Definition
A radionuclide scan of the kidneys shows a picture of your kidneys while they are at work making urine.A kidney radionuclide scan, also called a kidney scan or renal scan, is a diagnostic imaging test that involves administering a small amount of radionuclide, also called a radioactive tracer, into the body and then imaging the kidneys with a gamma camera. The images obtained can help in the diagnosis and treatment of various kidney diseases and conditions. This test can be useful to evaluate infection, blockages, injury to the kidneys, and some causes of high blood pressure.

Precautions
A kidney scan requires the use of a radioactive material; therefore, patients who are pregnant or suspect they may be pregnant are cautioned not to have the test unless the benefits outweigh the risks. Women should inform their doctor if they are breast feeding. The doctor will recommend the woman stop breast feeding for a specified period of time, depending on the particular tracer and dose used.

Description
Kidney scans are performed either in a hospital nuclear medicine department or in an outpatient radiology or nuclear medicine facility. The patient is positioned in front of, or under, a gamma camera—a special piece of equipment that detects the radiation emitted from the body and produces an image. An intravenous injection of the radionuclide is administered. Immediately after the injection imaging begins, and, in most studies, the flow of blood to each kidney is evaluated. Serial images of the kidneys are obtained over a specified period of time, depending upon the particular radiopharmaceutical used. Kidney scans may be performed to determine the rate at which the kidneys are filtering a patient’s blood. These studies use a radiopharmaceutical called technetium DTPA (Tc99m DTPA). This radiopharmaceutical also can identify obstruction in the renal collecting system. To establish the function of the renal tubules, the radiopharmaceutical Technetium DMSA (Tc99m DMSA) is used.

A kidney scan ranges from 45 minutes to three hours in length, depending upon the goals of the test, but the test typically takes about an hour to an hour and a half. It is important to understand that kidney scans can reveal an abnormality, but they do not always identify the specific problem. They are very useful in providing information about how the various parts of the kidneys function, which, in turn, can assist in making a diagnosis.

Typically, posterior images are obtained but images are also obtained at oblique angles. If indicated, the patient may be positioned so that mobility of the kidney is demonstrated by sitting up or lying down for the images. If obstruction or renal function is being evaluated, a diuretic (drug to induce urination), such as Lasix, may be injected. If hypertension or renal artery sterosis is being evaluated, Captopril or Enalapril (ACE inhibitors) may be injected.
Preparation
No special preparation is necessary for a kidney scan. In some instances the patient may be required to drink additional liquids and to empty their bladder before the exam. If another nuclear medicine study was recently performed, the patient may have to wait for a specified period to avoid any interference from residual radioactivity in the body. The patient is instructed to remove metal items from the area to be scanned.

Let your doctor know if you could be pregnant or if you are breast-feeding a baby. The medicine used in this test would expose your baby to radiation.

What happens when the test is performed.
You have an IV (intravenous) line placed into a vein. A slightly radioactive version of a substance called sodium pertechnetate is injected through the IV. This substance helps your kidneys and urine show up on pictures.

A camera that is specially designed to detect radioactivity is placed against your back or abdomen. A number of pictures are taken over time. The camera itself does not expose you to any additional radiation, so the number of pictures is not harmful in any way. The test is usually completed within an hour.

Risk Factors:
Many people worry when they hear that the medicine used in this test is slightly radioactive. In truth, this test exposes you to a very small amount of radiation-no greater than that of routine xrays.

Moreover, Nuclear medicine procedures are very safe. Unlike some of the dyes that may be used in x-ray studies, radioactive tracers rarely cause side effects. There are no long-lasting effects of the tracers themselves, because they have no functional effects on the body’s tissues. If pharmaceuticals are injected these can temporarily raise or lower blood pressure, or cause one to urinate.

Aftercare
Patients can resume their normal daily activities immediately after the test. Most radioactive tracers are excreted through the urinary system, so drinking fluids after a kidney scan can help flush the tracer out of the body more quickly.

Results
The scan should reveal normal kidney function for the patient’s age and medical status, as well as show normal relative position, size, configuration, and location of the kidneys. Initial blood flow images should reflect that blood circulation to both kidneys is equal. Patients whose images suggest a space-occupying lesion or obstruction may require other imaging procedures, such as CT or ultrasound, to provide more information. Also, if the kidneys appear to be abnormal in size, have an unusual contour, or are unusually positioned, other imaging procedures may be required.

