The term coronary heart disease covers a group of clinical syndromes arising particularly from failure of the coronary arteries to supply sufficient blood to the heart. They include angina pectoris, coronary thrombosis or heart attack, and sudden death without infarction. click & see

Flue is a viral fever.Symptoms of flu includes: fever (usually high), headache, extreme tiredness, dry cough, sore throat, runny or stuffy nose, and muscle aches. Symptoms such as nausea, vomiting and diarrhea are more common among children than adults.Symptoms usually start four days after becoming infected with the virus. There are three types of influenza, A, B and C each with different symptoms but all treated basically the same. Fever, nasal congestion, deep chest cough, malaise, fatigue, sweating, diarrhea, vomiting, cramping, muscle aches and pains, weakness, dizziness, swollen glands, loss of appetite are just to name a few. The list expands depending on the severity of the illness.

Over the counter remedies are also suggested since this is a virus and viral infections do not respond to antibiotics it is very doubtful they would help at all. There is a medication that can be given if caught within the first three days of the illness. Contact your physician and let them know if you feel you have the flu. They often perform a test that lets them know what type of influenza has hit their area.

PREVENTION
Flu Vaccine

The main way to keep from getting flu is to get a yearly flu vaccine. You can get the vaccine at your doctor’s office or a local clinic, and in many communities at workplaces, supermarkets, and drugstores. You must get the vaccine every year because it changes.

Scientists make a different vaccine every year because the strains of flu viruses change from year to year. Nine to 10 months before the flu season begins, they prepare a new vaccine made from inactivated (killed) flu viruses. Because the viruses have been killed, they cannot cause infection. The vaccine preparation is based on the strains of the flu viruses that are in circulation at the time. It includes those A and B viruses expected to circulate the following winter.

Sometimes, an unpredicted new strain may appear after the vaccine has been made and distributed to doctor’s offices and clinics. Because of this, even if you do get the flu vaccine, you still may get infected. If you do get infected, however, the disease usually is milder because the vaccine will still give you some protection.

Until recently, you could get the flu vaccine only as an injection (shot). In 2003, however, the Food and Drug Administration (FDA) approved a nasal spray flu vaccine called FluMist that you can get from your health care provider. The FDA approved it for use in healthy people aged 5 to 49 years.

You should not use FluMist if

* You have certain lung conditions, including asthma, or heart conditions
* You have metabolic disorders such as diabetes or kidney dysfunction
* You have an immunodeficiency disease or are on immunosuppressive treatment
* You have had Guillain-Barré syndrome
* You are pregnant
* You have a history of allergy or hypersensitivity, including anaphylaxis, to any of the parts of FluMist or to eggs

Children or teenagers who regularly take aspirin or products containing aspirin also should not take FluMist.

Your immune system takes time to respond to the flu vaccine. Therefore, you should get vaccinated 6 to 8 weeks before flu season begins in November to prevent getting infected or reduce the severity of flu if you do get it. Because the flu season usually lasts until March, however, it’s not too late to get it after the season has begun. The vaccine itself cannot cause the flu, but you could become exposed to the virus by someone else and get infected soon after you are vaccinated.

Possible side effects

You should be aware that the flu vaccine can cause side effects. The most common side effect in children and adults is soreness at the site of the vaccination. Other side effects, especially in children who previously have not been exposed to the flu virus, include fever, tiredness, and sore muscles. These side effects may begin 6 to 12 hours after vaccination and may last for up to 2 days.

Viruses for producing the vaccine are grown in chicken eggs and then killed with a chemical so that they can no longer cause an infection. The flu vaccine may contain some egg protein, which can cause an allergic reaction. Therefore, if you are allergic to eggs or have ever had a serious allergic reaction to the flu vaccine, CDC recommends that you consult with your health care provider before getting vaccinated.

Vaccine recommendations
* You are 50 years of age or older
* You have chronic diseases of your heart, lungs, or kidneys
* You have diabetes
* Your immune system does not function properly
* You have a severe form of anemia
* You will be more than 3 months pregnant during the flu season
* You live in a nursing home or other chronic-care housing facility
* You are in close contact with children 0 to 23 months of age

Psoriasis is a disease/disorder, where angry red lesions on the skin , multiply, and scale over with silvery patches, Of course this is an over simplification.

But Psoriasis is non contagious, Usually inherited. It is an Autoimmune disorder and very rarely life threatening.

The word Psoriasis has come from ancient Greece and it means itch. Red eruptions appear on the surface of the skin and begin to eatch.These areas form plaques over the reddendend lesions. The plaques resemble multi-layered scales of skin. Psoriasis varies in intensity from a few random spots to a massive outbreak sometimes covering the entire body and requiring hospitalization too.

