“I can’t breathe,” said the woman, panic stricken and one hand clutched at her chest. Her forehead was lined with sweat and fear of death etched on her face. Breathing, after all, is essential for life.

We take breathing or respiration for granted. It is an involuntary, autonomous and automatic reflex that is present from birth to death. We can consciously take deep or shallow breaths, but we cannot stop breathing at will. In short, you cannot commit suicide by voluntarily ceasing to breathe.

Breathing difficulty is a perception. Some people may have fairly severe lung restriction, but be able to continue their normal activities. Others may feel breathless with the slightest unaccustomed exertion. This is more likely to occur if the person is unfit, obese and unaccustomed to exercise. A sudden “panic attack” owing to an external factor can be “breath stopping”. High altitudes and high temperature can also cause similar problems.

Breathing difficulty can occur suddenly because of a foreign body like a peanut which has got lodged in the food pipe. This then becomes a medical emergency. Immediately stand behind the person, make a fist with both hands and exert a sudden forceful upward thrust in the stomach area below the chest bone (sternum). This is called a “Heimlich maneuver” and it has saved many lives.

Mucous secretions — watery in case of viral infections and allergy, and thick and viscous in case of secondary bacterial infection — can clog the nasal passages and bronchi. Some of these secretions can drip backwards into the throat, causing a post nasal drip with a sense of suffocation.

The nasal passages can be cleared with nose drops. It is important to use saline nasal drops. This is commercially manufactured by many companies. It needs to be used every two hours. Nasal drops containing chemicals may provide faster relief. But as the effect wears off, they may cause “rebound congestion” with worse blockage. Long-term frequent usage of chemical nose drops may cause habituation, wherein the body stops responding to the medication. If the nasal block is due to an allergy, prescribed locally acting steroid nasal sprays are very effective.

The bronchi can narrow in response to exposure to ingested allergens like peanuts and food colour, or inhaled ones like cigarette or wood smoke, room fresheners and mosquito repellents. This is called reactive airways disease or asthma.

Smokers may have damaged their lungs structure permanently, causing a disease called COAD (chronic obstructive airways disease). This sets in around 20-30 years after the first puff. It attacks people in their 40s and 50s. They become breathless with exertion and sometimes even at rest. Some require continuous artificial oxygen supply.

Initially — that is, before damage sets in — airway constriction is reversible. Inhalers or rotahalers deliver relief producing bronchodilating medication directly to the bronchi. The respite is immediate and sustained if the medication is “puffed” as prescribed. Direct delivery systems make medication more effective and are less likely to cause side effects.

The rate of breathing increases during pregnancy because of the oxygen demand of the baby. Moreover, pregnancy causes the uterus to enlarge which pushes the abdominal contents upwards. This may cause a feeling of inadequacy while breathing. This is self-limited and disappears with delivery.

Congenital heart disease, heart failure and myocarditis (disease of the heart muscles) can cause difficulty in breathing and a feeling of air hunger. This is because a failing heart provides inefficient circulation of blood and insufficient oxygenation to the tissues of the body. The blood carrying capacity of the body itself may be compromised because of anaemia. Specific medication for the diseases will help with the problem.

Breathlessness needs to be evaluated if:

The difficulty occurred suddenly for no apparent reason

• It was accompanied by chest discomfort, pain or pressure

• It comes with a slight exertion or at rest

• There is an inability to lie down flat and sleep

• It is accompanied by fever

• There is pressure, fullness or a squeezing pain in the chest.

Good breathing techniques require the co-ordinated use of intercostals (groups of muscles that run between the ribs, and help form and move the chest wall), diaphragm and stomach muscles. As the breath goes in, the intercostals and stomach expand and the latter moves outwards. And as we breathe out, the chest contracts, the diaphragm moves up and the stomach moves in. Breathing is a natural response and occurs normally and naturally in children. As age advances, we tend to lose the ability to breathe efficiently.

Long hours at work seated in a sloppy posture, obesity or an inactive life without the mandatory one hour of aerobic activity (running, swimming, jogging or cycling) results in inefficient and inadequate breathing, even in a normal individual.

Place a hand on your abdomen, stand in front of a mirror and breathe in and out to check if your breathing is correct. Consciously correct deficiencies and practise breathing exercises (yoga pranayam) for a healthier tomorrow.

Asthma is a respiratory disease characterized by chronic inflammation, labored or shortened breathing, wheezing, coughing and sticky mucous from chest. Before we get into discussing the details of this disease, a brief snapshot of what follows in the paragraphs below would be helpful. We will discuss the health benefits of yoga with focus on yoga and asthma with a dedicated section on yoga poses for asthma.

The Symptoms of Asthma :-
Symptoms of asthma are all apparent unlike certain other diseases. The first symptom that shows up is inflammation in the trachea (wind pipe, connecting throat to lungs) immediately followed by tightness in the chest or shortness of breath. When the asthma attack goes to unmanageable limits, you feel complete constriction of the wind pipe and chest. At this time it is extremely difficult to breathe. Asthma is not known to take death toll unless proper and timely care is not taken. However, the figure of asthma deaths has been on the rise since 1970s in the United States as well as around the globe. There are about 20 million asthma sufferers in the United States itself. What is alarming is slightly less than half of them (about 9 million) are children below 18 years. Out of the total asthmatics, about 70% have other allergies and 10 million of Americans suffer from allergic asthma. Cases of asthma in children have shot up by a mind boggling 160% is the 1980-1994 period. Incidences of reported deaths have mounted to 5000 annually and direct cost of treatment is pegged at $11.5 billion and indirect costs at %4.1 billion. Prevalence is 39% higher in African Americans than white Americans. Total loss of work days is 24.5 million and school days lost is 12.8 million.

