Hearing Loss Is Common, but Often Untreated

Americans who suffer from hearing loss. They include a third of Americans over 60 and up to half of those over 75, most of whom have age-related hearing loss, a condition known medically as presbycusis. Hearing loss is the third most common chronic condition among older Americans, after hypertension and arthritis. Hearing difficulties in older people can have serious consequences, including social isolation, functional decline and depression. Hearing loss can also impair memory and cognitive function, according to a study by neuroscientists at Brandeis University.

A survey of 2,000 hearing-impaired adults conducted in 1999 by the National Council on Aging found that those using aids had better feelings about themselves, greater independence, improved mental health and better relationships with their families. Yet only one person in five with hearing loss wears a hearing aid — partly because of their cost, which is not covered by Medicare and rarely by private insurance.

Acknowledging the Problem
Some people do not know — or they deny — that they have a hearing problem, complaining instead that everyone seems to mumble or talk too fast. Even those who get a yearly physical rarely have their hearing checked. Others are embarrassed to wear a hearing aid. About 30 percent of people who have hearing aids don’t wear them daily.

Hearing aids have improved vastly in the past decade, in both design and selection. Even so, some people, having once had a bad experience, refuse to explore the many new options.

As with the eye and vision, there are many steps between the ear and hearing, a process that takes but a tiny fraction of a second. Sound entering the ear canal causes the eardrum to vibrate. These vibrations are picked up by three tiny bones in the middle ear that connect the drum to the cochlea, a snail-shell-like structure with three tubes filled with fluid. The resulting waves in the fluid signal hair cells in the cochlea that transmit electrical signals to the auditory nerve that connects to the brain stem. These signals then travel to the brain’s auditory center, where the message is processed.

Disruption or damage at any stage in this chain can result in hearing loss. Among factors that can damage hearing are trauma, chronic infection, wax buildup, fusion of ear bones, diseases like diabetes and medications like the antibiotics vancomycin and gentamicin. Some anticancer drugs are also toxic to the ear. Heredity, too, plays a role; some people carry gene mutations that make them more susceptible to hearing loss.

The most common environmental factor is loud noise, either a sudden very loud noise like an explosion or gunshot next to the ear or, more commonly, repeated exposure to loud noises like those produced by rock bands or earbuds and headphones. Some rockers and countless rock fans have developed hearing problems.

Hearing loss associated with aging most often results from cumulative damage to the hair cells in the cochlea, which, like other body parts, suffer the wear and tear of age. The first to decline are those in the outer part of the cochlea that are sensitive to high-frequency sounds, including those produced by the consonants f, sh, ch, p, s and t, which are crucial to clarity in perceiving speech. The low-frequency vowel sounds are the last to go.

Finding a Solution
Detection of a hearing problem is the first step. Hearing specialists have long urged family physicians to check the hearing of patients over 60 at every annual visit by doing a whisper test in each ear or administering a short written quiz.

Anyone with a suspected hearing problem should be referred to an audiologist for detailed testing, or to an otolaryngologist if the cause is medical. Anyone experiencing sudden loss of hearing in one or both ears should consult an otolaryngologist without delay. That could be a reversible problem if treated quickly.

Audiologists are certified clinicians trained to analyze a hearing problem, prescribe hearing aids and help people adjust to their use. In areas where there is no audiologist, look for a licensed hearing aid specialist who is trained to fit and dispense hearing aids.

Choosing a Hearing Aid

Four styles of aids are now available, ranging in price from about $400 to $3,000:

*  A behind-the-ear model fits over the ear and directs sound into the ear canal through a tube and custom-fitted ear mold. This model offers the most circuit and feature options and is easiest to handle for people with limited dexterity.

*An in-the-ear model fits into the outer ear and projects slightly into the ear canal. It is relatively easy to handle and also supports many features.

* An in-the-canal model protrudes only slightly into the outer ear but can accommodate fewer features and is more difficult to handle.

* A completely-in-the-canal model, the smallest and most difficult to handle, is not noticeable in the outer ear but has the fewest features.

Audiologists can help patients select the most appropriate model based on their hearing and living needs and dexterity. When circumstances change, audiologists can also reprogram hearing aids. New designs help patients distinguish speech in noisy environments; some adjust automatically while others require the user to make adjustments. For people with severe hearing loss who need a lot of amplification, new devices have been designed to suppress the high-pitched whistle that can be produced by a hearing aid turned to high volume amplification.