Fatty Fish-oil May Help Reduce Tumour

An omega 3 fatty acid found in fish oils reduced the size of tumours in mice and made a chemotherapy drug more potent while limiting its  harmful effects, Egyptian researchers reported.

The findings, published in publisher BioMed Central‘s peer-reviewed Cell Division journal, add to evidence showing a range of health benefits from eating the fatty acids found in foods such as salmon. A.M. El-Mowafy and colleagues from Mansoura University in Egypt looked at how an omega 3 fatty acid called docosahexanoic acid, or DHA, affected solid tumours growing in mice and how well it interacted with the chemotherapy drug cisplatin.

“Our results suggest a new, fruitful drug regimen in the management of solid tumours based on combining cisplatin and possibly other chemotherapeutics with DHA,” El-Mowafy said in a statement. “DHA elicited prominent chemo-preventative effects on its own, and appreciably augmented those of cisplatin as well.” In March, U.S. researchers showed that a diet high in omega 3 fatty acids– the kind found in fish such as salmon, mackerel, herring and sardines- protected against advanced prostate cancer even in men more at risk of the disease.

The fatty acids, also found in foods such as walnuts and leafy greens, have been shown to provide an anti-inflammatory effect and have been linked to a lower risk of heart disease. In their study, El-Mowafy’s team found that, at the molecular level, DHA reduces the accumulation of white blood cells, systemic inflammation, and a harmful condition marked by decreased antioxidant levels- all of which have been linked to tumour growth. Their experiment also showed that the fatty acid reduced toxicity and injury to kidney tissue caused by the chemotherapy drug, the researchers said.

Non-Essential Amino Acid Glycine Can Improve ATP Production

The non-essential amino acid glycine is needed to generate muscle tissue and also for the conversion of blood glucose into energy. It is referred to as being ‘non-essential’ because the body can manufacture its own glycine, and is therefore not an essential component of your diet. Other uses to which glycine is put by the body includes the maintenance of a healthy nervous system, and is necessary for the proper functioning of the digestive system.

Amino acids play three essential roles in the human body:

1. They are the building blocks of proteins: proteins comprise about half of the dry weight of the majority of your body cells, and without them there would be no life. They are produced using monomers known as amino acids, and there are about 20 different amino acids used to make the vast variety of proteins that make up the human body. Proteins are needed to form enzymes, the catalysts that permit the majority of chemical reactions within our bodies, and also genes, the building blocks of DNA.

2. More relevant here, amino acids play an important role in the production of ATP (adenosine triphosphate) from ADP (adenosine diphosphate) by phosphorylation with creatine phosphate. The more creatine phosphate available, the more ATP can be produced. Since ATP is the molecule responsible for the generation of energy, then the more ATP available the more energy is generated. Although creatine is available from many food sources, it is destroyed by cooking, and over half of what you use is made from the three amino acids, glycine, arginine and methionine. The energy produced in this way is very short-lived, and last only a few seconds – more on that later.

3. Glycine is heavily involved in the production of collagen, which is the substance that maintains the flexibility of your skin and other connective tissues while still maintaining their strength and firmness. Without glycine your skin would become slack due to the degrading effect of sunlight, free radicals and oxidation.

The non essential amino acid, glycine, is believed to offer other benefits to the human body, but it is the second of those above, the production of ATP, which interests us here. ATP is an extremely important nanomolecule, second in importance to the body only to DNA, and possibly also RNA since the two are linked. RNA makes copies of your DNA structure for use in cell division and growth.

When a cell expends energy for whatever reason, such as when I am typing this, or when your heart beats, or even when your liver synthesizes a protein, one of the phosphate groups is removed from the adenosine triphosphate molecule, and converts it to adenosine diphosphate (ADP). The ATP is then said to be ‘spent’, just as your energy is spent when you are tired and can exercise no more.

The ADP is then immediately reconverted to ATP in the mitochondria, a part of every cell in your body. A cell can contain hundreds, or even thousands, of mitochondria, the number depending upon that particular cell’s need for energy. Hence, cells in your muscles, or in your liver where most of the body’s chemistry takes place, contain thousands of mitochondria whereas those in your scalp contain a lot less. Once changed to ATP, a phosphate is again lost when energy is expended, and so the cycle continues.

Glucose is needed allow the ADP to be converted to ATP, hence the need for sugars, or the carbohydrates from which they are manufactured in your body. Each cell can contain up to a billion molecules of ATP, although the couch potatoes among you probably have a lot less! Your store of ATP molecules last about 2 to 5 seconds before being changed to ADP although more rapidly for athletes that expend a lot of energy. Then the energy stored in the form of glycogen in the liver kicks in for another 4 – 6 seconds.

