Definition:
Rheumatism or Rheumatic disorder is a non-specific term for medical problems affecting the heart, bones, joints, kidney, skin and lung. The study of, and therapeutic interventions in, such disorders is called rheumatology.The term “rheumatism” is still used in colloquial speech and historical contexts, but is no longer frequently used in medical or technical literature; it would be fair to say that there is no longer any recognized disorder simply called “rheumatism.” Some countries use the word Rheumatism to describe fibromyalgia syndrome. The traditional term covers such a range of different problems that to ascribe symptoms to “rheumatism” is not to say very much. Nevertheless, sources dealing with rheumatism tend to focus on arthritis. However, “non-articular rheumatism”, also known as “regional pain syndrome” or “soft tissue rheumatism” can cause just as much discomfort and difficulty. Furthermore, arthritis and rheumatism between them cover at least 200 different conditions.

Rheumatism is a medical term once frequently used to describe disorders associated with many different parts of the body. Most often, people associate rheumatism with arthritis, or with rheumatic fever, a complication of strep throat that can result in damage to the heart. However, the term rheumatism might apply to the symptoms of numerous conditions that can cause pain and/or weakness.

Some conditions that were once given the general label of rheumatism or called rheumatic diseases include, rheumatoid arthritis, osteoarthritis, lupus, fibromyalgia, and tendonitis. Frequently autoimmune disorders, since they remained unnamed but caused pain as well as affecting other organs, were classed as rheumatism. Illnesses like lupus were particularly susceptible to being called rheumatism. Later understanding of the actions of these illnesses show that the problem is not dysfunction of the joints, but rather immune systems that can attack joints, muscles and organs.

Some forms of rheumatism are called non-articular rheumatism and may affect the soft tissues causing pain throughout the body. Conditions like tendonitis and fibromyalgia fall into this category. As well, non-articular rheumatism can be localized to specific areas in the body. Bursitis is a non-articular form of rheumatism that affects and inflames the bursa, which are special sacs that protect joints and overlapping muscles. Bursitis most frequently occurs at the site of one joint that may have been injured through overuse.

Other forms of non-articular rheumatism may also result from repetitive motion. Carpal Tunnel Syndrome is classed as non-articular rheumatism and is often caused by poor position when typing, or by positional problems when assembling multiple products of the same type.

Another type of non-articular rheumatism is temporomandibular joint syndrome (TMJ), which only affects the joints in the jaws. Numerous people are treated yearly, to help prevent the mouth from getting stuck when open or closed, or the painful popping and clicking that may be associated with moving the jaws.

The general term rheumatism is seldom heard now in medical communities because health professionals feel that specific naming of illnesses can better point toward standards or treatment and care. Treating lupus is hugely different from treating bursitis or TMJ. With more specified names comes specified research that can help determine a range of information about an illness. Overly general terms lack the specificity required to define the action of a condition, which best directs effective treatment.

Types
The major rheumatic disorders currently recognised include:

* Ankylosing spondylitis

Although these disorders probably have little in common in terms of their epidemiology, they do share two characteristics: they cause chronic (though often intermittent) pain, and they are difficult to treat. They are also, collectively, very common.

Rheumatism symptoms:

Fever, pain, intense soreness and stiffness
The onset of the acute variety of rheumatism is characterized by fever, intense soreness, and pain. In the acute muscular type, the area becomes so sensitive that even the weight of bed clothing aggravates the pain. It may settle into a chronic state under a wrong mode of treatment. If the disease is not treated properly in the acute stage, it may become chronic. The symptoms of chronic muscular rheumatism are pain and stiffness of the affected muscles. In the case of chronic articular rheumatism (pain in the joints), pain and stiffness are felt in one or more joints of the body, with swelling in most cases

Rheumatism causes:

Toxic waste products in the blood
The chief cause of rheumatism is the presence of toxic waste products in the blood. The liberal consumption of meat, white bread, sugar, and refined cereals leaves a large residue of toxic wastes in the system. When the vitality is low, the toxic wastes are concentrated around the joints and bony structure, where they form the basis of rheumatism.