Most important for anyone getting a hearing aid is to take the time needed to adjust to its use. No hearing aid can replace normal hearing, but when properly fitted and adjusted, an aid can greatly improve quality of life.

Could Stem Cells Make You More Beautiful?

Stem Cells Could Have Cosmetic Applications, but They’re Likely Far Off.

The prospect is a tantalizing one. To erase wrinkles and fine lines, or to get bigger breasts, without cosmetic surgery. Forget silicone, forget collagen. All you would need is stem-cell therapy.

Realistically speaking, though, such applications remain a pipe dream.

Of course, it wouldn’t be the first time a medical therapy had been bent in the direction of aesthetics. Take a look at Botox   the deadly botulinum toxin initially used to treat spasms is now used to improve the appearance of frown lines.

And while stem-cell applications for the vanity market may have to wait, some researchers have begun to research the possibilities of stem cells in plastic and reconstructive medicine.

“Stem-cell research appears promising for medicine and particularly for plastic surgery,” said Dr. Ronald Friedman, director of the West Plano Plastic Surgery Center and a board-certified plastic surgeon practicing in Plano, Tex.

“Hair follicular stem cells, tooth stem cells and skin stem cells all show therapeutic promise,” said Denis English, editor in chief of the journal Stem Cells and Development and director of cell biology at the Center of Excellence for Aging and Brain Repair at the University of South Florida in Tampa, Fla. “These can restore hair to a bald man, teeth to those in need and skin to scarred patients.”

The use of stem cells to regenerate tissue is believed to hold promise because stem cells can be nudged to develop into specialized cell types. And some researchers have turned an eye toward stem cells for this very purpose.

In October, a University of Pittsburgh team led by Dr. Peter Rubin received a three-year grant from the National Cancer Institute to explore the possibility of using stem cells derived from a patient’s own fat. Rubin, assistant professor of plastic surgery and co-director of the university’s Adipose Stem Cell Center, used those stem cells to create a durable, shaped piece of replacement tissue.

The research may one day allow breast cancer survivors to take advantage of a natural replacement after a mastectomy.

But with these possible applications in reconstruction, could cosmetic applications be far behind?

“Naturally, the public shows more interest in applications like breast augmentation,” said Dr. Peter Constantino, director of the Center for Facial Reconstruction and Restoration at Roosevelt Hospital in New York.

“In our society, there is such a huge demand for these rejuvenation surgeries, despite their significant risks, that the pragmatist in me cannot deny the likelihood that it will not be long before someone offers a two-stage procedure starting with liposuction followed by injection of these autologous stem cells for breast augmentation or into the face to rejuvenate,” said Dr. Daniel Salomon of the department of molecular and experimental medicine at the Scripps Research Institute in La Jolla, Calif.

Real-World Applications Still Far Off
Though initial research into the potential of stem cells in reconstructive surgery is promising, actual applications    particularly those of a purely cosmetic nature    are still distant.

“This is still very far in the future, except for tabloid speculation,” said Dr. Garry Brody, professor emeritus of plastic and reconstructive surgery at the University of Southern California’s Keck School of Medicine in Los Angeles. “By the time it becomes practical    and affordable    I suspect it will be beyond our lifetimes.”

“Stem cells do have the potential to revolutionize things, but it is not “just around the corner,'” said Constantino. “You can’t just inject ‘fat’ stem cells into a breast and just assume that it’s going to make a nice-looking breast. You could just end up with something fairly lumpy and unappealing.”

The cosmetic applications of stem cells are “25 to 30 years away, at the earliest,” said Thoru Pederson of the department of biochemistry and molecular biology at the University of Massachusetts Medical School in Worcester, Mass.

Yet some studies are already under way.
“We are starting to see clinical trials with stem cells for reconstructive surgery,” Rubin said. “A group from Japan reported on enriching liposuctioned fat with fat-derived stem cells and using the material successfully for breast enlargement.”

Cosmetic Uses of Stem Cells a Low Priority
Most experts agree, however, that many other potentially curative and life-saving applications of stem cells take precedence over cosmetic uses.

“Applications to rejuvenation or enhanced personal appearance are much harder to justify at this point and will be driven more by market forces in affluent countries   not just the U.S. certainly    rather than by science,” Salomon said.

“In my opinion, use of any cells for cosmetic surgery is still problematic,” said Dr. Darwin Prockop, director of the Center for Gene Therapy at Tulane University Health Sciences Center in New Orleans. “The trials that can be justified are in patients with terminal diseases in which the potential risks and benefits are carefully evaluated.”

