Drug Combos Pose Risk for Elderly

Older adults in the United States are popping prescription pills, over-the-counter drugs and dietary supplements in record numbers, and in combinations that could be deadly, US researchers said on Tuesday.

They said more than half of US adults aged 57 to 85 are using five or more prescription or non-prescription drugs, and one in 25 are taking them in combinations that could cause dangerous drug interactions.

“Older adults in the United States use medicine and they use a lot of it,” said Dr. Stacy Tessler Lindau of the University of Chicago Medical Center in Illinois, whose study appears in the Journal of the American Medical Association.

“While medications are often beneficial, they are not always safe,” she said in a telephone interview.

She noted a recent report that estimated U.S. adults over 65 make up more than 175,000 emergency department visits a year for adverse drug reactions, and commonly prescribed drugs accounted for a third of these visits.

For the study, Lindau teamed up with Dima Qato, a pharmacist and researcher at the University of Chicago. They used data from a national survey of adults aged 57 to 85 and interviews with nearly 3,000 people in their homes to get a read on the medications they used on a regular basis.

They analyzed potential interactions among the top 20 prescription and over-the-counter drugs and the top 20 dietary supplements, and found that 68 percent of adults surveyed who took prescription drugs also used over-the-counter drugs or dietary supplements.

Men in the 75 to 85-year-old age group were at the highest risk, they said. “One in 10 men between the ages of 75 to 85 were at risk for a drug-to-drug interaction,” Qato said in a telephone interview.

Nearly half of the potential drug-to-drug interactions could cause bleeding problems. The blood thinner warfarin, often sold by Bristol-Myers Squibb Co. under the brand name Coumadin, was most commonly cited in potentially dangerous combinations.

Some 2 million Americans take warfarin after a heart attack, stroke or major surgery. The team found warfarin was commonly teamed up with aspirin, a drug often taken to prevent heart attacks that also interferes with clotting.

Warfarin and the cholesterol-lowering statin drug simvastatin, which is sold by Merck & Co under the brand name Zocor, was another combination that could cause potential bleeding risks.

Among non-prescription drugs, they found many people were taking the popular nutritional supplement Ginkgo biloba in combination with aspirin, another potential cause of bleeding.

The team was reassured that they found no instances of people taking absolutely forbidden drug combinations, but the finding of widespread use of drugs that could cause major drug reactions was worrisome.

“We think the patient needs to know about these risks,” Qato said.

The researchers recommend patients carry a list in a wallet or purse of all of the drugs and supplements they take.

And they said doctors, pharmacists and other health professionals should remember to ask about all of the medications their patients are taking.

Gallbladder and it’s functions: A pear-shaped organ just below the liver that stores the bile secreted by the liver. During a fatty meal, the gallbladder contracts, delivering the bile through the bile ducts into the intestines to help with digestion. Abnormal composition of bile leads to formation of gallstones, a process termed cholelithiasis. The gallstones cause cholecystitis, inflammation of the gallbladder.

Some 20 million Americans have gallstones, crystallized pellets in the gallbladder that can suddenly cause painful spasms a few hours after eating a rich meal. A high-fiber diet, along with certain supplements, can help prevent, relieve, or even dissolve these troublesome stones.

Symptoms

Symptoms of gallbladder disease occur when gallstones irritate the gallbladder. The most common symptoms associated with gallstone disease include:

Severe and intermittent pain in the right upper abdomen. This pain can also spread to the chest, shoulders or back. Sometimes this pain may be mistaken for a heart attack.

Chronic indigestion and nausea.
Intermittent pain on the right side of the upper abdomen. The pain typically develops after a meal, lasts from 30 minutes to 4 hours, and may move to the back, chest, or right shoulder.
Nausea and vomiting may accompany pain. Heartburn, gas, or bloating may also be present.

The gallbladder is a small pear-shaped organ that averages three to six inches in length. It lies underneath the liver in the upper right side of the abdomen. It is connected to the liver and small intestine by small tubes called bile ducts. Bile, a greenish-brown fluid, is utilized by the body to digest fatty foods and assists in the absorption of certain vitamins and minerals. The gallbladder serves as a reservoir for bile. Between meals, bile accumulates and is concentrated within this organ. During meals, the gallbladder contracts and empties bile into the intestine to assist in digestion.

There are two major types of gallstones:

Causes for Gall stone :
Though the exact cause of gallstones is not known, several factors may contribute to their formation, including a low-fiber, high-fat diet; intestinal surgery; inflammatory bowel disease; or other disorders of the digestive tract. Gallstones tend to occur in people over age 40 and are three times more common in women than in men. Obesity is also strongly linked to gallstones, as is rapid weight loss. There may be a genetic component as well: Among Arizona’s Pima Indians, nearly 70% of women over age 30 have gallstones.