“Rheumatism” and weather

There has long been said to be a link between “rheumatic” pain and the weather. There appears to be no firm evidence in favour or against, but a 1995 questionnaire given to 557 people by A. Naser and others at the Brigham and Women’s Hospital’s Pain Management Center concludes that “changes in barometric pressure are the main link between weather and pain. Low pressure is generally associated with cold, wet weather and an increase in pain. Clear, dry conditions signal high pressure and a decrease in pain

Treatment:
A vast number of traditional herbal remedies were recommended for “rheumatism”. Modern medicine, both conventional and complementary, recognises that the different rheumatic disorders have different causes (and several of them have multiple causes) and require different kinds of treatment.

Nevertheless, initial therapy of the major rheumatological diseases is with analgesics, such as paracetamol and non-steroidal anti-inflammatory drugs (NSAIDs), members of which are ibuprofen and diclofenac. Often, stronger analgesics are required.

Home Remedies for Rheumatism:

Rheumatism treatment using Potato Juice
The juice of raw potato is regarded as an excellent remedy fur rheumatism. One or two teaspoons of the juice, taken out by pressing mashed raw potatoes, should be taken before meals. This will help to eliminate the toxic condition and relieve rheumatism. The skin of the potato is also an excellent remedy fur rheumatism. The skin is exceptionally rich in vital mineral salts, and the water in which the peelings are boiled is one of the best medicines for ailments caused by excess toxic matter in the system. Approximately thirty grams of the potato peelings should be thoroughly washed and boiled in half a litre of water till it is reduced to half. The decoction should then be strained and a glass of the same should be taken three or four times daily.
Rheumatism treatment using Bitter Gourd
Bitter gourd is considered beneficial in the treatment of rheumatism. A cup of juice, extracted from the vegetable, should be mixed with a teaspoon of honey, and taken daily for treating this condition. This treatment should be continued for at least three months to provide relief
Rheumatism treatment using Celery

Celery is another effective remedy for rheumatism. A fluid extract of the seeds is more powerful than the raw vegetable. This also has a tonic action on the stomach and kidneys. Five to ten drops of this fluid should be taken in hot water before meals. Powdered seeds can be used as a condiment
Rheumatism treatment using Lemon

Lemons are beneficial in the treatment of rheumatism. The patient should take the juice of two or three lemons each day. This will bring good results
Rheumatism treatment using Walnuts

Walnuts are valuable in rheumatism. They should, however, be thoroughly masticated to achieve beneficial results. Half a dozen can be taken daily in the treatment of this condition
Rheumatism treatment using Rhubarb

The herb rhubarb has been found valuable in rheumatism. The green stalks of this herb should be pounded with an equal quantity of sugar. A teaspoonful should be taken three or four times a day. This remedy seldom fails

Other Rheumatism treatment:
Warm-water enema
In the case of acute rheumatism, the bowels should be cleansed daily with a warm-water enema during the first three or four days of the juice fast
Appply heat and hot packs to the affected parts

Other helpful methods in the treatment of rheumatism are application of heat and hot packs to the affected parts, a hot tub bath, a cabinet steam bath, dry friction, and a sponge bath
Hot Epsom salts bath

Hot Epsom salts baths are also beneficial and should be taken twice a week for three months in case of chronic rheumatism and once weekly thereafter. The affected parts should also be bathed twice daily in hot water containing Epsom salts, after which some olive oil should be applied
Fresh air exposure and light outdoor exercises

Fresh air, deep breathing, and light outdoor exercises are also beneficial
Avoid dampness and cold

Dampness and cold should be avoided

Rheumatism diet

Orange juice and water
In the case of acute rheumatism, the patient should be put on a short fast of orange juice and water for three or four days. After the juice fast, the patient should be placed on a restricted diet for fourteen days. In this regimen, orange or grapefruit may be taken for breakfast; lunch may consist of raw salad of seasonal vegetables with raisins, prunes, figs, or dates; and dinner may comprise of one or two steamed vegetables
Well-balanced diet

Thereafter, the patient may gradually adopt a well-balanced diet consisting of seeds, nuts, grains, vegetables, and fruits. In case of chronic rheumatism, the patient may be placed on an all-fruit diet for four or five days. He may, thereafter, gradually adopt a well-balanced diet. The patient should take ripe fruits, fresh vegetables, and buttermilk in abundance
Avoid meat, indigestible and highly-seasoned foods

He should avoid all meat and fish; white bread, sugar, and refined cereals; rich, indigestible and highly-seasoned foods; tea and coffee; alcohol; sauces, pickles, and condiments.

