem to be localized to the sinuses, the sinuses are not always infected. To make a correct diagnosis, a physician will take a detailed history and perform a physical examination. The physician may also order tests, if indicated. These tests can include allergy testing, sinus X-ray, CT scans (which make precise images of the sinus cavities), or a sampling of the nasal secretions or lining.

The physician also may perform an endoscopic examination. This involves inserting a narrow, flexible fiber-optic scope into the nasal cavity through the nostrils, which allows the physician to view the area where the sinuses and middle ear drain into the nose in an easy, painless, “patient friendly” manner.

Treatment
Sinus infections generally require a combination of therapies. In addition to prescribing an antibiotic when the sinusitis is caused by bacterial infection, your physician may prescribe a medication to reduce blockage or control allergies. This will help keep the sinus passages open. This medicine may be a decongestant, a mucus-thinning medicine or a cortisone nasal spray. Antihistamines, cromolyn and topical steroid nasal sprays help control allergic inflammation.

For people with allergies, long-term treatment to control and reduce allergic symptoms can also be effective in preventing the development of sinusitis. This treatment may include immunotherapy (also called “allergy shots”), anti-inflammatory medications, decongestants, and environmental control measures. Preventative use of low dose antibiotics and sinus drainage medications during times when symptoms will likely be worse, such as winter, also may prevent sinusitis.

Several non-drug treatments can also be VERY helpful. These include breathing in hot, moist air, applying hot packs and washing the nasal cavities with salt water. In cases of obstructed sinus passages that may require surgery, your allergist/immunologist may refer you to an otorhinolaryngologist, or an ear-nose-throat physician (ENT).

Sinusitis vs. rhinitis
Although many symptoms are similar, sinusitis differs from allergic rhinitis, known as “hay fever,” or non-allergic (vasomotor) rhinitis. Rhinitis is an inflammation of the mucous membranes of the nose—not the sinuses. Allergic rhinitis is caused by allergies and is often characterized by a runny nose, sneezing and congestion, and itchy eyes, nose, throat and inner ears. Non-allergic rhinitis is characterized by a swollen, inflamed nasal lining overflowing clear nasal drainage and a stuffy nose. It may be triggered by irritants such as smoke, changes in barometric pressure or temperature, or overuse of over-the-counter decongestant nasal sprays. Poorly controlled chronic or recurring rhinitis can lead to sinusitis.

As you can see, symptoms of sinusitis can vary depending on the severity of the inflammation and the sinuses involved—all of the symptoms listed above may be present, or only a few. It’s best to consult your physician promptly if any of the described symptoms of sinusitis develop.

Definition:
Pancreatitis is an inflammation of the Pancreas. The pancreas is a large gland behind the stomach . It secretes insulin, which is of fundamental importance in the handling of glucose. If the pancreas is not functioning properly diabetes may develop.

Another function of the pancreas is to secrete digestive enzymes into the small intestine through a tube called the pancreatic duct. These enzymes help digest fats, proteins, and carbohydrates in food.

Symptoms:
The symptoms of acute pancreatitis usually begins with severe pain in the upper abdomen. The pain may last for a few days. Some of the other symptoms are: swollen and tender abdomen, sweating, nausea, vomiting,fever, mild jaundice, and rapid pulse.

Diagnosis:

To diagnose pancreatitis, your doctor will take your medical history and perform a complete medical examination. He or she will be particularly interested in how much alcohol you drink and if you have had symptoms of gallstones. Diagnostic tests include blood and urine studies for pancreatic enzymes and sugars, x-rays of the abdomen and chest, ultrasound exam of the pancreas and gallbladder, and computed tomography (CT) scan of the pancreas. In severe cases of chronic pancreatitis, your doctor may order an endoscopic retrograde cholangiopancreatography (ERCP). An ERCP is a way of looking at your pancreas through a slim flexible tube, called an endoscope, that is inserted into your mouth and down to the pancreas. An endoscope is fitted with a tiny fiber optic camera that gives the physician a detailed view of the pancreas. During the ERCP, the physician can remove a sample of tissue, a biopsy, from the pancreas.In some cases the doctor may want to do Endoscoip Ultrasonography(EUS) to detect the cause of Pancreatities. Your doctor may also want a stool sample to test for excess fats.

Types Of Pancreatities:

There are two main typs of pancreatitis: acute and chronic. Acute pancreatitis occurs suddenly and lasts for a short period of time. It ususally resolves. Some people with acute pancreatitis may have more than one attack and recover completely after each. However, acute pancreatitis can be a severe, life-threatening illness with many complications. About 80,000 cases occur in the United States each year. The usual cause of chronic pancreatitis is many years of alcohol abuse, but the chronic form may also be triggered by only one acute attack, especially if the pancreatic ducts are damaged. The result of chronic pancreatitis is an inability to properly digest fat caused by a lack of pancreatic enzymes. The production of insulin is also affected.

Causes Of Pancreatities:

Chronic pancrestitis occurs when digestive enzymes attack and destroy the pancreas and nearby tissues.. Chronic pancratitis is usually caused by many years of alcohol abuse, excess iron in the blood, and other unknown factors. However, it may also be triggered by only one acute attack, especially if the pancreatic ducts are damaged.