“In all honesty, the more promising (and more quickly realized) aspects of stem cell use in plastic and reconstructive surgery will probably be in producing skin replacement grafts on a large scale,” Constantino said. “This could help many, many burn and chronic wound patients.”

But for now?
“Though there is an enormous amount of promise with stem cells in plastic and reconstructive surgery, the devil is in some pretty important details,” Constantino said.

The flowers are yellow, tubular, 3 to 5 cm (1.2 to 2.0 in) long, with a four-lobed mouth. The flowers may vary in colour with some being white, blue or purple.

Sesame fruit is a capsule, normally pubescent, rectangular in section and typically grooved with a short triangular beak. The length of the fruit capsule varies from 2 to 8 cm, its width varies between 0.5 to 2 cm, and the number of loculi from 4 to 12. The fruit naturally splits opens (dehisces) to release the seeds by splitting along the septa from top to bottom or by means of two apical pores, depending on the varietal cultivar. The degree of dehiscence is of importance in breeding for mechanised harvesting as is the insertion height of the first capsule.

Sesame seeds are small. The size, form and colours vary with the thousands of varieties now known. Typically, the seeds are about 3 to 4 millimeters long by 2 millimeters wide and 1 millimeter thick. The seeds are ovate, slightly flattened and somewhat thinner at the eye of the seed (hilum) than at the opposite end. The weight of the seeds are between 20 and 40 milligrams. Sesame seeds come in many colours depending on the cultivar harvested. The most traded variety of sesame is off-white coloured. Other common colours are buff, tan, gold, brown, reddish, gray and black.

Sesame seed is sometimes sold with its seed coat removed (decorticated). This is the variety often present on top of buns in developed economies

African slaves brought sesame seeds, which they called benné seeds, to America, where they became a popular ingredient in Southern dishes.

Sesame seeds can be sprinkled on breads or on main dishes and vegetables to add a mild nutty flavor.
Tahini is a paste made of ground sesame seeds which is used in many Near and Far East recipes. You can purchase it prepared in most markets, or make your own.

Sesame seed oil is still the main source of fat used in cooking in the Near and Far East.

Sesame Seeds is neither a herb or a spice but one of the oldest annual plants grown for its seeds and oil. It is native to Africa and Asia but today is grown in China, India, Mexico and southwest United States as a commercial crop.

Sesame seeds come in a variety of colors depending on the plant variety, including shades of brown, red, black, yellow, and most commonly, a pale grayish ivory. The darker seeds are said to be more flavorful.

Cultivation:
Sesame is grown in many parts of the world on over 5 million acres (20,000 km²). The largest producer of the crop in 2007 was China, followed by India, Myanmar, Sudan, Ethiopia, Uganda and Nigeria. Seventy percent of the world’s sesame crop is grown in Asia, with Africa growing 26%.

Beginning in the 1950s, U.S. production of the crop has been largely centered in Texas, with acerage fluctuating between 10,000 to 20,000 acres (40 to 80 km²) in recent years. The country’s crop does not make up a significant global source; indeed imports have now outstripped domestic production

Aroma and Flavour: In spite of their high oil content, sesame seeds have little aroma, but when they are dry-fried their nutty aroma is very pronounced and their flavor heightened.

Culinary Use: Sesame oil is used in margarines and as a cooking medium and a flavouring ingredient. The seeds are ground to an oily, beige-coloured paste known as tahini, which is used in hummus, a Middle Eastern dip. Sometimes the tahini is mixed with lemon juice and gralic and used as a dip with hot pitta bread as starter or picnic food.

The Chinese are fond of sesame; sesame oil is widely used in Chinese cooking as a flavouring. The seeds are also used, for example sesame prawn toasts are scattered with seeds before they are deep-fried. They are also sprinkled over Chinese toffee apples, pieces of apple fried in a light batter and coated in caramel. Both oil and seeds are sued in the cooking of other Far Eastern countries such as Singapore, Malaysia and Indonesia. Gomasio is a Japanese specialty using sesame seeds: s mixture of the ground seeds and salt sued as a seasoning.

The seeds are popular scattered on bread, sweet and savoury biscuits, particularly in Greece and Turkey.

