A: Psoriasis can cause nail dystrophy. This means the nails may be pitted, discoloured or have ridges, and break easily, crumble or separate from the nail bed. Sometimes this can co-exist with fungal infection of the nail or secondary bacterial infection of the nail bed.
Psoriasis of the nails has no specific treatment. The nails should be kept short. Nail varnish or henna may be used to disguise the appearance. If the psoriasis is complicated by fungal or bacterial infection, specific treatment needs to be taken after consulting a doctor.

A: Good control of diabetes requires dieting and exercise. If you aren’t compliant, don’t eat in a measured way and refuse to walk or exercise, your diabetes will probably stay uncontrolled. Try to correct these factors first.
Metformin is a tried and tested medication. It does not cause the blood sugars to drop to dangerously low levels. The biggest advantage is it is taken after food. Indians tend to fast on certain days. If a person administers insulin or takes medication after fasting, the sugars may drop to dangerously low levels.

Zinc is used to make metal alloys and is usually an ingredient in making batteries and coins. Zinc oxide, on the other hand, is an ingredient in sun screen. Zinc is also needed by the body. An average person needs 11 mg of zinc ever day; lack of zinc can lead to hair loss and diarrhea. Too much zinc, on the other hand, can cause anemia. Luckily it is possible to get the recommended daily allowance of zinc through food. Some foods that are rich in zinc are seeds and whole grains. However, it is also possible to get zinc supplements, or on the other hand, multi-vitamins that are enriched with zinc.
Zinc is included in most single tablet over-the-counter daily vitamin and mineral supplements. Foods and spices containing zincIn the U.S., the Recommended Dietary Allowance (RDA) is 8 mg/day for women and 11 mg/day for men. Median intake in the U.S. around 2000 was 9 mg/day for women and 14 mg/day in men. Red meats, especially beef, lamb and liver have some of the highest concentrations of zinc in food.
The concentration of zinc in plants varies based on levels of the element in soil. When there is adequate zinc in the soil, the food plants that contain the most zinc are wheat (germ and bran) and various seeds (sesame, poppy, alfalfa, celery, mustard). Zinc is also found in beans, nuts, almonds, whole grains, pumpkin seeds, sunflower seeds and blackcurrant.
Other sources include fortified food and dietary supplements, which come in various forms. A 1998 review concluded that zinc oxide, one of the most common supplements in the United States, and zinc carbonate are nearly insoluble and poorly absorbed in the body. This review cited studies which found low plasma zinc concentrations after zinc oxide and zinc carbonate were consumed compared with those seen after consumption of zinc acetate and sulfate salts. However, harmful excessive supplementation is a problem among the relatively affluent, and should probably not exceed 20 mg/day in healthy people, although the U.S. National Research Council set a Tolerable Upper Intake of 40 mg/day.
For fortification, however, a 2003 review recommended zinc oxide in cereals as cheap, stable, and as easily absorbed as more expensive forms. A 2005 study found that various compounds of zinc, including oxide and sulfate, did not show statistically significant differences in absorption when added as fortificants to maize tortillas. A 1987 study found that zinc picolinate was better absorbed than zinc gluconate or zinc citrate. However, a study published in 2008 determined that zinc glycinate is the best absorbed of the four dietary supplement types available.
Zinc deficiency is usually due to insufficient dietary intake, but can be associated with malabsorption, acrodermatitis enteropathica, chronic liver disease, chronic renal disease, sickle cell disease, diabetes, malignancy, and other chronic illnesses. Symptoms of mild zinc deficiency are diverse. Clinical outcomes include depressed growth, diarrhea, impotence and delayed sexual maturation, alopecia, eye and skin lesions, impaired appetite, altered cognition, impaired host defense properties, defects in carbohydrate utilization, and reproductive teratogenesis. Mild zinc deficiency depresses immunity, although excessive zinc does also. Animals with a diet deficient in zinc require twice as much food in order to attain the same weight gain as animals given sufficient zinc.
Groups at risk for zinc deficiency include the elderly, vegetarians, and those with renal insufficiency. The zinc chelator phytate, found in seeds and cereal bran, can contribute to zinc malabsorption in those with heavily vegetarian diets. There is a paucity of adequate zinc biomarkers, and the most widely used indicator, plasma zinc, has poor sensitivity and specificity. Diagnosing zinc deficiency is a persistent challenge.