Psoriasis has a tendency to be genetically inherited.Recently it has been classified as being an autoimmune disorder.This disorder can originate in juveniles or not be evident until adulthood.It has been reported to initiate as early as birth or not occur until very late in life.Once Psoriasis begins, there are only remissions and replaces of varying degree of intesity..There is no known cure yet,only possible control over the severity.

Psoriasis reacts.It has triggers(such as systematic step infection) which can cause the body to go from mild to severe case within days.There are also other factors,such as sunlight,which may help to decrease the severity.

Psoriasis occurs in 2% of the total population.It can be mildly annoying problem or can destroy the self-esteem and life of the victim.Although it is not at al contagious, it definitely an ugly disease that can alenate coworkers and acquitances.

Arthritis can sometimes stem from psoriasis, attacking the joint spaces,giving the victim another disease to deal with.This disease can be consuming.The ugliness of the patches,the chronic eatching and flaking(although not life threatening) impact the self-esteem and life style of the victim.Time and money are spent to keep it under control.

Treatment Advances Improve Outlook
With the emergence of several new therapies, including the biologic agents, more people are experiencing substantial improvements and reporting a greatly improved quality of life.

Topical treatment

Bath solutions and moisturizers help sooth affected skin and reduce the dryness which accompanies the build-up of skin on psoriasis plaques. Medicated creams and ointments applied directly onto psoriasis plaques can help reduce inflammation, remove built-up scale, reduce skin turn over, and clear affected skin of plaques. Ointment and creams containing coal tar, anthralin, corticosteroids, vitamin D3 analogues (for example, calcipotriol), and retinoids are routinely used. The mechanism of action of each is probably different but they all help to normalise skin cell production and reduce inflammation.

The disadvantages of topical agents are variabily that they can often irritate normal skin, can be awkward to apply, cannot be used for long periods, can stain clothing or have a strong odour. As a result, it is sometimes difficult for people to maintain the regular application of these medications. Abrupt withdrawal of some topical agents, particularly corticosteroids, can cause an aggressive recurrence of the condition. This is known as a rebound of the condition.

Some topical agents are used in conjunction with other therapies, especially phototherapy.

Phototherapy

It has long been recognised that daily, short, nonburning exposure to sunlight helped to clear or improve psoriasis. Niels Finsen was the first physician to investigate the theraputic effects of sunlight scientifically and to use sunlight in clinical practice. This became known as phototherapy.

Sunlight contains many different wavelengths of light. It was during the early part of the 20th century that it was recognised that for psoriasis the therapeutic property of sunlight was due to the wavelengths classified as ultraviolet (UV) light.

Ultraviolet wavelengths are subdivided into UVA (380–315 nm), UVB (315–280 nm), and UVC (< 280 nm). Ultraviolet B (UVB) (315–280 nm) is absorbed by the epidermis and has a beneficial effect on psoriasis. Narrowband UVB (311 to 312 nm), is that part of the UVB spectrum that is most helpful for psoriasis. Exposure to UVB several times per week, over several weeks can help people attain a remission from psoriasis.

Ultraviolet light treatment is frequently combined with topical (coal tar, calcipotriol) or systemic treatment (retinoids) as there is a synergy in their combination. The Ingram regime, involves UVB and the application of anthralin paste. The Goeckerman regime, combines coal tar ointment with UVB.

A form of phototherapy called Grenz Rays (also called ultrasoft X-rays or Bucky rays) was a popular treatment of psoriasis during the middle of the 20th century. This type of therapy was superseded by ultraviolet therapy and is no longer commonly used.

Photochemotherapy

Psoralen and ultraviolet A phototherapy (PUVA) combines the oral or topical administration of psoralen with exposure to ultraviolet A (UVA) light. Precisely how PUVA works is not known. The mechanism of action probably involves activation of psoralen by UVA light which inhibits the abnormally rapid production of the cells in psoriatic skin. There are multiple mechanisms of action associated with PUVA, including effects on the skin immune system.

Dark glasses must be worn during PUVA treatment because there is a risk of cataracts developing from exposure to sunlight. PUVA is associated with nausea, headache, fatigue, burning, and itching. Long-term treatment is associated with squamous-cell and melanoma skin cancers.

Systemic treatment

Psoriasis which is resistant to topical treatment and phototherapy is treated by medications that are taken internally by pill or injection. This is called systemic treatment. Patients undergoing systemic treatment are required to have regular blood and liver function tests because of the toxicity of the medication. Pregnancy must be avoided for the majority of these treatments. Most people experience a recurrence of psoriasis after systemic treatment is discontinued.