Pseudogout

Definition:
Pseudogout is a form of arthritis that occurs when a particular type of calcium crystal accumulates in the joints. As more of these crystals are deposited in the affected joint, they can cause a reaction that leads to severe pain and swelling. The swelling can be either short-term or long-term and occurs most frequently in the knee, although it can also affect the wrist, shoulder, ankle, elbow, or hand. The pain caused by pseudogout is sometimes so excruciating that it can incapacitate someone for days.

It is a type of arthritis that, as the name implies, can cause symptoms similar to gout, but in reaction to a different type of crystal deposit.

As its name suggests, the symptoms of pseudogout are similar to those of gout (see “Gout“). Pseudogout can also resemble osteoarthritis or rheumatoid arthritis. A correct diagnosis is vital, as untreated pseudogout can lead to joint degeneration and osteoarthritis. Pseudogout is most common in the elderly, occurring in about 3% of people in their 60s and as many as half of people in their 90s.

Causes:
The cause of this condition is unknown. Because risk increases significantly with age, it is possible that the physical and chemical changes that accompany aging increase susceptibility to pseudogout.

Pseudogout develops when deposits of calcium pyrophosphate crystals accumulate in a joint. Crystals deposit first in the cartilage and can damage the cartilage. The crystals also can cause a reaction with inflammation that leads to joint pain and swelling. In most cases it is not known why the crystals form, although crystal deposits clearly increase with age. Because the condition sometimes runs in families, genetic factors are suspected of contributing to the disorder as can a severely underactive thyroid (hypothyroidism), excess iron storage (hemochromatosis), low magnesium levels in blood, an overactive parathyroid gland, and other causes of excessive calcium in the blood (hypercalcemia).

Pseudogout also can be triggered by joint injury, such as joint surgery or a sprain, or the stress of a medical illness. If the underlying condition causing pseudogout can be identified and treated, it may be possible to prevent future attacks. Frequently, however, there is no identifiable trigger; in those cases there is no way to prevent pseudogout from recurring.


Who gets pseudogou

The calcium crystal deposits seen in pseudogout affect about 3 percent of people in their 60s and as many as 50 percent of people in their 90s. Any kind of insult to the joint can trigger the release of the calcium crystals, inducing a painful inflammatory response. Attacks of pseudogout also can develop following joint surgery or other surgery. However, not everyone will experience severe attacks.

Symptoms:
* pain, swelling, and stiffness around a single joint
* occasionally, more then one joint affected at a time
* fever, usually low-grade

Diagnosis:

It may be difficult to diagnose pseudogout because it shares so many symptoms with gout, infection, and other causes of joint inflammation. In fact, pseudogout often occurs in people with other joint problems, such as osteoarthritis. Therefore, even when pseudogout is correctly identified, it is important to investigate whether there are other conditions present as well.

Diagnosis is to be done on the basis of symptoms and medical tests. The physician will use a needle to take fluid from a swollen or painful joint to determine whether calcium pyrophosphate crystals are present.This is done with a needle, after applying a numbing medication to the joint.This joint fluid is then analyzed for evidence of calcium crystals, inflammation, or infection. Your doctor may also order tests for other conditions that can trigger pseudogout, including tests of calcium and thyroid function.

An X-ray of the joint may be taken to determine whether calcium-containing deposits are present, creating a condition known as chondrocalcinosis. Other potential causes of symptoms, such as gout, rheumatoid arthritis, or infection, must be ruled out. Pseudogout often is present in people who have osteoarthritis.

Treatment:
To combat joint pain and swelling, your doctor may prescribe NSAIDs such as indomethacin and naproxen, or may give you glucocorticoid injections to keep the swelling down (see “Corticosteroid injections”). Your doctor may also remove fluid from the inflamed joint, a procedure called aspiration, as this may help to ease the pressure and inflammation.

The combination of joint aspiration and medication usually eliminates symptoms within a few days, although the doctor may also recommend treatment with oral corticosteroids over a short period of time. Daily use of a low-dose NSAID or colchicine, a medicine that is also used in the treatment of gout, may help to prevent further attacks. Unfortunately, there is no treatment available that can dissolve the calcium crystal deposits, although the joint degeneration that often goes along with pseudogout may be slowed by treatments that decrease joint swelling. Occasionally, people with recurrent or chronic pseudogout may develop osteoarthritis. In this case, surgery (such as joint replacement) may be the only effective treatment.