Nearly two billion people in the developing world are deficient in zinc. In children it causes an increase in infection and diarrhea, contributing to the death of about 800,000 children worldwide per year. The World Health Organization advocates zinc supplementation for severe malnutrition and diarrhea. Zinc supplements help prevent disease and reduce mortality, especially among children with low birth weight or stunted growth. However, zinc supplements should not be administered alone, since many in the developing world have several deficiencies, and zinc interacts with other micronutrients.
Zinc deficiency is crop plants’ most common micronutrient deficiency; it is particularly common in high-pH soils. Zinc-deficient soil is cultivated in the cropland of about half of Turkey and India, a third of China, and most of Western Australia, and substantial responses to zinc fertilization have been reported in these areas. Plants that grow in soils that are zinc-deficient are more susceptible to disease. Zinc is primarily added to the soil through the weathering of rocks, but humans have added zinc through fossil fuel combustion, mine waste, phosphate fertilizers, limestone, manure, sewage sludge, and particles from galvanized surfaces. Excess zinc is toxic to plants, although zinc toxicity is far less widespread.
Although zinc is an essential requirement for good health, excess zinc can be harmful. Excessive absorption of zinc suppresses copper and iron absorption. The free zinc ion in solution is highly toxic to plants, invertebrates, and even vertebrate fish. The Free Ion Activity Model is well-established in the literature, and shows that just micromolar amounts of the free ion kills some organisms. A recent example showed 6 micromolar killing 93% of all Daphnia in water.
The free zinc ion is a powerful Lewis acid up to the point of being corrosive. Stomach acid contains hydrochloric acid, in which metallic zinc dissolves readily to give corrosive zinc chloride. Swallowing a post-1982 American one cent piece (97.5% zinc) can cause damage to the stomach lining due to the high solubility of the zinc ion in the acidic stomach.
There is evidence of induced copper deficiency at low intakes of 100–300 mg Zn/day; a recent trial had higher hospitalizations for urinary complications compared to placebo among elderly men taking 80 mg/day. The USDA RDA is 15 mg Zn/day. Even lower levels, closer to the RDA, may interfere with the utilization of copper and iron or adversely affect cholesterol. Levels of zinc in excess of 500 ppm in soil interfere with the ability of plants to absorb other essential metals, such as iron and manganese. There is also a condition called the zinc shakes or “zinc chills” that can be induced by the inhalation of freshly formed zinc oxide formed during the welding of galvanized materials.
The U.S. Food and Drug Administration (FDA) has stated that zinc damages nerve receptors in the nose, which can cause anosmia. Reports of anosmia were also observed in the 1930s when zinc preparations were used in a failed attempt to prevent polio infections. On June 16, 2009, the FDA said that consumers should stop using zinc-based intranasal cold products and ordered their removal from store shelves. The FDA said the loss of smell can be life-threatening because people with impaired smell cannot detect leaking gas or smoke and cannot tell if food has spoiled before they eat it. Recent research suggests that the topical antimicrobial zinc pyrithione is a potent heat shock response inducer that may impair genomic integrity with induction of PARP-dependent energy crisis in cultured human keratinocytes and melanocytes.
In 1982, the United States Mint began minting pennies coated in copper but made primarily of zinc. With the new zinc pennies, there is the potential for zinc toxicosis, which can be fatal. One reported case of chronic ingestion of 425 pennies (over 1 kg of zinc) resulted in death due to gastrointestinal bacterial and fungal sepsis, while another patient, who ingested 12 grams of zinc, only showed lethargy and ataxia (gross lack of coordination of muscle movements). Several other cases have been reported of humans suffering zinc intoxication by the ingestion of zinc coins.
Pennies and other small coins are sometimes ingested by dogs, resulting in the need for medical treatment to remove the foreign body. The zinc content of some coins can cause zinc toxicity, which is commonly fatal in dogs, where it causes a severe hemolytic anemia, and also liver or kidney damage; vomiting and diarrhea are possible symptoms. A specialist light treatment for psoriasis is just as effective and safe when given at home as in hospital, say Dutch researchers.

Phototherapy using UVB light is rarely used in the UK because of limited availability and the number of hospital visits required.
But a study of 200 patients found the same results with home treatment.
One UK expert said the British Medical Journal study highlighted an important treatment gap in psoriasis care.

