Kidney dialysis

Introduction:
In order for blood to perform its essential functions of bringing nutrients and oxygen to the cells of the body, and carrying waste materials away from those cells, the chemical composition of the blood must be carefully controlled. Blood contains particles of many different sizes and types, including cells, proteins, dissolved ions, and organic waste products. Some of these particles, such as proteins like hemoglobin, are essential for the body. Others, such as urea (a waste product from protein metabolism), must be removed from the blood or they will accumulate and interfere with normal metabolic processes. Still other particles, including many of the simple ions dissolved in the blood, are required by the body in certain concentrations that must be tightly regulated, especially when the intake of these chemicals varies. The body has many different means of controlling the chemical composition of the blood. For instance, you learned in the “Iron Use and Storage in the Body: Ferritin and Molecular Representations” tutorial that the ferritin protein can help to control the amount of free iron in the blood. As you will discover in the tutorial entitled, “Blood, Sweat, and Buffers: pH Regulation During Exercise”, buffers dissolved in the blood can help regulate the blood’s pH. But the largest responsibility for maintaining the chemistry of the blood falls to the kidneys, a pair of organs located just behind the lining of the abdominal cavity. It is the job of the kidneys to remove the harmful particles from the blood and to regulate the blood’s ionic concentrations, while keeping the essential particles in the blood

Healthy kidneys clean the blood by removing excess fluid, salt and wastes. When they fail, harmful wastes build up, blood pressure may rise, and the body may retain excess fluid. When this happens, treatment – dialysis or a kidney transplant – is needed to replace the work of the failed kidneys, which is known as end-stage renal failure (ESRF).

There are three primary and two secondary types of dialysis: hemodialysis (primary), peritoneal dialysis (primary), hemofiltration (primary), hemodiafiltration (secondary), and intestinal dialysis (secondary).

Hemodialysis:
Haemodialysis (HD) is the most common method used to treat ESRF and has been available since the 1960s. Despite some advances in dialysis machines in recent years, HD is still a complicated and inconvenient therapy requiring a coordinated effort from a large healthcare team, including:

One important step before starting HD is a small operation to prepare a site on the body. One of the arteries in your arm is re-routed to join a vein, forming a fistula. Blood is removed from the fistula, cleaned and returned to it, allowing dialysis process to take place.

Needles are inserted into a fistula (the point of access to the bloodstream) at the start of HD. You may find this one of the hardest parts, although most people report getting used to them after a few sessions. If it’s painful, an anesthetic cream or spray can be applied to the skin.

In HD, blood is allowed to flow, a small amount at a time, through a special filter (the ‘dialyser’ or ‘artificial kidney’) that removes wastes and extra fluids. The clean blood is then returned to your body via the fistula. This helps to keep the correct amount of water in the body, control blood pressure – and keep the proper balance of chemicals such as potassium, sodium and acid.

Most people have HD three times a week for three to five hours, with a morning, afternoon or evening ‘slot’; depending on availability and capacity at a dialysis unit, usually in a large hospital. Some receive it at a smaller satellite unit nearer home, and a few have HD in their own homes.

By learning about the treatment, and working with your healthcare team, it’s possible to have a full, active life

Peritoneal dialysis:
Peritoneal dialysis (PD) became an alternative to HD in the 1980s, with many preferring the independence it brings them.

It means you don’t have to have dialysis sessions at a unit, but can give treatments at home, at work or on holiday. Like HD, by learning about the treatment, and working with the medical team, it’s possible to have a full and active life.

In PD, a soft tube called a catheter is used to fill the abdomen with a cleansing liquid called dialysis solution. The abdominal cavity is lined with a layer called the peritoneum. Waste products and extra fluid (and salt) then pass through the peritoneum from the blood into the dialysis solution. They then leave the body when the dialysis solution is drained. This used solution is then thrown away.

The process of draining and filling is called an ‘exchange’ and takes about 30 to 40 minutes. The period the dialysis solution is in the abdomen is called the ‘dwell time’. A typical schedule is four exchanges a day, each with a dwell time of four to eight hours.

There are many forms of PD. One doesn’t even require a machine and it’s possible to walk around with the dialysis solution in your abdomen. Talk to your specialist about what’s best for your particular situation.

