Families are scattered all over  the globe and they travel to stay in touch. Airplanes are safe, despite the high flying altitude, relatively lower partial pressure of oxygen, variable air circulation, low humidity, sustained periods of noise, vibration and turbulence.

The rapid changes that occur during a flight (typically during descent) can give rise to ear pain, a blocked feeling, ringing in the ears, giddiness, hearing loss or even rupture of the eardrum. These complications are more likely if the Eustachian tube (connecting the ear and throat) is blocked by allergy, colds, sinusitis or middle ear infections. Chewing gum and frequent swallowing during descent can help ease the discomfort.

Decongestant nose drops will clear a blocked nose. Air travel should be avoided for 10 days if there has been a recent ear surgery or tonsillectomy.

Women often need to travel during pregnancy — as part of their jobs, because of transfers or simply to head home to have the baby. Air travel during pregnancy is safe and poses no special risks. Mid pregnancy, from the 14th to 28th week, is the safest time. In the case of multiple pregnancy (twins), a history of premature delivery, cervical incompetence, bleeding or increased uterine activity (irritable uterus), flying is inadvisable. If you need to be elsewhere for the delivery, it is better to leave before the 36th week or use an alternative mode of transport.

Most airlines refuse to allow pregnant passengers after the 36th week because of the fear that labour may set in during the flight. It is better to carry certified medical documentation about the expected date of delivery.

During pregnancy,

• the seat belt should be fastened under the abdomen, not across it;

• an aisle seat is preferable to facilitate visits to the toilet;

• try to get out of the seat every 30 minutes and walk a short distance;

• if this is not possible, flex and extend the ankles.

Babies should, preferably, not fly till they are at least seven days old.

There is a 10-day ban on air travel (not prohibited but inadvisable) after a stroke, brain surgery, an epileptic seizure, eye surgery or ear, nose or throat procedures.

Even in normal people abdominal gas increases by 25 per cent during air travel. A three to four week gap is advisable after abdominal surgery even if it is a “keyhole” or laparoscopic surgery as gas is introduced into the abdomen during the procedure. This extra gas can expand and cause the sutures to give way.

A person with congestive cardiac failure (when the heart does not function properly) should be stable for at least 10 days prior to travel.

In the case of a heart attack the person should have been stable for three to four weeks.

After pneumonia or chest surgery, a person should wait for three weeks
. Even after this time they should be able to walk unassisted for at least 50 metres without becoming breathless.

Anaemia, with haemoglobin count less than 7.5 grams per decilitre, reduces the oxygen carrying capacity of blood. This can get critical during flights.

People with fractures can travel two days after the cast has been applied. In traditional casts air can be trapped between the cast and the leg. As this air expands during the flight, it can compress the limb and cut off blood supply. If a person needs to fly immediately, the doctor needs to be informed beforehand. A bivalved or split cast, which does not trap air, can be applied.

People with mental illness should be well controlled, on medication and preferably have a companion.

Diseases are spread from one country to another by infected travellers. In the recent swine flu epidemic, the spread of the disease could be plotted by tracking the flights out of Mexico (where the epidemic started).

People with open tuberculosis or measles should also defer travel.
If a person has an infectious disease, travelling should be postponed until recovery. Infected air keeps circulating in a plane and this will result in the disease spreading.

The economy class has little legroom. The edge of the seat can compress the veins at the bent knee.
Together with the forced immobility, blood pools in the legs and the feet swell. This can result in deep vein thrombosis and pulmonary embolism. Sudden unexpected death can occur hours or days after travel.

Generally, try to drink plenty of fluids and balance any alcohol consumed with an equal amount of water. Walk around the airport while waiting. Remember, the most dangerous thing to do is to sit still with your legs crossed.

Definition:
Varicose veins are gnarled, enlarged veins. Any vein may become varicose, but the veins most commonly affected are those in your legs and feet. That’s because standing and walking upright increases the pressure in the veins of your lower body.