Up to 3% of the UK population is affected by the non-contagious condition which can cause significant disability.
For those who have access to UVB treatment at their local dermatology unit, a course usually entails three visits each week for between eight to 10 weeks.
It works by dampening down the immune overreaction in the skin.
One reason that the treatment is usually done in hospital is because most dermatologists believe that home phototherapy is inferior and that it carries more risks.
In the latest study, patients with psoriasis from 14 hospital dermatology departments were randomly assigned to receive either home UVB phototherapy or hospital-based treatment.
Home treatment was equivalent to hospital therapy both in terms of safety and the effectiveness of clearing the condition.
And those treated at home reported a significantly lower burden of treatment and were more satisfied.
Study leader Dr Mayke Koek, from Utrecht University Medical Centre, said: “We knew a lot of dermatologists are not convinced of the safety and effectiveness of UVB phototherapy but our theory was they should be equally safe.”
“One of the most important findings was a lot of patients treated at home were more satisfied.”
Professor Alex Anstey from the Royal Gwent Hospital in Newport, Wales, said with the exception of Scotland, phototherapy in the UK was limited to people who lived near a big hospital.
“In my area there are very large numbers of people who don’t have access to phototherapy which is a shame as it’s a very effective and safe treatment.” ………Says Professor Alex Anstey, Royal Gwent Hospital, Wales
He said that the equipment – similar to a tanning bed, but a different type of light – costs between £5,000 and £10,000 but was very cheap compared with some of the new biological therapies in use and could be lent to patients for the duration of their treatment.
Gladys Edwards, chief executive of the Psoriasis Association, said the guidelines on treating psoriasis should be reviewed in light of the new research.
“Patient choice is important and for some patients managing their UV treatment at home would clearly be preferable.
“It is crucial, however, that there is absolutely clear guidance and information on when this is appropriate and how it should be managed for patients and clinicians.”

Definition: Seborrhea (say: seb-uh-ree-uh) is a common skin problem. It causes a red, itchy rash and white scales. When it affects the scalp, it is called “dandruff.” It can be on parts of the face as well, including the folds around the nose and behind the ears, the forehead, and the eyebrows and eyelids. On the body, seborrhea often occurs in the middle part of the chest, around the navel and in the skin folds under the arm, below the breasts and in the groin and buttocks area.
Seborrhoeic eczema (also Seborrheic dermatitis AmE, seborrhea) is a skin disorder affecting the scalp, face, and trunk causing scaly, flaky, itchy, red skin. It particularly affects the sebum-gland rich areas of skin.
Who gets seborrhea?
Infants may get seborrhea. It’s known as “cradle cap.” Cradle cap goes away after about 6 months. It may also affect the diaper area and look like a diaper rash.
Seborrhea also affects adults and elderly persons, and is more common in men than in women. Seborrhea occurs more frequently in persons with oily skin.
It affects 3 percent of the general population. It occurs more commonly in older people who are bedridden or have neurologic conditions such as Parkinson’s disease. Seborrhea also affects almost 85 percent of people with AIDS.
Causes:The cause of seborrheic dermatitis is not fully understood, although many factors have been implicated.. It is likely that a number of factors, such as hormones and stress, can cause it.
The widely present yeast, Malassezia furfur (formerly known as Pityrosporum ovale), is involved, as well as genetic, environmental, hormonal, and immune-system factors. A theory that seborrhoeic dermatitis is an inflammatory response to the yeast has not been proven. Those afflicted with seborrhoeic dermatitis have an unfavourable epidermic response to the infection, with the skin becoming inflamed and flaking.
Acute form of seborrhoeic dermatitis on scalpIn children, excessive vitamin A intake can cause seborrhoeic dermatitis. Lack of biotin, pyridoxine (vitamin B6) and riboflavin (vitamin B2) may also be a cause.
It is a chronic inflammatory skin disorder that affects the areas of the head and trunk that have sebaceous glands. A type of yeast that has an affinity for these glands called Pityrosporum ovale may be the cause, but this has not been proven yet. It is believed that the build-up of yeast in these glands irritates the skin causing redness and flaking.
Seborrhea is more common in men than women and affects 3 percent of the general population. It occurs more commonly in older people who are bedridden or have neurologic conditions such as Parkinson’s disease. Seborrhea also affects almost 85 percent of people with AIDS.
Seborrheic dermatitis typically affects areas of the skin where sebaceous glands appear in high frequency and are most active. The distribution is classically symmetric, and common sites of involvement are the hairy areas of the head, including the scalp , the scalp margin , eyebrows, eyelashes, mustache and beard. Other common sites are the forehead , the nasolabial folds , the external ear canals and the postauricular creases. Seborrhea of the trunk may appear in the presternal area and in the body folds, including the axillae, navel, groin, and in the inframammary and anogenital areas. Figure 7 illustrates the typically symmetric distribution of seborrheic dermatitis.
More severe seborrheic dermatitis is characterized by erythematous plaques frequently associated with powdery or greasy scale in the scalp (Figure 8), behind the ears (Figure 9) and elsewhere in the distribution described above. Besides an itchy scalp, patients may complain of a burning sensation in facial areas affected by seborrhea. Seborrhea frequently becomes apparent when men grow mustaches or beards and disappears when the facial hair is removed. If left untreated, the scale may become thick, yellow and greasy and, occasionally, secondary bacterial infection may occur.