Additionally, you cannot expend more energy that the (eventual) sugars that you take in your diet, which can be in the form of ordinary ‘sugar’ (sucrose), fruit (fructose), glucose, carbohydrates that are metabolized into sugars, or any other member of the sugar family (e.g. lactose, maltose, etc.).

Glycine is one of what are called glucogenic amino acids, which refers to their ability to provide glucose to the blood. Because it helps to maintain proper blood glucose levels, it is often prescribed for conditions that are caused by low glucose levels, such as hypoglycemia that shows symptoms of fatigue and tiredness, and also anemia and what is known as CFS (chronic fatigue syndrome).

The one activity of the human body, in fact that of any mammal, for which ATP is essential, is the heartbeat. Without that no mammal could survive, or any other creature that relies on a circulation system for life. The only reason you heart has to beat is to pump your blood around your body, and it is your blood that contains the oxygen and nutrients needed to sustain life. Your cardiovascular health relies on lots of ATP being available to power each and every heartbeat.

Analysis of the heart during the final stages of heart failure has revealed that there is a general decrease in the myocardial arginine: glycine amidinotraferase (AGAT) gene expression, which is indicative of the necessity of this enzyme for proper heart function. The enzyme is responsible for the first stage in the biosynthesis of creatine from glycine.

Creatine is well known to athletes, and while it is available naturally from some food sources, it can be destroyed during cooking, and at least 50% of the creatine needed by the body is produced in the liver, pancreas and kidneys. It is creatine phosphate that is broken down into creatine and phosphate, the latter of which is used by the mitochondria to regenerate ATP from ADP.

The study carried out on the reduced AGAT levels found in heart failure patients indicates the importance of glycine to heart health. Without a good supply of glycine, there will insufficient creatine produced biochemically to generate the phosphate needed to for the ATP to produce the energy required to keep the heart pumping with the required strength. The energy provided by the mitochondria is used locally by the cells in which it is produced, and within a few seconds of that production. As explained earlier, ATP stores are used up within 2 – 5 seconds, and glycogen stores within another 4 – 6 seconds.

That is why sprinters cannot keep running at maximum speed for more than around 10 seconds or so, because the immediate availability of glycine, and hence creatine, are insufficient to last longer than that. That is one reason why they have to finish those 100 meters as fast as possible, because otherwise they would run out of energy. Other than trying to win, of course!

However, when it comes to the heart, ATP stores are essential, and the cells in your heart require a constant supply of ATP from creatine, which itself depends upon your intake or biosynthesis of glycine. Since dietary sources are insufficient to meet all your needs, and destroyed by cooking, a glycine supplement is the only way to ensure a sufficient intake. You cannot undernourish your heart and remain healthy.

ATP biosynthesis is essential if that of glycine theoretically is not, but the fact that 50% of your glycine requirement has to be produced by your body and the other 50% is sensitive to heat during cooking, a supplement of glycine could be essential to many people.

Linseed Food to Keep Cancer at Bay

Linseed is believed to protect against cancer, but the taste is a real turn-off for most people.

Researchers have now isolated the valuable components of linseed (flax seeds), which when mixed with bread, cakes or dressings, don’t leave an unpleasant aftertaste

Cakes that can ward off cancer or noodles that lower the cholesterol level could soon be a reality. Scientists at the Fraunhofer Institute for Process Engineering and Packaging IVV in Freising (Germany) have isolated valuable components of linseed and lupin seeds.

They have experimentally incorporated them in various foodstuffs: the linseed in cakes, bread, dressings and sauces, the lupins in bread, rolls and pasta. The result is not only delicious, but healthy as well.

“Flax is not only high in soluble fibre, but also contains lignans. These substances are phytoestrogens, so they have a similar effect to that of the isoflavones that we know from soya beans,” said IVV project manager Katrin Hasenkopf.

“According to literature, they protect the organism against hormone-dependent forms of cancer – that is, breast and prostate cancer,” added Hasenkopf. “The lupins, on the other hand, contain substances that our studies have found to have a positive impact on the cholesterol level.”

But how do the researchers isolate the valuable components? “We make use of the differing solubility of the various constituents: If the pH value is acidic, the unwanted bitter substances are the first to dissolve.

pH value is a way of expressing the acidity or alkalinity of a solution. The neutral point is pH value 7.0, with acids having lower values and alkalis having higher values.