Varicose veins affect 1 out of 2 people over age 50. They are more common in women than men. Hemorrhoids are a type of varicose vein. Spider veins are like varicose veins, but they are smaller.

For many people, varicose veins and spider veins — a common, mild variation of varicose veins — are simply a cosmetic concern. For other people, varicose veins can cause aching pain and discomfort. Sometimes varicose veins lead to more-serious problems. Varicose veins may also signal a higher risk of other circulatory problems. Treatment may involve self-care measures or procedures by your doctor to close or remove veins.

Symptoms:
Varicose veins usually don’t cause any pain. Signs you may have varicose veins include:

*Veins that are dark purple or blue in color
*Veins that appear twisted and bulging; often like cords on your legs
*Varicose veins may also form in other places on your legs, from your groin to your ankle.

Causes:

Arteries carry blood from your heart to the rest of your tissues. Veins return blood from the rest of your body to your heart, so the blood can be recirculated. To return blood to your heart, the veins in your legs must work against gravity. Muscle contractions in your lower legs act as pumps, and elastic vein walls help blood return to your heart. Tiny valves in your veins open as blood flows toward your heart then close to stop blood from flowing backward.

Causes of varicose veins can include:

*Age. As you get older, your veins can lose elasticity causing them to stretch. The valves in your veins may become weak, allowing blood that should be moving toward your heart to flow backward. Blood pools in your veins, and your veins enlarge and become varicose. The veins appear blue because they contain deoxygenated blood, which is in the process of being recirculated through the lungs.

*Pregnancy. Some pregnant women develop varicose veins. Pregnancy increases the volume of blood in your body, but decreases the flow of blood from your legs to your pelvis. This circulatory change is designed to support the growing fetus, but it can produce an unfortunate side effect — enlarged veins in your legs. Varicose veins may surface for the first time or may worsen during late pregnancy, when your uterus exerts greater pressure on the veins in your legs. Changes in your hormones during pregnancy also may play a role. Varicose veins that develop during pregnancy generally improve without medical treatment within three months after delivery.

Risk factors:
These factors increase your risk of developing varicose veins:

*Your age. Varicose veins occur most often in people ages 30 to 70, with your risk increasing as you age. Aging causes wear and tear on the valves in your veins that help regulate blood flow. Eventually, that wear causes the valves to allow blood to flow back into your veins where it collects instead of flowing up to your heart.

*Your sex. Women are more likely to develop the condition. Hormonal changes during pregnancy, premenstruation or menopause may be a factor. Female hormones tend to relax vein walls. Taking hormone replacement therapy or birth control pills may increase your risk of varicose veins.

*Genetics. If other family members had varicose veins, there’s a greater chance you will too.

*Obesity. Being overweight puts added pressure on your veins.

*Standing for long periods of time. Your blood doesn’t flow as well if you’re in the same
position for long periods.

Complications:-
Complications of varicose veins, although rare, can include:
Ulcers. Extremely painful ulcers may form on the skin near varicose veins, particularly near the ankles. Ulcers are caused by long-term fluid buildup in these tissues, caused by increased pressure of blood within affected veins. A brown colored spot on the skin usually begins before an ulcer forms. See your doctor immediately if you suspect you’ve developed an ulcer.

Blood clots. Occasionally, veins deep within the legs become enlarged. In such cases, the affected leg may swell considerably. Any sudden leg swelling warrants urgent medical attention because it may indicate a blood clot — a condition known medically as
thrombophlebitis.

“Eczema”- That Irritating Itch!

Eczema is a strange disease. If you have it, the frustrating itching and scratching starts and then just never seems to go away. The number of people with eczema is increasing. One in five now develops it in childhood. Many factors, such as early weaning, chemicals (preservatives and pesticides) in the human diet, perfumes and pollution, have been blamed. The fact is no one really knows the cause of eczema, but we do know that exclusively breast-fed infants are less prone to it.

Eczema appears as red, itchy, dry and flaky skin, with or without small pustules. A few months of constant itching may make the skin permanently rough, thick and hyperpigmented.