UV-A and UV-B light inhibit the growth of P. ovale,9 and many patients report improvement in seborrhea during summer.
Soaps and detergents such as sodium laureth sulfate may precipitate a flare-up, as they strip moisture from the top layers of the skin, and the drying property of these can cause flare-ups and may worsen the condition. Accordingly a suitable alternative should be used instead.
Among dermatologist recommended treatments are shampoos containing coal tar, ciclopiroxolamine, ketoconazole, selenium sulfide, or zinc pyrithione. For severe disease, keratolytics such as salicylic acid or coal tar preparations may be used to remove dense scale. Topical terbinafine solution (1%) has also been shown to be effective in the treatment of scalp seborrhoea, as may lotions containing alpha hydroxy acids or corticosteroids (such as fluocinolone acetonide). Pimecrolimus topical lotion is also sometimes prescribed.
Chronic treatment with topical corticosteroids may lead to permanent skin changes, such as atrophy and telangiectasia.

According to the American Academy of Family Physicians(AAFP), one treatment that has proven successful, especially when steroid topicals and shampoos aren’t working, and the patient continues to suffer from rapid hair loss and rashes, has been low doses(10mg-30mg daily) of the perscription drugAccutane,(Isotretinoin). The exact mechanism isn’t known, but it is thought to work by reducing sebum, which plays an important role in seborrhoeic dermatitis. Patients should be evaluated monthly, while examing the proper liver functions when putting a patient on accutane therapy. Special screening should be in place for women patients, because of the risk of birth defects. This therapy can last, when the condition is chronic and the isotretinoin does is low, for years. But, patients should be given a one to two month break off this particular therapy every 6 months to see if the condition still is affecting the patient
Adults who have seborrhea usually experience a waxing and waning course. In other words it can’t be “cured”. The good news is with proper maintenance, seborrhea can be controlled. Furthermore, most of the treatments can be found over-the-counter.

Dandruff Shampoo If you have dandruff, use medicated shampoos.
When using dandruff shampoo, first wet your hair. Rub some shampoo into your scalp and hair. Leave the shampoo on your scalp and hair for at least 5 minutes. Then rinse it out. Use the dandruff shampoo every day until your dandruff goes away. Then use the medicated shampoo 2 or 3 times a week to keep dandruff away. Having dandruff does not mean that your scalp is too dry! Dandruff comes because you need to wash your hair more often.
Medicated Shampoos should always be used.For black persons, daily shampooing may not be needed. Ask your doctor about a special steroid preparation in oil that can be used on the scalp like a pomade. Or you can use a steroid-containing shampoo.

Proper hygiene plays an important role in treatment. Frequent washing with soap gets rid of the oils in the affected areas and improves symptoms. Sunlight inhibits the growth of the yeast; therefore exposure of affected areas to sun is helpful, although caution should be exercised to avoid sun damage. The main medical treatments are antifungal shampoos and topical.
Cradle cap in infants also gets better with daily shampooing. First try a mild, nonmedicated baby shampoo. If that doesn’t work, try an a dandruff shampoo. If the patch of cradle cap is large and thick, first try softening it by rubbing on warm mineral oil. Next, gently brush with a baby hairbrush. Then use shampoo.
There are several good antifungal shampoos on the market that can be purchased without a prescription. The main shampoos are selenium sulfide found in Selsun, pyrithione zinc found inHead & Shouldersulders and Sebulon, coal tar found in Sebutone and Tegrin, and finally ketoconazole found in Nizoral.
All of these shampoos have a medicated smell. The way to use them is to shampoo and leave on for at least 10 minutes then rinse off. The shampoos can be used on the face and other parts of the body as a lotion with the same instructions as long as precaution is used around the eyes. Do this daily until the redness and flaking is controlled then use 2-3 times a week as needed to keep symptoms from returning.
Topical Steroids For Seborrhea
Topical steroids reduce the inflammatory response and help control itching. You can buy hydrocortisone cream 1% over-the-counter, and it’s safe to use on the face. Apply twice a day to the affected area until the redness resolves. Save the hydrocortisone for flare-ups and use the antifungal shampoo for maintenance because long-term steroid use can cause side effects like acne and thinning of the skin.
Herbal Treatment:The World Health Organization mentions Aloe vera gel as a yet to be scientifically proven traditional medicine treatment for Seborrhoeic dermatitis.