Prevention:

It is not known how to prevent pseudogout. If the condition has developed because of some other medical conditions, such as hemochromatosis (too much iron stored in the body), or parathyroid problems, treatment of that condition may prevent progression of other features of that potentially dangerous illness and may, in some cases, slow the development of pseudogout.

Points to Remember:
When a patient complains of joint pain, physicians often do not consider pseudogout because it can be confused with gout and other types of arthritis. Diagnosis is confirmed by microscopic identification of calcium pyrophosphate crystals. Anti-inflammatory agents can help lessen symptoms but there is currently no way to eliminate the crystals themselves.

Rheumatologists are actively engaged in research into the causes of pseudogout to better prevent and treat this form of arthritis. Because people with pseudogout tend to be older and more susceptible to side effects from anti-inflammatory medications, they benefit from seeing rheumatologists, who offer valuable expertise in using such drugs.

Rheumatologists are experts at diagnosing pseudogout and direct a team approach to the chronic, degenerative consequences of crystal deposits. This is important because the patient may need advice about surgery or may require additional information and support from physical and occupational therapists and nurses.

Arthritis

Definition:-
Arthritis means inflammation of the joints. Inflammation generally includes symptoms of redness, heat, swelling, and pain. Many different diseases can result in inflammation of the joints. Arthritis is therefore a general term that describes more than one hundred different diseases of the joints of your body.

There are different forms of arthritis; each has a different cause. The most common form of arthritis, osteoarthritis (degenerative joint disease) is a result of trauma to the joint, infection of the joint, or age. Emerging evidence suggests that abnormal anatomy might contribute to the early development of osteoarthritis. Other arthritis forms are rheumatoid arthritis and psoriatic arthritis, autoimmune diseases in which the body attacks itself. Septic arthritis is caused by joint infection. Gouty arthritis is caused by deposition of uric acid crystals in the joint, causing inflammation. There is also an uncommon form of gout caused by the formation of rhomboid crystals of calcium pyrophosphate. This gout is known as pseudogout.

In some types of arthritis, the cause of the disease is known, but in others it is still unknown. Some types of arthritis come on suddenly, and others develop slowly. Any joint can be affected, including your knees, hips, neck, shoulders, and fingers.

Causes:-

Arthritis involves the breakdown of cartilage. Cartilage normally protects the joint, allowing for smooth movement. Cartilage also absorbs shock when pressure is placed on the joint, like when you walk. Without the usual amount of cartilage, the bones rub together, causing pain, swelling (inflammation), and stiffness.

You may have joint inflammation for a variety of reasons, including:

*Broken bone

*Infection (usually caused by bacteria or viruses)

*An autoimmune disease (the body attacks itself because the immune system believes a body part is foreign)

*General “wear and tear” on joints

Often, the inflammation goes away after the injury has healed, the disease is treated, or the infection has been cleared.

With some injuries and diseases, the inflammation does not go away or destruction results in long-term pain and deformity. When this happens, you have chronic arthritis. Osteoarthritis is the most common type and is more likely to occur as you age. You may feel it in any of your joints, but most commonly in your hips, knees or fingers.

Risk factors for osteoarthritis include:-

*Being overweight

*Previously injuring the affected joint

*Using the affected joint in a repetitive action that puts stress on the joint (baseball players, ballet dancers, and construction workers are all at risk)

Arthritis can occur in men and women of all ages. About 37 million people in America have arthritis of some kind, which is almost 1 out of every 7 people.

Other types or cause of arthritis include:

*Rheumatoid arthritis (in adults)

*Juvenile rheumatoid arthritis (in children)

*Systemic lupus erythematosus (SLE)

*Gout

*Scleroderma

*Psoriatic arthritis

*Ankylosing spondylitis

*Reiter’s syndrome (reactive arthritis)

*Adult Still’s disease

*Viral arthritis

*Gonococcal arthritis

*Other bacterial infections (non-gonococcal bacterial arthritis )

*Tertiary Lyme disease (the late stage)

*Tuberculous arthritis

*Fungal infections such as blastomycosis

Symptoms:-

If you have arthritis, you may experience:

*Joint pain

*Joint swelling

*Stiffness, especially in the morning

*Warmth around a joint

*Redness of the skin around a joint

*Reduced ability to move the joint

Diagnosis:-

Exams and Tests :

First, your doctor will take a detailed medical history to see if arthritis or another musculoskeletal problem is the likely cause of your symptoms.