The classification of eczema is not very clear. It can be due to atopy (tendency to develop allergy), allergic contact, infantile seborrhoea (dandruff-like condition of the head that occurs in infants) or varicose veins.

Atopic eczema is the commonest form. It usually occurs in childhood as an itchy rash distributed on the head and scalp, neck, inside of elbows, behind the knees and on the buttocks. It has a hereditary component and runs in families.

Infants can develop an eczema variant called cradle cap — with a greasy, itchy flaky scalp. This can extend to the eyebrows, face and the trunk. Although the condition is self limiting and harmless, the physical appearance can be distressing to the family.

Irritant contact eczema can be distinguished from the atopic form by the typical distribution and family history. It occurs as an immediate or delayed reaction to contact with an allergen. It may be nickel in the safety pins used on clothes, fashion jewellery containing unacceptable metals or colouring, plants in the garden or chemicals in the workplace. Makeup or face powder can cause an idiosyncratic reaction. Sometimes the contact eczema may be photosensitive and flare up only when the skin is exposed to sunlight. Eczema caused by a specific chemical or disease process can be cured if the underlying factor is removed.

Bacterial, viral or fungal infection of the skin, or infestations with skin parasites like scabies or body lice can cause secondary itching and eczema. Poor blood circulation to the legs as a result of varicose veins can lead to itching and discolouration, particularly near the ankles. These forms are totally curable and tend to disappear forever when the infection is treated or the blood circulation is improved.

Unfortunately most varieties of eczema do not fall into the curable category. They recur time and again, and sometimes become self perpetuating as scratching and picking the skin becomes a habit.

Dry skin aggravates eczema. Therefore it pays to keep the skin moist and oiled. Coconut oil can be applied to the skin half an hour before a bath. A teaspoon can also be added to the bath water. Mild and non-perfumed soaps should be used. The body should not be scrubbed with a loofah. The skin should be patted dry, and not wiped. After the bath a non-greasy oil or lotion can be applied.

There are several baby oils, aloe vera preparations, ceramide (a natural oil in the skin) and vaseline-based creams available in the market. If it says “non greasy” it means that it is unlikely to stain your clothes.
Traditional oils, like coconut, stain the clothes and that is why they need to be applied before a bath.

Salt water reduces eczema. People who immerse themselves regularly in the sea improve gradually over a period of time.

Detergents are widely used to wash clothes. Most contain chemicals like sodium lauryl sulphate, which remain behind in the clothes in small quantities. They can penetrate the skin when sweating occurs, aggravating eczema. Commercially available “hypoallergic” or “doctor tested and recommended” detergents have unsubstantiated claims and have not been proven safer. Since clothes have to be washed and kept clean, it is preferable to use non-allergic soaps made from neem oil. They are marketed in India by the government run chain of khadi stores.

Eczema disappears when steroid creams are applied. These should be used under medical supervision for a short period, till the symptoms subside. After that antihistamines (to control itching) and moisturisers should be continued. Patients, unfortunately, purchase and apply the ointments themselves. Prolonged use of topical corticosteroids causes side effects like thinning of the skin and secondary bacterial or fungal infection. Small quantities of the more potent steroid creams can become absorbed in the body through the skin. If they are inadvertently applied to the eye, cataracts and glaucoma can result.

Specific pinprick allergy testing can be done to identify allergens. Without this, drastic potentially harmful changes should not be made to the diet (particularly a child’s) presuming that the eczema is precipitated by allergies to milk, fish or certain vegetables.

A herb that is a part of almost every Indian kitchen continues to make news. Garlic, or Allium sativum, one of the oldest medicinal herbs known to human beings, is now in laboratories, while scientists look at what makes it so beneficial.

Indeed, garlic seems to possess near-magical health properties. Yet science has not found it an easy herb to understand. Despite tall claims from practitioners of alternative medicine, no one clearly knows how good garlic is and why it is considered to be so beneficial. But now scientists are rapidly unravelling its secrets.