Whatever form is chosen, an operation is needed to have the soft catheter placed in the abdomen, which will carry the dialysis solution in and out of the abdomen. It’s usually inserted two weeks before dialysis proceeds, to allow scar tissue to build up that will hold it in place.

Hemofiltration:
Hemofiltration is a similar treatment to hemodialysis, but it makes use of a different principle. The blood is pumped through a dialyzer or “hemofilter” as in dialysis, but no dialysate is used. A pressure gradient is applied; as a result, water moves across the very permeable membrane rapidly, “dragging” along with it many dissolved substances, importantly ones with large molecular weights, which are cleared less well by hemodialysis. Salts and water lost from the blood during this process are replaced with a “substitution fluid” that is infused into the extracorporeal circuit during the treatment. Hemodiafiltration is a term used to describe several methods of combining hemodialysis and hemofiltration in one process.

Hemodiafiltration:
Hemodialfiltration is a combination of hemodialysis and hemofiltration. In theory, this technique offers the advantages of both hemodialysis and hemofiltration.

Intestinal dialysis:
In intestinal dialysis, the diet is supplemented with soluble fibres such as acacia fibre, which is digested by bacteria in the colon. This bacterial growth increases the amount of nitrogen that is eliminated in fecal waste.  An alternative approach utilizes the ingestion of 1 to 1.5 liters of non-absorbable solutions of polyethylene glycol or mannitol every fourth hour.

Which is better?
Neither technique ‘cures’ ESRF, as they only provide about five per cent of normal kidney function. In other words, they control kidney failure to an extent. It’s hard to state which technique is ‘better’ for which patient, as both have pros and cons. Many patients will have both in their continuing treatment.

Living with dialysis
Adjusting to the effects of ESRF and the time spent on dialysis can be difficult. Aside from the ‘lost time’ (dialysis can take six to eight hours a day) most patients feel they have less energy. Many need to make changes in their work or home life, and can feel depressed when starting the process, or after several months of treatment. It’s good to talk with a social worker, nurse or doctor as this is a common problem that can often be treated effectively.

If you’re feeling well, your kidney specialist should measure the effectiveness of the dialysis with blood tests at least once a month in HD, and once every three months in PD.

Description:
It is a fall flowering evergreen perennial plant of the genus Baccharis which is commonly found in the southeastern United States, although it may be found as far north as Maine. It is typically found in coastal plains and wet areas. It is dioecious — male and female flowers are found on separate plants. Shrub growing to 3.5m by 3.5m at a medium rate.

It is hardy to zone 5. It is in flower in October. The flowers are dioecious (individual flowers are either male or female, but only one sex is to be found on any one plant so both male and female plants must be grown if seed is required) and are pollinated by Insects. The plant is not self-fertile.
The plant prefers light (sandy), medium (loamy) and heavy (clay) soils, requires well-drained soil and can grow in nutritionally poor soil. The plant prefers acid, neutral and basic (alkaline) soils and can grow in very alkaline and saline soils. It cannot grow in the shade. It requires dry or moist soil. The plant can tolerate maritime exposure.

Cultivation :
Succeeds in a sunny position in any well-drained soil, from heavy clays to pure sands. Tolerates saline conditions and dry soils. A useful shrub for coastal situations, resisting maritime exposure. Plants are hardy to about -15°c. A fast-growing plant, it is very tolerant of pruning and can be cut right back to the base if required. The presence of this plant growing wild was supposed to indicate areas where oil might be found. Dioecious. Male and female plants must be grown if seed is required.

Propagation:
Seed – no pre-treatment is required. Surface sow in pots a cold frame in the spring, do not let the compost dry out. Germination usually takes place within 1 – 2 weeks. When they are large enough to handle, prick the seedlings out into individual pots and grow them on in the cold frame for at least their first winter. Plant them out into their permanent positions in late spring or early summer, after the last expected frosts. Cuttings of half-ripe wood, July/August in a frame. Very easy. Cuttings of mature wood of the current seasons growth, November in a frame. Easy

Medicinal Actions & Uses
Demulcent.
The plant is used as a palliative and demulcent in consumption and cough.

Other Uses:
Fuel; Hedge; Soil stabilization.
A good fast-growing hedge for exposed maritime conditions. It retains its leaves into the new year but is rather bare in late winter. Plants have an extensive root system and can be grown on sand or thin coastal soils in order to bind the soil. Resinous secretions on the leaves and wood make this a useful fuel. It is a fairly small plant though and would not be a very productive source.