Complecations of Pancreatities:
Some of the complications from pancreatitis are: low blood pressure heart and kidney failure, ARDS(adult respiratory distress symdrome),diabetes, ascites,accumulation of fluid in the abdomen, ans cysts or abscesses in the pancreas.

Treatments of Pancreatities:

Treatment depends on how bad the attack is. If no complications in the form of kidney failure or lung problems occur, acute pancreatitis usually improves on its own.
The goal of therapy is to maintain circulation and fluid volume. Treatment measures must also relieve pain and decrease pancreatic secretions. In 90% of patients with acute pancreatitis, the disease occurs as a mild self-limiting illness and requires simple supportive care alone. In the remaining 10% of patients, the disease can evolve into a severe form of acute pancreatitis with significant complications, a lengthy duration of illness, and a significant mortality rate.

Prevention:

Although pancreatitis isn’t always preventable, you can take steps to reduce your risk:

  • Avoid excessive alcohol use. Overuse of alcohol (in empty stomac specially) is the leading cause of chronic pancreatitis and a contributing factor in many acute attacks.
  • Stop smoking. Tobacco use increases your risk of pancreatitis, especially if you also drink alcohol.
  • Limit fat. Eating a high-fat diet can raise your blood-fat levels and increase your risk of gallstones – both risk factors for pancreatitis. A healthy diet emphasizes fresh fruits and vegetables, whole grains, and lean protein, and limits fats, especially saturated fats such as butter.

Diabetes is a disease in which the body does not produce or properly use insulin. Insulin is a hormone that is needed to convert sugar, starches and other food into energy needed for daily life. The cause of diabetes continues to be a mystery, although both genetics and environmental factors such as obesity and lack of exercise appear to play roles.

In order to determine whether or not a patient has pre-diabetes or diabetes, health care providers conduct a Fasting Plasma Glucose Test (FPG) or an Oral Glucose Tolerance Test (OGTT). Either test can be used to diagnose pre-diabetes or diabetes.
With the FPG test, a fasting blood glucose level between 100 and 125 mg/dl signals pre-diabetes. A person with a fasting blood glucose level of 126 mg/dl or higher has diabetes....click & see

In the OGTT test, a person’s blood glucose level is measured after a fast and two hours after drinking a glucose-rich beverage. If the two-hour blood glucose level is between 140 and 199 mg/dl, the person tested has pre-diabetes. If the two-hour blood glucose level is at 200 mg/dl or higher, the person tested has diabetes.

Major Types of Diabetes


Type 1 diabetes

Results from the body’s failure to produce insulin, the hormone that “unlocks” the cells of the body, allowing glucose to enter and fuel them. It is estimated that 5-10% of Americans who are diagnosed with diabetes have type 1 diabetes.
Symptoms of type 1 diabetes are often dramatic and come on very suddenly.
* Type 1 diabetes is usually recognized in childhood or early adolescence, often in association with an illness (such as a virus) or injury.
* The extra stress can cause diabetic ketoacidosis.
* Symptoms of ketoacidosis include nausea and vomiting. Dehydration and often-serious disturbances in blood levels of potassium follow.
* Without treatment, ketoacidosis can lead to coma and death.

Symptoms of type 2 diabetes are often subtle and may be attributed to aging or obesity.
* A person may have type 2 diabetes for many years without knowing it.
# Type 2 diabetes can be precipitated by steroids and stress.
# If not properly treated, type 2 diabetes can lead to complications like blindness, kidney failure, heart disease, and nerve damage

Type 2 diabetes
Results from insulin resistance (a condition in which the body fails to properly use insulin), combined with relative insulin deficiency. Most Americans who are diagnosed with diabetes have type 2 diabetes.

Gestational diabetes
Gestational diabetes affects about 4% of all pregnant women – about 135,000 cases in the United States each year.

Pre-diabetes

Pre-diabetes is a condition that occurs when a person’s blood glucose levels are higher than normal but not high enough for a diagnosis of type 2 diabetes. There are 41 million Americans who have pre-diabetes, in addition to the 20.8 million with diabetes.

CAUSES OF DIABETES:
Diabetes mellitus occurs when the pancreas doesn’t make enough or any of the hormone insulin, or when the insulin produced doesn’t work effectively. In diabetes, this causes the level of glucose in the blood to be too high.

In Type 1 diabetes the cells in the pancreas that make insulin are destroyed, causing a severe lack of insulin. This is thought to be the result of the body attacking and destroying its own cells in the pancreas – known as an autoimmune reaction.

* infection with a specific virus or bacteria;
* exposure to food-borne chemical toxins; and
* exposure as a very young infant to cow’s milk, where an as yet unidentified component of this triggers the autoimmune reaction in the body.

However, these are only hypotheses and are by no means proven causes.