Over the years, people with varying backgrounds have claimed that the bulb is good for controlling blood pressure and reducing cholesterol. It is supposed to have anti-inflammatory, antimicrobial, antiviral, antioxidant and anticoagulant properties, apart from a few other benefits. There is now good evidence that most of these claims are true. And scientists have recently discovered new properties as well: it can reduce blood sugar levels, and is thus good for controlling diabetes. Yet conventional wisdom is not always right: it may not, after all, reduce cholesterol.

While evidence on the utility mounts, scientists are also beginning to understand why. For example, garlic’s antioxidant properties have been a mystery to scientists. It has been known to be a powerful antioxidant (a compound that destroys damaging free radicals); in fact a bit too powerful for comfort. It has a compound called allicin that is an antioxidant, but its structure could not explain its power. Till now, that is.

Derek Pratt, professor of chemistry at Queen’s University in Canada, has found out why garlic is so powerful.

A compound akin to allicin is found in other plants of the family alliaceae — such as shallots, onions and leeks. However, none of these plants has garlic’s beneficial powers. This is because the allicin found in garlic breaks down into another compound called sulphenic acid, which rapidly cleans up free radicals in its path. Without this breakdown, allicin cannot be so effective an antioxidant.

“This compound is the most powerful antioxidant known to us,” says Pratt, who published his results last week in the journal Angewandte Chemie.

While its efficacy in dealing with free radicals is now beyond doubt, garlic is probably not so effective in reducing bad cholesterol, the Low Density Lipoproteins (LDL). Several studies on its effect on blood cholesterol led to conflicting results, but one in Stanford University more than a year ago was probably the most conclusive. This six-month-long study found no correlation between consumption of garlic and reduction of LDL. “We are convinced now that garlic does not reduce bad cholesterol,” Christopher Gardner, the Stanford professor who led the research, had told Knowhow soon after publishing the results of the study.

But that does not mean it is not useful in treating high cholesterol. Its antioxidant properties are useful in treating cardiovascular diseases in general, and even for treating high cholesterol. This is because garlic suppresses the oxidation of LDL in the blood. LDL is called bad cholesterol because it sticks to the artery walls and clogs the arteries. However, it is not LDL that actually does the damage but oxidised LDL. Several studies have shown that garlic suppresses oxidation of LDL and thus prevents the formation of plaques in the arteries. It makes bad cholesterol not so bad.

“Garlic does reduce LDL oxidation,” stresses Khalid Rahman, reader in the physiological biochemistry at Liverpool University in the UK, who has conducted several lab and clinical studies on the herb.

There is increasing evidence that it can lower blood pressure, particularly when BP is elevated only mildly. A recent meta-analysis (analysis of all published literature) by scientists at the University of Adelaide showed that it does lower blood pressure. However, the scientists also warn that the evidence is not strong enough to use garlic as the only means of therapy.

These results are from clinical studies, which mean that they have been done on people. The results are equally encouraging in pre-clinical studies done in the laboratory. There, the herb has been shown to be anti-inflammatory and anticoagulant. It may be able to help dissolve clots and improve blood circulation. A few months ago, Japanese scientists (at the RIKEN and other institutions) showed that it could lower blood glucose levels in rats.

The list of beneficial properties is actually lengthening every day, but the topic is not without its controversy either.

This is because there are some studies showing that garlic had no effect on lowering blood pressure or reducing cholesterol, while some others showed that it did do so. This variability, fortunately, is not hard to explain. Scientists explain this contradiction through differences in the duration of the trials, and also on the variability in the properties of garlic. “The factors influencing a clinical study with garlic are difficult to control,” says Pratt.

Although we know that it is beneficial, not all kinds of garlic may act in the same manner. “In my view there is a group of people who are non-responders to garlic, like to any other medication,” says Rahman.

However, garlic has caught the attention of hundreds of scientists throughout the world. We will learn more about this wonder herb in the coming years.