“If the pH value is then set back to neutral, you get the valuable proteins – without the bitter taste. We are also able to separate large components from small ones by a series of filtration steps,” explained Hasenkopf.

“The healthy effects of linseed and lupin seeds are already known from literature, but so far there is a lack of conclusive scientific investigations on the subject. These substances undoubtedly have very high potential,” she said.

In about three years, the new cholesterol-lowering foodstuffs are expected to be available on supermarket shelves – maybe even including cakes, bread rolls and sauces enriched with the valuable substances obtained from flax seeds.

The researchers will be presenting the linseed and lupin foods at the Biotechnica trade fair in Hanover on Oct 7-9.

Habitat :Taxus Brevifolia  is native to  Europe, incl Britain, from Scandanavia south and east to N. Africa, the Caucasus, Iran, Himalayas.   It grows in woods and scrub, usually on limestone. It sometimes forms pure stands in sheltered sites on chalk in the south-east and on limestone in the north-west.  

Description: It is a small to medium-sized evergreen tree, growing 10-15 m tall and with a trunk up to 50 cm diameter, rarely more. forming with age a very stout trunk covered with red-brown, peeling bark and topped with a rounded or wide-spreading head of branches; leaves spirally attached to twigs, but by twisting of the stalks brought more or less into two opposed ranks, dark, glossy, almost black-green above, grey, pale-green or yellowish beneath, 1/2 to 1 1/2 inches long, 1/16 to 1/12 inch wide. Flowers unisexual, with the sexes invariably on different trees, produced in spring from the leaf axils of the preceding summer’s twigs. Male, a globose cluster of stamens; female, an ovule surrounded by small bracts, the so-called fruit bright red, sometimes yellow, juicy and encloses the seed.

It has thin scaly brown bark. The leaves are lanceolate, flat, dark green, 1-3 cm long and 2-3 mm broad, arranged spirally on the stem, but with the leaf bases twisted to align the leaves in two flat rows either side of the stem except on erect leading shoots where the spiral arrangement is more obvious.

The seed cones are highly modified, each cone containing a single seed 4-7 mm long partly surrounded by a modified scale which develops into a soft, bright red berry-like structure called an aril, 8-15 mm long and wide and open at the end. The arils are mature 6-9 months after pollination, and with the seed contained are eaten by thrushes and other birds, which disperse the hard seeds undamaged in their droppings; maturation of the arils is spread over 2-3 months, increasing the chances of successful seed dispersal. The male cones are globose, 3-6 mm diameter, and shed their pollen in early spring. It is mostly dioecious, but occasional individuals can be variably monoecious, or change sex with time.

No tree is more associated with the history and legends of Great Britain than the Yew. Before Christianity was introduced it was a sacred tree favoured by the Druids, who built their temples near these trees – a custom followed by the early Christians. The association of the tree with places of worship still prevails.

Many cases of poisoning amongst cattle have resulted from eating parts of the Yew.

Cultivation:
Landscape Uses:Espalier, Firewood, Hedge, Screen, Standard, Superior hedge, Specimen. A very easy plant to grow, it is extremely tolerant of cold and heat, sunny and shady positions, wet and dry soils, exposure and any pH[200]. Thrives in almost any soil, acid or alkaline, as long as it is well-drained. Grows well in heavy clay soils. Sensitive to soil compaction by roads etc. Very shade tolerant. Tolerates urban pollution.  In general they are very tolerant of exposure, though plants are damaged by severe maritime exposure. A very cold hardy plant when dormant, tolerating temperatures down to about -25°c. The fresh young shoots in spring, however, can be damaged by frosts. Plants are dioecious, though they sometimes change sex and monoecious trees are sometimes found. Male and female trees must be grown if fruit and seed is required. The fruit is produced mainly on the undersides of one-year old branches. A very long lived tree, one report suggests that a tree in Perthshire is 1500 years old, making it the oldest plant in Britain. Another report says that trees can be up to 4000 years old. It is, however, slow growing and usually takes about 20 years to reach a height of 4.5 metres. Young plants occasionally grow 30cm in a year but this soon tails off and virtually no height increase is made after 100 years. A very ornamental tree, there are many named varieties. Very resistant to honey fungus, but susceptible to phytopthera root rot. The bark is very soft and branches or even the whole tree can be killed if the bark is removed by constant friction such as by children climbing the tree. Plants produce very little fibrous root and should be planted in their final positions when still small. The fruit is greatly relished by thrushes. Special Features: Not North American native, All or parts of this plant are poisonous, Inconspicuous flowers or blooms.