Known Hazards: The plant is potentially toxic to livestck.

White Snakeroot

Description:
It is a poisonous perennial herb in the family Asteraceae, native to eastern North America. An older binomial name for this species was Eupatorium rugosum, but the genus Eupatorium has undergone taxonomic revision by botanists and a number of the species once included there have been moved to other genera.

This  perennial plant is about 1½–3′ tall, branching occasionally. The light green to tan stems are round and largely hairless. The opposite leaves are up to 6″ long and 3½” across, becoming smaller as they ascend the stems. The lower leaves are cordate to cordate-ovate, while the upper leaves are broadly lanceolate to lanceolate. All of the leaves are largely hairless and strongly serrated along the margins. There are 3 prominent veins on the upper surface of each leaf (particularly the lower ones), while the lower surface has an elevated network of veins. The rather long petioles are ½–2½” in length.

The upper stems terminate in compound corymbs of flowerheads that span several inches across. Each flowerhead is about ½” across and contains 10-30 disk florets that are brilliant white. There are no ray florets. Each disk floret is about 1/5″ across when fully open; it consists of a small tubular corolla with 5 lobes that are spreading and pointed, and it has a divided style that is strongly exerted from the corolla. At the base of each flowerhead, there is a single series of linear floral bracts that are green and non-overlapping. The blooming period occurs from late summer through the fall and lasts about 2 months. This is one of the last wildflowers to bloom during the fall. The flowers are often fragrant. Each disk floret is replaced by a dark linear achene with a small tuft of white hairs. These achenes are distributed by the wind. The root system consists of spreading rhizomes and shallow fibrous roots. This plant can spread vegetatively by means of its rhizome, or it can reseed itself into new areas.

They are found in woods and brush thickets where they bloom mid to late summer or fall. The flowers are a clean white color and after blooming small seeds with fluffy white tails are released to blow in the wind. This species is adaptive to different growing conditions and can be found in open shady areas with open bare ground; it can be weedy in shady landscapes and in hedgerows. There are two different varieties Ageratina altissima var. angustata and Ageratina altissima var. roanensis (Appalachian white snakeroot); they differ in the length of the flower phyllaries and shape of the apices

It is hardy to zone 6. It is in flower from August to September. The flowers are hermaphrodite (have both male and female organs) and are pollinated by Insects. The plant is self-fertile.

The plant prefers light (sandy), medium (loamy) and heavy (clay) soils and requires well-drained soil. The plant prefers acid, neutral and basic (alkaline) soils. It can grow in semi-shade (light woodland) or no shade. It requires moist soil.

Cultivation
Succeeds in an ordinary well-drained but moisture retentive garden soil in sun or part shade. There is some difference of opinion over the correct name for this species with some authorities using Eupatorium rugosum.

Propagation
Seed – sow spring in a cold frame, only just covering the seed. When large enough to handle, prick the seedlings out into individual pots and plant them out in the summer. Division in spring.

Medicinal  Actions & Uses
Diaphoretic; Diuretic; Febrifuge; Odontalgic; Stimulant; Tonic.

The root is diaphoretic, diuretic, febrifuge, stimulant and tonic. It has been used in the treatment of diarrhoea, gravel and urinary diseases. It has also been used in herbal sweat baths to encourage sweating. A decoction or infusion of the root has been taken to treat a fallen or inflamed womb. The root has been chewed and held in the mouth as a treatment for toothache.

Known Hazards : .
White Snakeroot contains the toxin tremetol; when the plants are consumed by cattle, the meat and milk become contaminated with the toxin. When milk or meat containing the toxin is consumed, the poison is passed onto humans, and if consumed in large enough quantities can cause tremetol poisoning in humans. The poisoning is also called milk sickness, as humans often ingested the toxin by drinking the milk of cows who had eaten snakeroot. During the early 19th century, when large numbers of Europeans (who were unfamiliar with snakeroot) began settling in the plant’s habitat of the Midwest and Upper South, many thousands were killed by milk sickness, and it was several decades before the cause was traced to snakeroot. Notably, it was the cause of death of Nancy Hanks, mother of Abraham Lincoln. The plants are also poisonous to horses, goats, and sheep. Signs of poisoning in these animals include depression and lethargy, hind feet placed close together (horses, goats, cattle) or held far apart (sheep), nasal discharge, excessive salivation, arched body posture, and rapid or difficult breathing.