The three main traditional systemic treatments are the immunosupressant drugs methotrexate and ciclosporin, and retinoids, which are a synthetic forms of vitamin A. Other additional drugs, not specifically licensed for psoriasis, have been found to be effective. These include the antimetabolite tioguanine, the cytotoxic agent hydroxyurea, sulfasalazine, the immunosupressants mycophenolate mofetil, azathioprine and oral tacrolimus. These have all been used effectively to treat psoriasis when other treatments have failed. Although not licensed in many other countries fumaric acid esters have also been used to treat severe psoriasis in Germany for over 20 years.

Biologics[4] are the newest class of systemic treatment for psoriasis. These are manufactured proteins that interrupt the immune process involved in psoriasis. Unlike generalised immunosuppressant therapies such as methotrexate, biologics focus on specific aspects of the immune function leading to psoriasis. These drugs are relatively new, and their long-term impact on immune function is unknown. Examples include Amevive®, etanercept (Enbrel®), Humira®, infliximab (Remicade®) and Raptiva.

Alternative Therapy

  • Antibiotics are not indicated in routine treatment of psoriasis. However, antibiotics may be employed when an infection, such as that caused by the bacteria Streptococcus, triggers an outbreak of psoriasis, as in certain cases of guttate psoriasis.
  • Climatotherapy involves the notion that some diseases can be successfully treated by living in particular climate. Several psoriasis clinics are located throughout the world based on this idea. The Dead Sea is one of the most popular locations for this type of treatment.
  • In Turkey, doctor fish which live in the outdoor pools of spas, are encouraged to feed on the psoriatic skin of people with psoriasis. The fish only consume the affected areas of the skin. The outdoor location of the spa may also have a beneficial effect. This treatment can provide temporary relief of symptoms. A revisit to the spas every few months is often required.
  • Some people subscribe to the view that psoriasis can be effectively managed through a healthy lifestyle. This view is based on anecdote, and has not been subjected to formal scientific evaluation. Nevertheless, some people report that minimizing stress and consuming a healthy diet, combined with rest, sunshine and swimming in saltwater keep lesions to a minimum. This type of “lifestyle” treatment is suggested as a long-term management strategy, rather than an initial treatment of severe psoriasis.
  • Some psoriasis patients use herbology as a holistic approach that aims to treat the underlying causes of psoriasis.
  • A psychological symptom management programme has been reported as being a helpful adjunct to traditional therapies in the management of psoriasis.
  • It is possible that Epsom salt may have a positive effect in reducing the effects of psoriasis.

Future Drug Development

Historically, agents used to treat psoriasis were discovered by experimentation or by accident. In contrast, current novel therapeutic agents are designed from a better understanding of the immune processes involved in psoriasis and by the specific targeting of molecular mediators. Examples can be seen in the use of biologics which target T cells and TNF inhibitors. Future innovation should see the creation of additional drugs that refine the targeting of immune-mediators further.

Research into antisense oligonucleotides is in its infancy but carries the potential to provide novel theraputic strategies for treating psoriasis.

Prognosis

Psoriasis is a chronic lifelong condition. There is currently no cure but various treatments can help to control the symptoms. Many of the most effective agents used to treat severe psoriasis carry an increased risk of significant morbidity including skin cancers, lymphoma and liver disease. However, the majority of people’s experience of psoriasis is that of minor localised patches, particularly on the elbows and knees, which can be treated with topical medication. Psoriasis does get worse over time but it is not possible to predict who will go on to develop extensive psoriasis or those in whom the disease may appear to vanish. Individuals will often experience flares and remissions throughout their life. Controlling the signs and symptoms typically requires lifelong therapy.

“The heartbreak of psoriasis”

The phrase “the heartbreak of psoriasis” is often used both seriously and ironically to describe the emotional impact of the disease. It can be found in various advertisements for topical and other treatments; conversely, it has been used to mock the tendency of advertisers to exaggerate (or even fabricate) aspects of a malady for financial gain. (In Bloom County, the character of Opus once considered the possibility of his suffering from “the heartbreak of nose hemorrhoids.”) While many products today use the phrase in their advertising, it originated in a 1960s advertising campaign for Tegrin, a coal tar-based medicated soap.

Haemorrhoids are very common. They are said to be more common in countries where the diet has traditionally been more processed and low in fibre. The main contributory causes are those things that cause us to raise the pressure in the abdomen. This causes the blood vessels to swell and become engorged.

Symptoms:
Irritation while passing stools, Bleeding, Itching and pain in the rectum region, wind formation in the stomach and sitting becomes a problem.

