Attention deficit hyperactive disorder or ADHD is erroneously considered to be a 20th century phenomenon affecting mainly children from developed nations. Actually, it was first described in 1845 by a psychiatrist in a boy called “fidgety Philip”. Today, the worldwide incidence is 3-5 per cent, irrespective of nationality. In referral paediatric clinics, it is as high as 15.5 per cent. The average age at diagnosis is eight years with a 6:1 male-to-female ratio.

Children who have the hyperactive impulsive type of ADHD are unbearably fidgety, restless and impatient, always running, jumping, climbing and blurting out inappropriate comments. They often receive corporal punishment from frustrated parents and teachers. Others, with the inattentive type of ADHD, are dreamy and bored, with difficulty in paying attention, learning something new or completing a task. Homework becomes a particular problem, with assignments forgotten, books misplaced and the final unsatisfactorily completed task full of erasures and errors.

Around 40 per cent of children with ADHD (especially boys) are argumentative, defiant, stubborn, non-compliant and belligerent. They lie, steal, fight, bully others, have temper tantrums and engage in vandalism. Eventually, as teenagers, they may gravitate towards drugs and alcohol.

To make a diagnosis of ADHD:

* The symptoms should have set in before the age of seven years and have lasted for at least six months

* They should cause difficulties in the child’s life, in school, at play, at home, in the community and in social settings

* The changes should not have been precipitated by a sudden traumatic event like the death of a parent

* There should be no diagnosed medical ailments like seizures, middle ear infections or a learning disability to explain the symptoms.

Society often finds fault with the parents of children with ADHD. They are criticised for faulty nurturing and lack of parental discipline. But parents are actually helpless, as ADHD has a genetic and neurobiologic basis. Scans have shown that the frontal lobes, temporal grey matter, caudate nucleus and cerebellum of the brains of these children are 34 per cent smaller than normal in volume. Also, the brain has lower levels of a signal-processing chemical called dopamine.

The exact reason for these changes is not known. However,

* ADHD runs in families. About 25 per cent of the close relatives of ADHD children also have similar disorders as opposed to 5 per cent in the general population

* Women who smoke and drink during pregnancy have a higher incidence of children with ADHD

* High blood lead levels have been demonstrated in some children with ADHD. This, however, is not a consistent finding

* A sugar high has been blamed for some of the symptoms. This is a label for the increased level of activity following the ingestion of highly refined sugars or carbohydrates, which enter the bloodstream rapidly and produce fluctuations in blood glucose levels. This is particularly true if (as in the case of cola drinks) the food also contains caffeine (a stimulant) and food additives. Diet restrictions reducing the quantities of such food help in some cases.

Children with ADHD hate change in any form. They need a scheduled, regimented life with the same routine  every day. All their belongings should also be organised and kept in specific places. With structured care, these children show a great deal of improvement and are able to integrate into society. About 30 to 70 per cent of children with ADHD continue to have symptoms in adult life. Academic excellence — a prelude to higher studies and a good job — may remain elusive. This is aggravated by poor social skills. They remain easily distracted, hyperactive and impulsive and have difficulty with deadlines, prioritisation and social engagements. Decision-making is an almost insurmountable hurdle. They also have problems holding down a steady job. Many are able to function on computers and are intelligent enough to do programming and other jobs which do not require social interaction. Around 80 per cent need to continue to live with parents or siblings.

Some children do not improve despite psychotherapy and a structured environment. They require medication with mental stimulants like methylphenidate and atmoxetine. They do well if they take their medication, which may need to be continued into adult life.

Competition is fierce in India, for education, jobs, promotions and success. Reservations and capitation fees are a way of life. In this scenario, parents may find it difficult to cope with a hyperactive, inattentive, disobedient and impulsive child who does not conform to social norms.

It is often difficult for the parents to accept that their child has ADHD. They feel depressed and guilty, even though it is not their fault. And despite all folklore to the contrary, an arranged marriage to an unsuspecting spouse does not cure the problem.