Type 2 diabetes: Type 2 diabetes is believed to have a strong genetic link, meaning that it tends to run in families. Several genes are being studied that may be related to the cause of type 2 diabetes.
Type 2 diabetes is believed to develop when:
* the receptors on cells in the body that normally respond to the action of insulin fail to be stimulated by it – this is known as insulin resistance. In response to this more insulin may be produced, and this over-production exhausts the insulin-manufacturing cells in the pancreas;
* there is simply insufficient insulin available; and
* the insulin that is available may be abnormal and therefore doesn’t work properly..

The following risk factors increase the chances of someone developing Type 2 diabetes:
* High blood triglyceride (fat) levels
* Gestational diabetes or giving birth to a baby weighing more than 9 pounds
* High-fat diet
* High alcohol intake
* Sedentary lifestyle
* Obesity or being overweight

* Ethnicity: Certain groups, such as African Americans, Native Americans, Hispanic Americans, and Japanese Americans, have a greater risk of developing type 2 diabetes than non-Hispanic whites.

* Aging: Increasing age is a significant risk factor for type 2 diabetes. Risk begins to rise significantly at about age 45 years, and rises considerably after age 65 years.
Rarer causes of diabetes include:
* Certain medicines;
* pregnancy (gestational diabetes); and
* any illness or disease that damages the pancreas and affects its ability to produce insulin e.g. pancreatitis.

It’s important to also be aware of the different myths that over the years have arisen about the causes of diabetes.
Eating sweets or the wrong kind of food does not cause diabetes. However, it may cause obesity and this is associated with people developing Type 2 diabetes.
Stress does not cause diabetes, although it may be a trigger for the body turning on itself as in the case of Type 1 diabetes. It does, however, make the symptoms worse for those who already have diabetes.
Diabetes is not contagious. Someone with diabetes cannot pass it on to anyone else.

What causes a sore throat?

A sore throat can have many causes.  Some of these are listed below:

  1. Many common viruses, and even the viruses that cause mononucleosis (mono) and the flu, can cause a sore throat. Some of the viruses that cause a sore throat can produce blisters in the mouth and throat (“aphthous stomatitis”).
  2. Breathing through the mouth can produce throat dryness and soreness.
  3. Sinus drainage may cause a sore throat.
  4. Shouting very loudly for a long period may cause sore throat.
  5. A sore throat can also be caused by bacteria. The two most common bacteria to cause as sore throat are Streptococcus (which causes strep throat) and Arcanobacterium haemolyticum. Arcanobacterium causes sore throats mainly in young adults and is sometimes associated with a fine red rash.
  6. Common cold may sometimes inflamate the gland and cause sore throat.
  7. A sore throat that lasts for more than 2 weeks can be a sign of a serious illness, such as throat cancer or AIDS.So, sore throat, if lasts more than 2 weeks, contact a doctor (preferably ENT spacilist)immediately.

Try these home remedies to cure it :-

Gargle with your own fresh hot urin will cures Sore Throat easily.
Drink a glass of hot water with 1 teaspoon lemon juice and some honey. You’ll feel better instantly. You can also drink a chamomile tea with lemon and honey added.

  • Gargle with warm salt water to help reduce swelling and relieve discomfort:
  • Gargle at least once each hour with 1 tsp (5 g) of salt dissolved in 8 fl oz (237 mL) of warm water.
  • If you have postnasal drip, gargle often to prevent more throat irritation
  • Prevent dehydration. . Fluids may help thin secretions and soothe an irritated throat. Hot fluids, such as tea or soup, may help decrease throat irritation
  • Two tablespoons of fenugreek seeds should be added to a litre of cold water and simmered on a gentle flame. This should be allowed to cool to a temperature that could be tolerated by the buccal cavity. Strain this mixture and gargle with it several times a day. This cures a sore throat.
  • Soak 8-10 almonds. Peel and grind with 8-10 pepper corns. Mix in 1 cup of water. Sieve and drink adding either salt or sugar to taste.
  • The leaves of holy basil have also been found beneficial in the in curing sore throat. Water boiled with basil leaves should be taken as a drink, and also used as a gargle to relieve a sore throat
  • Soak two teaspoonfuls of ajowan seeds in cold water. Add a pinch of common salt. Using this infusion as a common gargle substantially eases the acute sore throat.
  • Eat garlic. Garlic is an anti-septic and good for many conditions.
  • Boil one whole onion, mash and eat with a little butter, salt and pepper.
  • A teaspoon of honey raw helps releave throat inflammation.
  • Use a vaporizer or cool air humidifier in your bedroom.
  • Warm or cool mist may help you feel more comfortable by soothing the swollen air passages. It may also relieve hoarseness. However, don’t let your room become uncomfortably cold or very damp.
  • Use a shallow pan of water to provide moisture in the air through evaporation if you don’t have a humidifier. Place the pan in a safe location where no one will trip on it or fall into it.

  • Do not smoke or use other tobacco products and avoid secondhand smoke.

Alternative practitioners recommend a variety of natural products for the treatment of sore throat. For example, aromatherapists recommend inhaling the fragrances of lavender, thyme, eucalyptus, sage, and sandalwood. Herbalists recommend taking osha root, ginger, or slippery elm. Some practitioners suggest gargling with a mixture of water, salt, and turmeric.