Most common causes:

Heredity, Pregnancy, Obesity, Chronic constipation, Strainus work, Aging, Bowel disorder(IBS),prolonged periods of sitting and standing , mental tension, and sometimes Anal intercourse.

It seems that the people most at risk of developing haemorrhoids are those who have more causes for raised abdominal pressure

Treatment:

It is usually best, with haemorrhoids, to get by with the least treatment possible, as even after the most extensive treatments they may still return.

  • Many times they will settle down over a matter of days without any treatment.
  • Cream or suppositories (bullet-shaped tablets to be inserted into the anus) may be bought over the counter, or your doctor may prescribe one. These soothe itching and pain, and cause swelling and bleeding to diminish.
  • Cold compresses, even ice can be helpful.

If you do not get better with these approaches, your doctor may ask a specialist to see you who may:

  • Put little rubber bands round the haemorrhoids, which will cause them to shrivel and wither away.
  • Inject a substance into the haemorrhoids which causes them to wither away (sclerotherapy).
  • Cut away the problem, usually under a general anaesthetic.

Some Ayurvedic Home Remedies:

  • Radish juice should be taken twice a day, once in the morning and then later in the night. Initially drink about ¼ cup of radish juice and then gradually increase it to ½ cup.
  • Soak 3-4 figs in a glass of water. Keep it overnight. Consume the figs on an empty stomach, the next day in the morning.
  • Roast 1 tsp of black cumin seeds and mix with 1 tsp of unroasted black cumin seeds and powder them together. Consume a half tsp of this powder with a glass of water, once in a day.
  • Prevention: Haemorrhoids are very common, and will occur anyway, but, as implied above, useful aspects of prevention are:
  • Avoid becoming overweight, and lose weight if you are obase.
  • Eat a high fibre diet.
  • Exercise regularly.

Definition:    A cough is our body’s way of responding to irritants in our throat and airways. An irritant stimulates nerves there to send a cough impulse to the brain. The brain signals the muscles of our abdomen and chest wall to give a strong push of air to our lungs to try to expel the irritant.

It is a sudden and often repetitively occurring reflex which helps to clear the large breathing passages from secretions, irritants, foreign particles and microbes. The cough reflex consists of three phases: an inhalation, a forced exhalation against a closed glottis, and a violent release of air from the lungs following opening of the glottis, usually accompanied by a distinctive sound. Coughing is either voluntary or involuntary.

A coughing attack can be very forceful — the velocity of air from a vigorous cough through the nearly closed vocal cords can approach 500 miles per hour. Prolonged, vigorous coughing is exhausting and can cause sleeplessness, headaches, urinary incontinence, and even broken ribs.
Frequent coughing usually indicates the presence of a disease. Many viruses and bacteria benefit evolutionarily by causing the host to cough, which helps to spread the disease to new hosts. Most of the time, irregular coughing is caused by a respiratory tract infection but can also be triggered by choking, smoking, air pollution, asthma, gastroesophageal reflux disease, post-nasal drip, chronic bronchitis, lung tumors, heart failure and medications such as ACE inhibitors.

An occasional cough is normal and healthy. A cough that persists for several weeks or one that brings up discolored or bloody mucus may indicate an underlying condition that requires medical attention. A cough rarely requires emergency care.

Cough is a helpful phenomenon, it is the body’s natural reflex to irritation in the throat. There are two kinds of cough, one which is wet and associated with mucus (productive cough); and the other which is dry and there is no mucus (unproductive cough). Causes:  The list of possible causes of cough is long and highly varied. Doctors classify coughs into 2 categories, acute and chronic. Many doctors define an acute cough as one that been present for less than 3 weeks. Chronic coughs are those present for more than 3 weeks.

  • Acute coughs can be divided into infectious (caused by an infection) and noninfectious causes.
  • Noninfectious causes of cough include flare-ups of the following chronic conditions: chronic bronchitis, emphysema, asthma, and environmental allergies.
  • The easiest way to simplify the causes of chronic cough is to divide them into their locations with respect to the lungs. The categories are environmental irritants, conditions within the lungs, conditions along the passages that transmit air from the lungs to the environment, conditions within the chest cavity but outside of the lungs, and digestive causes.
  • Any environmental substance that irritates the air passages or the lungs is capable of producing a chronic cough with continued exposure. Cigarette smoke is the most common cause of chronic cough. Other cough-producing irritants include dusts, pollens, pet dander, particulate matter, industrial chemicals and pollution, cigar and pipe smoke, and low environmental humidity.
  • Within the lungs both common and uncommon conditions cause chronic cough. Common causes include asthma, emphysema, and chronic bronchitis. Less common causes of lung-induced chronic cough include cancer, sarcoidosis, diseases of the lung tissue, and congestive heart failure with chronic fluid build-up in the lungs.
  • The passages that connect the lungs to the external environment are known as the upper respiratory tract. Chronic sinus infections, chronic postnasal drip, diseases of the external ear, infections of the throat, and use of ACE inhibitors for high blood pressure have all been implicated in chronic cough.
  • In addition to disease processes within the lung and air passages, diseases elsewhere within the chest cavity may also be responsible for chronic cough. Conditions within the chest known to cause chronic cough include cancer, unusual growth of a lymph node, and an abnormal enlargement of the aorta, which is the main blood vessel leaving the heart.
  • An often-overlooked cause of the chronic cough is gastroesophageal reflux (GERD). GERD occurs when acid from the stomach travels up the esophagus. This abnormal condition can cause irritation of the esophagus and larynx resulting in the reflex production of a cough.