Lemon balm (Melissa officinalis), not to be confused with bee balm, Monarda species, is a perennial herb in the mint family Lamiaceae.
Other common names: Melissa, Balm, Balm Mint, Bee Balm, Blue Balm, Cure-all, Dropsy Plant, Garden Balm, Sweet Balm, Heart’s Delight

Part Used: Herb.
Habitat: A native of South Europe, especially in mountainous situations, but is naturalized in the south of England, and was introduced into our gardens at a very early period.


Description:

The root-stock is short, the stem square and branching, grows 1 to 2 feet high, and has at each joint pairs of broadly ovate or heart-shaped, crenate or toothed leaves which emit a fragrant lemon odour when bruised. They also have a distinct lemon taste. The flowers, white or yellowish, are in loose, small bunches from the axils of the leaves and bloom from June to October. The plant dies down in winter, but the root is perennial.
The genus Melissa is widely diffused, having representatives in Europe, Middle Asia and North America. The name is from the Greek word signifying ‘bee,’ indicative of the attraction the flowers have for those insects, on account of the honey they produce.

It grows to 70-150 cm tall. The leaves have a gentle lemon scent, related to mint. At the end of the summer, little white flowers full of nectar appear. These attract bees, hence the genus name Melissa (Greek for ‘honey bee‘). Its flavour comes from the terpenes citronellal, citronellol, citral, and geraniol.

Cultivation
Balm grows freely in any soil and can be propagated by seeds, cuttings or division of roots in spring or autumn. If in autumn, preferably not later than October, so that the offsets may be established before the frosts come on. The roots may be divided into small pieces, with three or four buds to each, and planted 2 feet apart in ordinary garden soil. The only culture required is to keep them clean from weeds and to cut off the decayed stalks in autumn, and then to stir the ground between the roots.
This herb can be easy to cultivate in United States Department of Agriculture Plant Hardiness Zones 4 to 9. In zone 4, it needs winter mulch and a well-drained sandy soil to survive. In zone 7, it can be harvested at least until the end of November. It is moderately shade-tolerant, much more so than most herbs. In dry climates, it grows best in partial shade.

Lemon Balm grows in clumps and spreads vegetatively as well as by seed. In mild temperate zones, the stems of the plant die off at the start of the winter, but shoot up again in spring. It can be easily grown from stem cuttings rooted in water, or from seeds. Under ideal conditions, it will seed itself prolifically and can become a nuisance in gardens.

Use in Food and drinks:
It is used as a flavouring in tisane and ice cream, but its most common use is to make herbal teas. It makes a particularly refreshing iced tea, especially when mixed with other herbs such as spearmint. It is also often paired with fruit dishes or candies.
Its use in cooking of different dishes is very much appreciated.


Medicinal uses:

Carminative, diaphoretic and febrifuge. It induces a mild perspiration and makes a pleasant and cooling tea for feverish patients in cases of catarrh and influenza. To make the tea, pour 1 pint of boiling water upon 1 oz. of herb, infuse 15 minutes, allow to cool, then strain and drink freely. If sugar and a little lemonpeel or juice be added it makes a refreshing summer drink.

New research shows that its polyphenols can help significantly in the treatment of herpes simplex and zoster infections. Two other secondary compounds of this plant, citral and citronellal, calm the central nervous system.

Balm is a useful herb, either alone or in combination with others. It is excellent in colds attended with fever, as it promotes perspiration .

Used with salt, it was formerly applied for the purpose of taking away wens, and had the reputation of cleansing sores and easing the pains of gout.

John Hussey, of Sydenham, who lived to the age of 116, breakfasted for fifty years on Balm tea sweetened with honey, and herb teas were the usual breakfasts of Llewelyn Prince of Glamorgan, who died in his 108th year. Carmelite water, of which Balm was the chief ingredient, was drunk daily by the Emperor Charles V.

Commercial oil of Balm is not a pure distillate, but is probably oil of Lemon distilled over Balm. The oil is used in perfumery.

Balm is frequently used as one of the ingredients of pot-pourri. Mrs. Bardswell, in The Herb Garden, mentions Balm as one of the bushy herbs that are invaluable for the permanence of their leaf-odours, which,
‘though ready when sought, do not force themselves upon us, but have to be coaxed out by touching, bruising or pressing. Balm with its delicious lemon scent, is by common consent one of the most sweetly smelling of all the herbs in the garden. Balm-wine was made of it and a tea which is good for feverish colds. The fresh leaves make better tea than the dry.’
The crushed leaves, when rubbed on the skin, are used as a repellant for mosquitos.