Description :
Dwarf wild indigo is a smooth, much-branched decidious perennial from a woody rootcrown. Plants are usually about 12-18 inches tall. Leaves are found on the upper half of the plant and are about two inches long. Each leaf is comprised of 8-15 pairs of small, oval leaflets arranged along a midrib. Leaves alternate, compound (odd-pinnate) to 5-8 cm long, 25-31 leaflets, each 7-11 mm long and 2-4 mm wide, narrow to broadly oblong, tip rounded, emarginate or mucronate (sharp pointed).  Flowers perfect, small, purplish, one petal, in terminal spike-like clusters (racemes), 3.5-4.5 cm long.  Fruit a small reddish-brown pod, 4-5 mm long, one seed.Several dozen violet-to-purple flowers are crowded into spikes about one half inch wide that form in the upper branches. The tiny pods (legumes) are smooth and glandular.

Look for dwarf wild indigo during late June in native prairie on gentle slopes at middle or low elevations. Plants are usually more abundant where grazing is light or moderate.
It is hardy to zone 4. It is in flower from July to August. The flowers are hermaphrodite (have both male and female organs)It can fix Nitrogen.

Cultivation :
Prefers a light well-drained sandy soil in sun or light shade. Fairly wind-resistant. A very hardy plant, tolerating temperatures down to about -25c. Plants resent root disturbance, they should be planted out into their final positions whilst small. Plants are said to be immune to insect pests. Flowers are produced on the current season’s growth. This species has a symbiotic relationship with certain soil bacteria, these bacteria form nodules on the roots and fix atmospheric nitrogen. Some of this nitrogen is utilized by the growing plant but some can also be used by other plants growing nearby.

Propagation:
Seed – presoak for 12 hours in warm water and sow early spring in a greenhouse. The seed usually germinates in 1 – 2 months at 20°c. When large enough to handle, prick the seedlings out into individual pots and grow them on in the greenhouse for their first winter. Plant them out in late spring or early summer, after the last expected frosts. Cuttings of half-ripe wood, June/July in a frame. High percentage. Cuttings of mature wood of the current seasons growth, autumn, in a sheltered position outdoors. Takes 12 months. Suckers in spring just before new growth begins. Layering in spring .

Medicinal Uses:
Expectorant.
The plant has been used as a snuff in the treatment of catarrh.

Other Uses
Insecticide; Soil stabilization.

The resinous pustules on some species yield the insecticide ‘amorpha’. The plant has a strong spreading root system and this makes it useful for controlling soil erosion.

Nicotiana Sylvestris (Flowering tobacco)

Description:
This imposing half hardy annual seed has stout spikes bearing a cascading head of fragrant 3½in long white blooms. It grows to a height of 90-120cm (36-48in) making it an excellent subject for the border. Occasionally a short lived perennial in a warm sheltered site. Flowers summer to autumn. Height 90-120cm (36-48in).

The leaves are simple, with the blade partially surrounding the stem. Flowers are tubular, white, borne in large clusters above the foliage.

This plant is thought to be one of the parents of Nicotiana tabacum, the basis of most modern tobacco production. It is usually combined with Nicotiana rustica to lower the strength of N. rustica’s tobacco.

It can be grown from seeds.When the leaves first form in spring they are a beautiful clean green colour, with patterns in the veins highlighted in the sun.

Later the flowers form as skinny long white trumpets and at dusk they perfume the air. Hopefully there are no strong winds to blow them down!Their seed pods start to form from January on, along with new flowers. The leaves retain their clean green appearance, though they get quite sticky, trapping flying insects. Apparently the leaves resemble commercial tobacco, to which Nicotiana Sylvestris is related.

Medicinal Uses:
A member of a large family of Nicotianas whose leaves are used in making prepartions taken by mouth to induce vomiting and diarrhea, to relieve pain and to sedate. Preparations are used externally as a poultice in the treatment of joint swelling from arthritis, of skin diseases and of insect bites. Nicotine is a very effective biodegradable insecticide.