Next, a thorough physical examination may show that fluid is collecting around the joint. (This is called an “effusion.”) The joint may be tender when it is gently pressed, and may be warm and red (especially in infectious arthritis and autoimmune arthritis). It may be painful or difficult to rotate the joints in some directions. This is known as “limited range-of-motion.”

In some autoimmune forms of arthritis, the joints may become deformed if the disease is not treated. Such joint deformities are the hallmarks of severe, untreated rheumatoid arthritis.

Tests vary depending on the suspected cause. They often include blood tests and joint x-rays. To check for infection and other causes of arthritis (like gout caused by crystals), joint fluid is removed from the joint with a needle and examined under a microscope. See the specific types of arthritis for further information.

Treatment:-

Treatment of arthritis depends on the particular cause, which joints are affected, severity, and how the condition affects your daily activities. Your age and occupation will also be taken into consideration when your doctor works with you to create a treatment plan.

If possible, treatment will focus on eliminating the underlying cause of the arthritis. However, the cause is NOT necessarily curable, as with osteoarthritis and rheumatoid arthritis. Treatment, therefore, aims at reducing your pain and discomfort and preventing further disability.

It is possible to greatly improve your symptoms from osteoarthritis and other long-term types of arthritis without medications. In fact, making lifestyle changes without medications is preferable for osteoarthritis and other forms of joint inflammation. If needed, medications should be used in addition to lifestyle changes.

Exercise for arthritis is necessary to maintain healthy joints, relieve stiffness, reduce pain and fatigue, and improve muscle and bone strength. Your exercise program should be tailored to you as an individual. Work with a physical therapist to design an individualized program, which should include:

Range of motion exercises for flexibility
Strength training for muscle tone

Low-impact aerobic activity (also called endurance exercise)
A physical therapist can apply heat and cold treatments as needed and fit you for splints or orthotic (straightening) devices to support and align joints. This may be particularly necessary for rheumatoid arthritis. Your physical therapist may also consider water therapy, ice massage, or transcutaneous nerve stimulation (TENS).

Rest is just as important as exercise. Sleeping 8 to 10 hours per night and taking naps during the day can help you recover from a flare-up more quickly and may even help prevent exacerbations. You should also:

Avoid positions or movements that place extra stress on your affected joints.
Avoid holding one position for too long.
Reduce stress, which can aggravate your symptoms. Try meditation or guided imagery. And talk to your physical therapist about yoga or tai chi.
Modify your home to make activities easier. For example, have grab bars in the shower, the tub, and near the toilet.
Other measures to try include:

Taking glucosamine and chondroitin — these form the building blocks of cartilage, the substance that lines joints. These supplements are available at health food stores or supermarkets. While some studies show such supplements may reduce osteoarthritis symptoms, others show no benefit. However, since these products are regarded as safe, they are reasonable to try and many patients find their symptoms improve.
Eat a diet rich in vitamins and minerals, especially antioxidants like vitamin E. These are found in fruits and vegetables. Get selenium from Brewer’s yeast, wheat germ, garlic, whole grains, sunflower seeds, and Brazil nuts. Get omega-3 fatty acids from cold water fish (like salmon, mackerel, and herring), flaxseed, rapeseed (canola) oil, soybeans, soybean oil, pumpkin seeds, and walnuts.
Apply capsaicin cream (derived from hot chili peppers) to the skin over your painful joints. You may feel improvement after applying the cream for 3-7 days.