Lemon Balm is also used medicinally as a herbal tea, or in extract form. It is claimed to have antibacterial, antiviral properties, and it is also used as a mild sedative or calming agent. At least one study has found it to be effective at reducing stress, although the study’s authors call for further research. Its antibacterial properties have also been demonstrated scientifically, although they are markedly weaker than those from a number of other plants studied.

Lemon Balm has been used for thousands of years as an effective calmative that is good for all kinds of nervous problems, including tension headaches, migraines, neuralgia, hysteria, nervous tension, stress, anxiety, excitability, heart palpations (resulting from anxiety) and agitation. Frequently called “the calming herb,” it may be effective in treating Attention Deficit and Hyperactivity Disorders, easing such symptoms as inability to listen, fidgeting, inability to sustain attention and shifting from one incomplete task to another. It also appears to relax muscle tension without daytime drowsiness.

To prevent insomnia, Lemon Balm is used to calm and relax the nerves, and Lemon Balm is an old and particularly reliable treatment for relieving the feelings of melancholy and depression.

Lemon Balm is also effective in calming the digestive tract. It relieves dyspepsia, colic, gas, upset stomach, indigestion and stomach cramps (particularly when related to nervous tension).

In the treatment of Alzheimer’s disease, Lemon Balm shows great promise, because of the herb’s possible central nervous system acetylcholine receptor activity and antioxidant properties. It may even positively affect cognitive abilities, enhance memory and improve mental clarity.

Further demonstrating Lemon Balm’s calmative qualities, the herb has been used to relieve irritable bowel syndrome (IBS), stopping the spasms and relieving the pain and cramps associated with the disease. In addition, although it is strong enough to ease spasms, it is not so strong as to cause constipation.

Lemon Balm is an old folk remedy for treating feverish patients. It promotes perspiration and cools the body by breaking a fever. It is especially helpful in cases of colds and flu. Lemon Balm is also said to relieve bronchial catarrh and some forms of asthma.

In cases of hyperthyroidism, the flavonoids and polyphenolics found in Lemon Balm induce thryroid-regulating actions and have been known to block the attachment of thyroid cells by antibodies that cause Graves’ disease, a condition that results in hyperthyroidism and over-stimulation of the thyroid gland.

Lemon Balm is said to possess excellent antiviral properties. Its volatile oils have been known to destroy viruses in test tubes in as little as three hours, and this quality makes the herb especially helpful in combating cold sores and herpes virus infection. In addition, it is also thought to relieve the pain, itching, and sting of an outbreak. According to recent research, topical use of Lemon Balm speeds healing time of herpes simplex virus sores on the mouth.

It is valuable for brain and strengthens memory.It prevents brain fatigue, sharpens comprehension,counteracts depression and reverse the spirit.A cold infusion of the balm has a calming effect on the nurves.

The herb is useful in treating several other diseases. It is used in strengthening the gums and remove the bad taste from the mouth.Leaves and stems are considered useful in liver and heart diseases as also venomous insects bite.

Lemon balm essential oil is very popular in aromatherapy. The essential oil is commonly co-distilled with lemon oil, citronella oil, or other oils.

A clinical multicentric study in Germany offers evidence of the antiviral activity of a specially prepared dried extract of lemon balm against herpes simplex infections. The extract was a concentrated (70:1) dry extract of lemon balm which was included at a level of 1% in a cream base. Patients applied the cream 2-4 times daily for 5-10 days. In the group receiving the active Melissa cream, there was a significant improvement in symptoms on day two compared to the placebo group and on day five over 50% more patients were symptom-free than in the placebo group. To be effective, the treatment must be started in the very early stages of the infection.

Research has clearly demonstrated the plant’s ability to impact the limbic system of the brain and   protect   the brain from the powerful stimuli of the body and should be part of any ADHD formula.

Pediatric Use:

Lemon balm may be used topically in children to treat cold sores. The dosage would be the same as the recommendations for use in adults.