Approximately 80% of all gallstones are completely asymptomatic and “silent.” The chance that a “silent” gallstone will become symptomatic is 2% for each year.

How are gallstones identified?

Nearly all gallstones can be easily identified by an ultrasound examination. This is a simple and painless procedure in which sound waves are utilized to create pictures of the gall-bladder, bile ducts and its contents. This test is highly sensitive for identifying either gallstones or sludge within the gallbladder.

What can be done at home?
Recurrent painful attacks, if mild, can be treated with over-the-counter painkillers. Placing something warm on your stomach may be helpful, taking care not to scald the skin. The frequency of attacks may be reduced by a low-fat diet.

Can gallstones be prevented?
Recent studies have suggested that persons at highest risk for gallstone formation, obese persons undergoing weight reduction, can virtually eliminate their risk for developing gallstones by taking KANCHANAR GUGGULU.

Description:
Aquillaria agallocha is a moderate-size evergreen tree  with a conical crown when young. Height of this plant is around 60-80 feet and leaves 5-8.7 cm long, linear-lanceolate to lanceolate. Flowers rather small, greenish on shortly peduncled umbels, arising laterally from the younger branches. Capsules 3.7-5 cm, obovate-cuneate, slightly compressed. It occurs sporadically in the forest of Hill Tracts region of India,Nepal and Bangladesh.

Cultivation method: Usually seeds of this plant are used for propagation. Aggor plants are grows well in a sloping area of Hill Tracts region.

Chemical Constituents:
Principal constituent of the wood is an essential oil, which contains agarospirol. Wood also contains a chromone, agarotetrol, 1,7-oxaporphine, liriodenine and two sesqiuterpenes, gmelofuran and agarol. Stem bark contains two cytotoxic compounds (Asolkar et al., 1992). The neuroleptic compounds, jinkoheremol and agarospirol have been isolated from the benzene extract of the wood (Ghani, 2003).

Medicinal Uses:
Wood is heating, alterative tonic, carminative, laxative, stomachic, diuretic, aphrodisiac and febrifuge; useful in diseases of ear and skin, hiccup, leucoderma, chronic diarrhoea, bronchitis, asthma. The fragrant resinous substance is prescribed in gout and rheumatism. The bark is used for heart disease in Khagrachari. Decoction of wood is used as medicine for febrifuge, stomachic, diuretic, aphrodisiac, carminative, laxative and tonic. It is also very effective in various skin disease, bronchitis, asthma and rheumatism.

Other Uses:    Powdered from wood of this plant is used as perfume.

The agarwood is very popular as ‘the wood of the Gods’, It is because of its uses range from incense for religious ceremonies, perfume for the Arabic world, medicinal wine in Korea and ornamental functions in China. As a healthy tree the Aquilaria is worth next to nothing, but wounded its defence mechanisms produce agarwood and the tree becomes a valuable commodity. Today the range of agarwood products and their uses is seemingly endless. Natural art Solid pieces of agarwood are highly appreciated as ‘natural art’ in Japan, Korea and Taiwan. Craftsmen carve raw pieces of agarwood into beautiful wooden sculptures . Agarwood is also turned into beads and bracelets. Most of the wood, however, is processed and either turned into oil which is used in perfumes and other cosmetic products, or the agarwood chips are ground into powder which is used as the raw material for incense making (and sometimes also for special cigarettes).

The oil is also used in the production of traditional medicine as anti-asthma antitoxic, antioxidant, hypertension (anti-stress), hepatitis, sirosis, diuretic, painkiller, and many other diseases.
It is because of the aromatic smell of some composition of oils as reported by some scientists there are dihydroagarofuran and isodihydroagarofuran ; sesquiterpene, agarol and a couinarinolignan, aquillochin; sesquiterpene alcohols, jinkohol II and jinkoheremol; agarospirol, jinkohol-eremol and kusenol.

This is why agarwood or gaharu so expensive. The high price of this woody species actually is corelated to the resin or so called “Gubal”. This resin looks like a dark brown to black solid lump or a chunk with a fragrant smell (if burned), which is found in the heartwood or roots of gaharu-producing trees undergoing a chemical and physical change due to fungus infection (as mentioned by Dr. Devang Pandya) . The trees frequently become infected with a parasite fungus or mold, and begin to produce an aromatic resin in response to this attack. So, not all plants can produce the resin because it depends on the attack. The fungus and decomposition process continue to generate a very rich and dark resin forming within the heartwood. While the unaffected wood of the tree is relatively light in color & almost useless, the resin dramatically increases the mass and density of the affected wood, changing its color from a pale beige to dark brown or black. In natural forest only as rare as 7% of the trees are infected by the fungus in natural way. Thus, agarwood develops very, very slowly over time, typically several hundred years. However, nowadays people inpurposely develop agarwood plantation using fungi-forming gaharu injection on the hole of the trunk for fast harvest.