In the case of productive cough, coughing helps in removing excess mucus or phlegm which has come up in the back of the throat from the lungs or the nose and sinuses. A common cause of coughing in the night is mucus dripping from the nose into the back of the throat. Expectorant syrups , specially herbal base cough syrup can be used to get rid of the phlegm. Note that productive coughs should never be suppressed since they serve a very useful purpose. Night-time irritation of a chield can be eased by turning the child on his side or his front. A doctor should be consulted in case the cough lasts for more than 48 hours.

Unproductive coughs on the other hand serve no useful purpose, and can be very irritating to a small child, sometimes to the extent of preventing sleep. In this case, cough suppressants can help ease the discomfort.

Complications:   The complications of coughing can be classified as either acute or chronic. Acute complications include cough syncope (fainting spells due to decreased blood flow to the brain when coughs are prolonged and forceful), insomnia, cough-induced vomiting, rupture of blebs causing spontaneous pneumothorax (although this still remains to be proven), subconjunctival hemorrhage or “red eye”, coughing defecation and in women with a prolapsed uterus, cough urination. Chronic complications are common and include abdominal or pelvic hernias, fatigue fractures of lower ribs and costochondritis.

Diagnosis:  The diagnose of the cause of a cough usually begins by determining if it is specific or nonspecific in nature. A specific cough is one associated with other symptoms and further workup is dependent on these symptoms while a non specific cough occurs without other signs and symptoms. Further workup may include labs, x rays, and spirometry.
Treatment:  Treatment should target the cause as per diagnosis; for example, smoking cessation or discontinuing ACE inhibitors. Cough suppressants such as codeine or dextromethorphan are frequently prescribed, but have been demonstrated to have little effect. Other treatment options may target airway inflammation or may promote mucus expectoration. As it is a natural protective reflex, suppressing the cough reflex might have damaging effects, especially if the cough is productive.

In children half of cases go away without treatment in 10 days and 90% in 25 days.

According to the American Academy of Pediatrics the use of cough medicine to relieve cough symptoms is supported by little evidence and thus not recommended for treating cough symptoms in children. There is tentative evidence that the use of honey is better than no treatment or diphenhydramine in decreasing coughing. It appeared similar to dextromethorphan. A trial of antibiotics or inhaled corticosteroids may be tried in children with a chronic cough in an attempt to treat protracted bacterial bronchitis or asthma respectively.
Home Care for prevention :
If one has asthma or another chronic lung disease, he or she should make sure to taking medicines prescribed by  the doctor.

Some helpful tips to ease cough:

*If one has a dry, tickling cough, try cough drops or hard candy. Never give these to a child under age 3, because they can cause choking.
*Use a vaporizer or take a steamy shower. These increase moisture in the air and help soothe a dry throat.
*Drink plenty of fluids. Liquids help thin the mucus in your throat making it easier to cough it up.
*Do not smoke and stay away from secondhand smoke

Medicines can be bought on own include:

*Guaifenesin helps break up mucus. Drink lots of fluids if you take this medicine.
*Decongestants help clear a runny nose and relieve postnasal drip. Check with your doctor before taking decongestants if you have high blood pressure.
* In case of children the doctor should be consulted before giving a child 6 years old or younger an over-the-counter cough medicine, even if it is labeled for children. These medicines likely do not work for children, and can have serious side effects.

If one has seasonal allergies, such as hay fever:
*Stay indoors during days or times of the day (usually the morning) when airborne allergens are high.
*Keep windows closed and use an air conditioner.
*Do not use fans that draw in air from outdoors.
*Shower and change clothes after being outside.
If one has allergies year-round, cover pillows and mattress with dust mite covers, use an air purifier, and avoid pets and other triggers.