For internal use, adjust the recommended adult dose to account for the child’s weight. Most herbal dosages for adults are calculated on the basis of a 150 lb (70 kg) adult. Therefore, if the child weighs 50 lb (20 – 25 kg), the appropriate dose of lemon balm for this child would be 1/3 of the adult dosage.

Precautions
The use of herbs is a time-honored approach to strengthening the body and treating disease. Herbs, however, contain components that can trigger side effects and interact with other herbs, supplements, or medications. For these reasons, herbs should be taken with care, under the supervision of a health care provider qualified in the field of botanical medicine.

No side effects or symptoms of toxicity have been reported with lemon balm use, but this herb should not be used by pregnant or breast-feeding women.

Possible Interactions
Sedatives, Thyroid medications — Although not yet demonstrated in clinical studies, lemon balm may interfere with sedatives and thyroid medications. If you are taking sedatives (for sleep disorders or anxiety) or medications to regulate your thyroid, you should consult a health care provider before taking lemon balm.

Every medical journal on heart health brings, it seems, another study demonstrating the cardiovascular benefits of the oil — specifically, its omega-3 fatty acids, a type of polyunsaturated fat found in few foods other than fish and flaxseed. In recent years, scientists have discovered that inflammation within our arteries — triggered in response to damage done by plaque, high blood pressure, and free radicals — is a major cause of heart disease. What is meditation?
Meditation is the practice of focusing your attention to help you feel calm and give you a clear awareness about your life. Eastern philosophies have recognized the health benefits of meditation for thousands of years. Meditation is now widely practiced in the West, with the belief that it has positive effects on health.

Two meditation techniques are most commonly used:

What is meditation used for?

3.High blood pressure. A report from the National Institutes of Health (NIH) recommends meditation as one of the first treatments for high blood pressure.

6.Most of these conditions also require conventional treatment for best results.

People also use meditation to relieve anxieties from long-term (chronic) conditions such as HIV and cancer.

Is meditation safe?
Since meditation usually involves sitting quietly for a period of time and breathing deeply, anyone who cannot sit comfortably or who has respiratory problems may have difficulty practicing meditation. Some people with mental health problems, such as attention deficit hyperactivity disorder or schizophrenia, may not be able to use meditation therapy effectively.

Meditation is not thought to have any negative side effects or complications when combined with conventional medical treatment, but it is not considered appropriate or safe for acute, life-threatening situations.

Always tell your doctor if you are using an alternative therapy or if you are thinking about combining an alternative therapy with your conventional medical treatment. It may not be safe to forgo your conventional medical treatment and rely only on an alternative therapy.

Getting a Grip on the Winter Blues (SAD)

It is that time of year again, when despite the ratcheting up of festivities for the holidays, fully one person in five in the United States ratchets down. The cause is a now well-known but still infrequently treated disorder, winter blues or SAD, for seasonal affective disorder.

There are several remedies to help those affected by SAD escape an affliction that leaves many wanting to climb into bed, put their heads under the covers and not come out until spring. Indeed, some experts refer to SAD as a form of hibernation.

The problem typically starts gradually as the days become shorter in late summer or fall and peaks in midwinter in regions where there may be just 9 or 10 hours of daylight, if that.

For the estimated 14 million severely affected American adults, SAD can send them into a tailspin that makes it difficult if not impossible to fulfill daily responsibilities and derive any joy from life. An additional 33 million people are less severely affected but may experience declines in energy, cheerfulness, creativity or productivity in the dark days of winter.

The most commonly used treatment is exposure for up to several hours a day to high-intensity artificial light, in an effort to simulate the longer days of summer when people with SAD function at top speed.

Jet Lag and Circadian Rhythm
Dr Alfred J. Lewy, a psychiatrist who has been studying the biology behind SAD, describes it as a form of jet lag, a concept he proposed 20 years ago. He recently published experimental evidence that he says attests to the validity of this theory. If true, this would make SAD a disturbance in the circadian rhythm, the 24-hour pattern that normally aligns the sleep-wake cycle with all the other bodily rhythms. Dr. Lewy suggests that with the delayed dawn and shorter days of fall and winter, the rhythms of people afflicted with SAD drift out of phase with the sleep-wake cycle, as if they had traveled across many time zones.

With jet lag, recovery occurs over a matter of days, and the circadian rhythm once again becomes synchronized with day and night. “In people with SAD, this adjustment takes five months,” Dr. Lewy said.