Medications:

Your doctor will choose from a variety of medications as needed. Generally, the first drugs to try are available without a prescription. These include:

Acetaminophen (Tylenol) — recommended by the American College of Rheumatology and the American Geriatrics Society as first-line treatment for osteoarthritis. Take up to 4 grams a day (2 extra-strength Tylenol every 6 hours). This can provide significant relief of arthritis pain without many of the side effects of prescription drugs. DO NOT exceed the recommended doses of acetaminophen or take the drug in combination with large amounts of alcohol. These actions may damage your liver.
Aspirin, ibuprofen, or naproxen –– these nonsteroidal anti-inflammatory (NSAID) drugs are often effective in combating arthritis pain. However, they have many potential risks, especially if used for a long time. They should not be taken in any amount without consulting your doctor. Potential side effects include heart attack, stroke, stomach ulcers, bleeding from the digestive tract, and kidney damage. In 2005, the U.S. Food and Drug Administration (FDA) asked makers of NSAIDs to include a warning label on their product that alerts users of an increased risk for heart attack, stroke, and gastrointestinal bleeding. If you have kidney or liver disease, or a history of gastrointestinal bleeding, you should not take these medicines unless your doctor specifically recommends them.
Prescription medicines include:

Cyclooxygenase-2 (COX-2) inhibitors — These drugs block an inflammation-promoting enzyme called COX-2. This class of drugs was initially believed to work as well as traditional NSAIDs, but with fewer stomach problems. However, numerous reports of heart attacks and stroke have prompted the FDA to re-evaluate the risks and benefits of the COX-2s. Celecoxib (Celebrex) is still available, but labeled with strong warnings and a recommendation that it be prescribed at the lowest possible dose for the shortest duration possible. Talk to your doctor about whether COX-2s are right for you.
Corticosteroids (“steroids”) — these are medications that suppress the immune system and symptoms of inflammation. They are commonly used in severe cases of osteoarthritis, and they can be given orally or by injection. Steroids are used to treat autoimmune forms of arthritis but should be avoided in infectious arthritis. Steroids have multiple side effects, including upset stomach and gastrointestinal bleeding, high blood pressure, thinning of bones, cataracts, and increased infections. The risks are most pronounced when steroids are taken for long periods of time or at high doses. Close supervision by a physician is essential.
Disease-modifying anti-rheumatic drugs — these have been used traditionally to treat rheumatoid arthritis and other autoimmune causes of arthritis. These drugs include gold salts, penicillamine, sulfasalazine, and hydroxychloroquine. More recently, methotrexate has been shown to slow the progression of rheumatoid arthritis and improve your quality of life. Methotrexate itself can be highly toxic and requires frequent blood tests for patients on the medication.
Biologics– these are the most recent breakthrough for the treatment of rheumatoid arthritis. Such medications, including etanercept (Enbrel), infliximab (Remicade) and adalimumab (Humira), are administered by injection and can dramatically improve your quality of life. Newer biologics include Orencia (abatacept) and Rituxan (rituximab).
Immunosuppressants — these drugs, like azathioprine or cyclophosphamide, are used for serious cases of rheumatoid arthritis when other medications have failed.
It is very important to take your medications as directed by your doctor. If you are having difficulty doing so (for example, due to intolerable side effects), you should talk to your doctor.

Surgery & Other Type of Approach:

In some cases, surgery to rebuild the joint (arthroplasty) or to replace the joint (such as a total knee joint replacement) may help maintain a more normal lifestyle. The decision to perform joint replacement surgery is normally made when other alternatives, such as lifestyle changes and medications, are no longer effective.

Normal joints contain a lubricant called synovial fluid. In joints with arthritis, this fluid is not produced in adequate amounts. In some cases, a doctor may inject the arthritic joint with a manmade version of joint fluid. The synthetic fluid may postpone the need for surgery at least temporarily and improve the quality of life for persons with arthritis.

Prognosis:
A few arthritis-related disorders can be completely cured with treatment. Most are chronic (long-term) conditions, however, and the goal of treatment is to control the pain and minimize joint damage. Chronic arthritis frequently goes in and out of remission.

Possible Complications:

*Chronic pain

*Lifestyle restrictions or disability

Call your doctor if:

*Your joint pain persists beyond 3 days.

*You have severe unexplained joint pain.

*The affected joint is significantly swollen.

*You have a hard time moving the joint.

*Your skin around the joint is red or hot to the touch.

*You have a fever or have lost weight unintentionally.

Prevention :

If arthritis is diagnosed and treated early, you can prevent joint damage. Find out if you have a family history of arthritis and share this information with your doctor, even if you have no joint symptoms.

Osteoarthritis may be more likely to develop if you abuse your joints (injure them many times or over-use them while injured). Take care not to overwork a damaged or sore joint. Similarly, avoid excessive repetitive motions.