Medicinal and Other Use: Sesame is used in laxatives, as an emollient and in poultices. Sesame oil, also called gingelly oil, is highly stable and it does not become rancid quickly in hot humid conditions; it is used in lubricants, soap, cosmetics and ointments. The mixture or ‘cake’ that remains after the pressing of the oil is full of protein and eaten as a subsistence food.
Sesame is a member of the Pedaliaceae family. It is native to tropical Asian countries. The sesame plant can grow to a height of three feet and is an annual herb. It is an erect plant covered in fine hair and has a square stem. The leaves are flat, lanceolate in shape and grow in clusters of twos and threes. The flowers are pinkish purple in color or white and are bell shaped. Sesame is planted in the month of May and is harvested by fall or autumn. The name sesame is derived from Middle English sisame and from the Latin sesamum.

Interestingly, nutrients from one seed to another vary, but they all contain protein, oils (oleic acid, liuoleic acid, palmitoleic acid, araehidic acid and tetracosanoic acid) lecithin, minerals (Ca, P, K, Fe) saccharide, cellulose, VB2, VE, niacin, folic acid, sterol, sesamd, sesamin and cytochrome C. Unhulled seeds contain more calcium then hulled seeds.

Sesame seeds may be the oldest condiment known to man dating back to as early as 1600 BC. They are highly valued for their oil which is exceptionally resistant to rancidity. “Open sesame,” the famous phrase from the Arabian Nights, reflects the distinguishing feature of the sesame seed pod, which bursts open when it reaches maturity. The scientific name for sesame seeds is Sesamun indicum.

Sesame is supposed to tonify kidney, liver and relax the bowel. It is used for the treatment of constipation due to hard stools, tinnitus, anaemia, clizziness and poor vision. Mix powdered toasted sesame seeds with ground tuckahoe. Stir one to two teaspoonful into warm water and take in the mornings.

Infuse the leaves in some hot boiling water and use this to gargle and treat inflamed membranes of the mouth. Use only after tea has cooled down.

In traditional Chinese medicine, black sesame seeds have sweet and neutral properties, and are associated with the Kidney and Liver meridians. They function to tonify yin jing and blood, moisten the intestines, and help build the spirit, or shen.

Women of ancient Babylon would eat halva, a mixture of honey and sesame seeds to prolong youth and beauty, while Roman soldiers ate the mixture for strength and energy .

Sesame seeds produce an allergic reaction in a small percentage of the general population (5-13 per 100).

There have been erroneous claims that sesame seeds also contain THC which may be detectable on random screening. This error stems from a misunderstanding of the commercial drug Dronabinol, a synthetic form of THC. The normal delivery mechanism for synthetic Dronabinol is via infusion into sesame oil and encapsulation into soft gelatin capsules. As a result some people are under the mistaken assumption that sesame oil naturally contains THC. In fact, THC, CBD, CBN and the other cannibinoids are unique to the Cannabis genus.

Sesame oil is used for massage and health treatments of the body in the ancient Indian ayurvedic system with the types of massage called abhyanga and shirodhara. Ayurveda views sesame oil as the most viscous of the plant oils and believes it may pacify the health problems associated with Vata aggravation.

Black sesame seeds are an extremely good source of calcium; studies have shown that one gram of seeds contains approximately 85 milligrams of calcium. Black sesame seeds also have high amounts of protein, phosphorous, iron and magnesium. In some patients, black sesame seeds are used to help patients recover from serious illnesses and fevers, treat constipation and promote regular bowel movements. Some practitioners recommend using black sesame seeds with polygonum to keep a person’s hair looking rich and dark.

No one on earth can escape the sounds of noise- an unwanted, disturbing sound that causes a nuisance in the eye of the beholder. Noise is a disturbance to the human environment that is escalating at such a high rate that it will become a major threat to the quality of human lives. In the past thirty years, noise in all areas, especially in urban areas, have been increasing rapidly. There are numerous effects on the human environment due to the increase in noise pollution.Although we attempt to set standards for some of the most major sources of noise, we often are unable to monitor them. Major sources of noise can be airplanes at takeoff and landing, and a truck just off the assembly line, yet we seem accept and enjoy countless other sounds, from hard rock music to loud Harley Davidson motor cycles. The following areas will be investigated in some detail; adolescent education, neural-effects, sleep, hearing damage, occupational environment, transportation, and physiological effects.

Most of society is now aware that noise can damage hearing. However, short of a threat that disaster would overtake the human race if nothing is done about noise, it is unlikely that many people today would become strongly motivated to do something about the problem. Yet, the evidence about the ill effects of noise does not allow for complacency or neglect.