STEP-1. Stand with your legs 3 1/2 to 4 feet apart. Hold your arms out to the sides at shoulder height. Turn your right leg out 90 degrees and turn your left leg in 15 degrees. (Your right heel should be in line with your left arch.) Inhale. On an exhale, move your hips toward the left and bend sideways toward your right leg, placing your right hand anywhere on the right shin that feels comfortable. Raise your left hand above your left shoulder.
STEP-2. Lean back slightly and look up just beyond your left thumb. Keep your eyes focused there as you continue to reach your left arm overhead. Move your shoulders down away from your ears and lengthen your neck. Press the right side of your buttocks forward and pull your abdominals in toward your spine. Hold this position for 20 to 30 seconds while you breathe evenly.

Alkalosis

Definition:Alkalosis is a condition in which the body fluids have excess base (alkali).It refers to a condition reducing hydrogen ion concentration of arterial blood plasma (alkalemia). Generally alkalosis is said to occur when pH of the blood exceeds 7.45. The opposite condition is acidosis.

Types:
More specifically, alkalosis can refer to:

*Respiratory alkalosis
*Metabolic alkalosis

Causes: The lungs and kidneys regulate the acid/base status of the body. Decreased carbon dioxide (an acid) or increased bicarbonate (a base) levels make the body too alkaline, a condition called alkalosis.

The kidneys are located in the posterior abdomen and are responsible for filtering urine from the blood.
The main cause of respiratory alkalosis is hyperventilation, resulting in a loss of carbon dioxide. Compensatory mechanisms for this would include increased dissociation of the carbonic acid buffering intermediate into hydrogen ions, and the related consumption of bicarbonate, both of which would lower blood pH.

Metabolic alkalosis can be caused by prolonged vomiting, resulting in a loss of hydrochloric acid with the stomach content. Severe dehydration, and the consumption of alkali are other causes. Compensatory mechanism for metabolic alkalosis involve slowed breathing by the lungs to increase serum carbon dioxide, a condition leaning toward respiratory acidosis. As respiratory acidosis often accompanies the compensation for metabolic alkalosis, and visa versa, a delicate balance is created between these two conditions.

*Respiratory alkalosis is caused by low carbon dioxide levels. Being at a high altitude or having a disease that reduces oxygen in the blood can cause you to breathe faster (hyperventilate), which lowers carbon dioxide levels.

*Metabolic alkalosis is caused by too much bicarbonate in the blood.

*Hypochloremic alkalosis is caused by an extreme lack or loss of chloride, which can occur with prolonged vomiting.

*Hypokalemic alkalosis is caused by the kidneys’ response to an extreme lack or loss of potassium, which can occur when people take certain diuretic medications.

*Compensated alkalosis occurs when the body returns the acid/base balance to normal in cases of alkalosis, but bicarbonate and carbon dioxide levels remain abnormal.

Symptoms :

*Confusion (can progress to stupor or coma)

*Hand tremor

*Light-headedness

*Muscle twitching

*Nausea, vomiting

*Numbness or tingling in the face or extremities

*Prolonged muscle spasms (tetany)

Other Symptoms are :over-excitability of the nervous system manifested by nervous “tics,” hyperventilation, sore muscles, creaking and popping joints, bursitis, drowsiness, “bugged-out” eyes, high blood pressure, edema, allergies, night cramps, asthma, chronic indigestion, night coughs, thick blood, menstrual problems etc.

Diagnosis:(Exams and Tests):Tests of pH can show whether you have alkalosis or acidosis. Carbon dioxide and bicarbonate tests indicate whether the cause of alkalosis or acidosis is respiratory (breathing-related) or metabolic (kidney-related).

Tests for alkalosis include:

*Arterial blood gas (or venous blood gas)

*Chem-20

*Litmus paper (urine dipstick tests)

*Urinalysis

*Urine pH

Contraction Alkalosis and Hypokalemia

Treatment : Treatment of alkalosis depends on finding the specific cause.

For alkalosis caused by hyperventilation, breathing into a paper bag causes you to retain more carbon dioxide. You may receive oxygen.

Some people need medications to correct chemical loss (such as chloride and potassium). Your health care provider will monitor your vital signs (temperature, pulse, rate of breathing, blood pressure).

Herbs that are more beneficial to Fight symptoms that include creaking and popping joints, allergies, night cramps, asthma and more with nature’s remedies are Alfalfa and Rose Hips.

Prognosis:

Most cases of alkalosis respond well to treatment.

A visit to the emergency room or call to the local emergency number (such as 911) is warranted for:

Prevention :Prevention depends on the cause of the alkalosis. Normally, people with healthy kidneys and lungs do not significantly experience alkalosis.