If his theory is substantiated by further research, it may one day be possible to treat SAD with tiny daily doses of time-released melatonin, the substance in the brain that regulates the sleep-wake cycle. Melatonin naturally increases in the evening, causing sleepiness, and falls off as morning approaches. The idea would be to tailor the administration of melatonin in a way that realigns the out-of-sync circadian rhythm in people with SAD, just as tiny doses (much smaller than those typically sold in health-food and drug stores) of melatonin can be used to speed recovery from jet lag.

In his study, conducted with three colleagues at Oregon Health Sciences University, Dr. Lewy identified two types of SAD patients. About two-thirds required morning light or evening melatonin to correct their body clocks. The remainder needed evening light or morning melatonin to put their body rhythms back on track. Currently, there is no commercial source of time-release low-dose melatonin that could be used, with or without light therapy, to help people with SAD.

Current Remedies
Dr. Norman E. Rosenthal, a native of South Africa who discovered his own serious problem with SAD while a resident in psychiatry at the New York State Psychiatric Institute in 1976, has become an expert in diagnosing and treating the problem. His knowledge and experience in helping himself and countless patients afflicted with SAD are summarized in “Winter Blues: Everything You Need to Know to Beat Seasonal Affective Disorder,” whose revised edition the Guilford Press published this year.

Dr. Rosenthal aptly describes SAD as “an energy crisis.” Patients are not depressed in the usual emotional sense, but rather feel as if their batteries have run down.

The symptoms of SAD do mimic those of serious depression. Patients say they have to drag themselves out of bed in the morning, even after 10 hours of sleep, and force themselves to perform necessary chores. They feel leaden and would just as soon not see anybody or do anything. They find it difficult to concentrate and think clearly and quickly.

Sex drive often dwindles markedly but is often replaced by an insatiable appetite for carbohydrates — breads, pasta, potatoes, rice and sweets — that results in weight gain. Many people with SAD have two wardrobes, the one for winter being two sizes larger.

The most common remedy is light therapy. But not just any light. Patients are advised to sit in front of a specially designed light box that emits about 10,000 lux from a fluorescent bulb, most often in the morning for at least 45 minutes. Some patients require hours of light therapy each day to ward off the symptoms of SAD, which may mean having one light box at home and a second at work.

Among commercial sources for these light boxes is the Center for Environmental Therapeutics, which sells them for $200. Its Web site, www.cet.org, is a useful source of information about SAD.

Among other light-enhancing suggestions from Dr. Rosenthal are planning a winter vacation in a sunny climate or relocating to someplace nearer the Equator, where the days are longer in winter. (But, he cautions, first be sure you can tolerate the summer there.)

Helpful Machines and Therapies
For those who remain in northern latitudes, Michael and Jiuan Su Terman of the New York State Psychiatric Institute at Columbia University, who have conducted pioneering studies of SAD remedies, suggest considering a “dawn simulator.” This device gradually turns on a bedroom light every morning while you are still asleep, helping ease SAD symptoms by making the body think that it is experiencing the early sunrises of summer.

This might also help people who do not have SAD but who hate getting up in the morning when it is still dark out.

The Termans have also found another helpful gadget, a negative-ion generator. They showed that sitting in front of a machine that emits negative ions at a high rate for 30 minutes every morning was as effective as sitting in front of a light box for the same time. The generators are available for $165 from the Center for Environmental Therapeutics (Michael Terman is the president of its board). The advantage of this device is that it can be used while sleeping.

A third approach that has proved effective is cognitive behavioral therapy, when used with or without light therapy. Kelly J. Rohan of the Uniformed Services University of the Health Sciences in Bethesda, Md., (and currently of the University of Vermont) found that this therapy, a brief form of psychotherapy that helps people change negative thoughts and behaviors, was as effective as light therapy in a study of 23 patients with SAD.

And unlike light therapy used alone, cognitive behavioral therapy helped prevent a relapse of SAD symptoms the next winter.

Dr. Rosenthal also recommends eating a diet relatively high in protein and low in carbohydrates and performing regular physical exercise, which is especially helpful if it is outdoors in the morning or, if indoors, in front of a light box.