Propagation:
Seed – can be very slow to germinate, often taking 2 or more years. It is best sown as soon as it is ripe in the autumn when it should germinate 18 months later. Stored seed may take 2 years or more to germinate. 4 months warm followed by 4 months cold stratification may help reduce the germination time. Harvesting the seed ‘green’ (when fully developed but before it has dried on the plant) and then sowing it immediately has not been found to reduce the germination time because the inhibiting factors develop too early. Prick out the seedlings into individual pots once they are large enough to handle and grow them on in pots in a cold frame. The seedlings are very slow-growing and will probably require at least 2 years of pot cultivation before being large enough to plant out. Any planting out is best done in late spring or early summer, after the last expected frosts. Cuttings of half-ripe terminal shoots, 5 – 8cm long, July/August in a shaded frame. Should root by late September but leave them in the frame over winter and plant out in late spring. High percentage. Edible Uses:

Fruit – raw. Very sweet and gelatinous, most people find it delicious though some find it sickly. A number of people who like the flavour do not like the texture which is often described as being ‘snotty’. All other parts of this plant, including the seed, are highly poisonous. When eating the fruit you should spit out the large seed found in the fruit’s centre. Should you swallow the whole seed it will just pass straight through you without harm (UPDATE: this is probably not true: unfortunately, the digestive system of most mammals, including humans, is robust enough to break down the seeds. This will release the toxic taxanes. Birds are able to eat the whole “berry” because they cannot digest the seeds). If it is bitten into, however, you will notice a very bitter flavour and the seed should immediately be spat out or it could cause some problems. The fruit is a fleshy berry about 10mm in diameter and containing a single seed. Some reports suggest using the bark as a tea substitute, this would probably be very unwise.

Constituents: The fruit and seeds seem to be the most poisonous parts of the tree. An alkaloid taxine has been obtained from the seeds; this is a poisonous, white, crystalline powder, only slightly soluble in water; another principle, Milossin, has also been found.

Medicinal Uses:

The yew tree is a highly toxic plant that has occasionally been used medicinally, mainly in the treatment of chest complaints. Modern research has shown that the plants contain the substance ‘taxol’ in their shoots. Taxol has shown exciting potential as an anti-cancer drug, particularly in the treatment of ovarian cancers. Unfortunately, the concentrations of taxol in this species are too low to be of much value commercially, though it is being used for research purposes. This remedy should be used with great caution and only under the supervision of a qualified practitioner. See also the notes below on toxicity. All parts of the plant, except the fleshy fruit, are antispasmodic, cardiotonic, diaphoretic, emmenagogue, expectorant, narcotic and purgative. The leaves have been used internally in the treatment of asthma, bronchitis, hiccup, indigestion, rheumatism and epilepsy. Externally, the leaves have been used in a steam bath as a treatment for rheumatism. A homeopathic remedy is made from the young shoots and the berries. It is used in the treatment of many diseases including cystitis, eruptions, headaches, heart and kidney problems, rheumatism etc. Ingestion of 50-100g of needles can cause death.

The chemotherapy drug paclitaxel, used in breast, ovarian and lung cancer treatment, is derived from Taxus brevifolia. Over-harvesting for production of this drug has resulted in the Pacific Yew becoming a rare species, despite the fact the drug can be produced semi-synthetically from cultivated yews. Pharmaceutical use of closely-related wild yew species in India and China threatens some of those species as well.

Other Uses:  
Very tolerant of trimming, this plant makes an excellent hedge. The plants are often used in topiary and even when fairly old, the trees can be cut back into old wood and will resprout. One report says that trees up to 1000 years old respond well to trimming. A decoction of the leaves is used as an insecticide. Some cultivars can be grown as a ground cover when planted about 1 metre or more apart each way. ‘Repandens’ has been recommended. Wood – heavy, hard, durable, elastic, takes a good polish but requires long seasoning.  Highly esteemed by cabinet makers, it is also used for bows, tool handles etc. It makes a good firewood. The wood is burnt as an incense

Known Hazards :  All parts of the plant, except the flesh of the fruit, are highly poisonous, having a paralyzing affect on the heart. Poisoning symptoms are dry mouth, vomiting, vertigo, abdominal pain, dyspnoea, arrhythmias, hypotension & unconsciousness.
