Causes:

It is a type of pneumonia caused by bacteria. You usually get it by breathing in mist from water that contains the bacteria.The Legionella bacteria are found naturally in the environment, usually in water. The bacteria grow best in warm water, like the kind found in hot tubs, cooling towers, hot water tanks, large plumbing systems, or parts of the air-conditioning systems of large buildings. They do not seem to grow in car or window air-conditioners. The mist may come from hot tubs, showers or air-conditioning units for large buildings. The bacteria don’t spread from person to person.The disease is fatal in approximately 5% to 15% of cases.

Symptoms:
Symptoms of Legionnaires’ disease include fever, chills, a cough and sometimes muscle aches and headaches. Other types of pneumonia have similar symptoms. You will probably need a chest x-ray to diagnose the pneumonia. Lab tests can detect the specific bacteria that cause Legionnaires’ disease.

The bacteria are more likely to make you sick if you:

* Are older than 65
* Smoke
* Have a lung disease
* Have a weak immune system

Legionnaires’ disease is serious and can be life-threatening. However, most people recover with antibiotic treatment. Legionnaires’ has an incubation period of between two and 10 days.
Initial symptoms of are similar to those of flu – headache, musclepain, and a general feeling of being unwell.These symptoms are followed by high fever and shaking chills. Nausea, vomiting, and diarrhoea may occur.On the second or third day, dry coughing begins and chest pain might occur. There may also be difficulty breathing.Mental changes, such as confusion, disorientation, hallucination and loss of memory, can occur to an extent that seems out of proportion to the seriousness of fever. Some patients may develop pneumonia. This could affect both lungs and lead to hospitalisation if severe.

Diagnosis:
Legionnaires’ disease is underreported and underdiagnosed, primarily because special tests are needed to distinguish Legionnaires’ disease from other types of pneumonia. To help identify the presence of legionella bacteria quickly, your doctor may use a test that checks your urine for legionella antigens — foreign substances that trigger an immune system response. You may also have one or more of the following:

* Blood tests
* A chest X-ray, which doesn’t confirm Legionnaires’ disease but does show the extent of infection in the lungs
* Tests on a sample of your sputum or lung tissue
* A CT scan of your brain or a spinal tap (lumbar puncture) if you have neurological symptoms such as confusion or trouble concentrating

Risk Factorts:

Legionnaires’ disease usually strikes middle-aged people. Those at risk include smokers and those with an existing health problem.Many others may contract the bug and yet show no signs of infection. It is likely that many cases of Legionnaires’ disease go undiagnosed.People suffering from cancer or chronic kidney diseases are among those less able to fight infections.Chronic diseases, such as diabetes and alcoholism, also seem to increase vulnerability to Legionnaires’ disease.Cigarette smokers are more likely to contract Legionnaires disease, perhaps because smokers are generally more likely than non-smokers to develop respiratory tract infections.

Treatment:

Legionnaires’ is most often treated with the antibiotic drugs erthryomycin and rifampin. Recovery often takes several weeks.

Prevention:

The likelihood of Legionella infection can be best reduced by good engineering practices in the operation and maintenance of air and water handling systems.Cooling towers and evaporative condensers should be inspected and thoroughly cleaned at least once a year.Corroded parts, such as drift eliminators, should be replaced. Algae and accumulated scale should be removed.Cooling water should be treated constantly. Ideally, an automatic water treatment system should be used that continuously controls the quality of the circulating water.Fresh air intakes should not be built close to cooling towers since contaminated water particles may enter the ventilation system.This page contains basic information. Corns and Calluses

 

The foot is an engineering marvel that cannot be duplicated by robotics. It is composed of 26 bones, 33 joints and around a hundred tendons, ligaments and muscles of various sizes, well oiled and sliding smoothly over each other.

The foot is encased in thick and leathery skin that protects it from injury. Our feet take us everywhere from the time we first learn to walk till we are finally laid to rest. Yet, somehow, we tend to take our feet for granted until they malfunction, cause pain or refuse to perform as instructed.

Some orthopaedic abnormalities of the feet, like clubfoot, flat feet, congenitally dislocated hips or knock knees, are present from birth. Sometimes shoes may be purchased for conformity to fashion disregarding the requirements of the feet. They maybe ill fitting, too tight or too large. Pointed toe stilettos are notoriously guilty. The gait becomes abnormal as pressure is applied unevenly to the skin of the foot. Irregularity may also occur in old age as a result of arthritis or injury.

When pressure is exerted unevenly on the foot, the skin tends to thicken abnormally. This callused skin may remain like that or develop into a corn. This happens in areas where the skin is rubbed persistently or where the skin is under uneven pressure. Common places are the heel, the ball of the foot and the sides of the toes.

The skin over the unsightly hardened area on the foot, a callus, is dead. So there is no inflammation or pain. Eventually the callus may harden to form a corn. The corn has a central area of inflammation and is painful and tender. It is usually situated near the base of the fifth toe. If the feet are pushed into tight fitting shoes, corns will form between the toes as well. Perspiration and moisture cannot escape from this area causing the corn to become macerated and tender.

If a callus or corn is beginning to develop, the first step would be to determine if there is any source of pressure which has set off the thickening of the skin. Sometimes it may even be due to a sudden increase in the level of exercise or interest in some new sport. Once the cause has been identified it should be removed, or else the callus will exacerbate.

The foot should first be soaked in warm water with rock salt and commercially available liquid soap. After 10 minutes the affected area should be gently rubbed with a pumice stone or a foot scrubber. Some baby oil or moisturiser should then be applied. This gets rid of the corn (or callus) at an early stage.

If the callus is hard and the punctum or tip of the corn is easily visible, commercially advertised OTC (over the counter) corn plasters may be used. Most of them contain salicylic acid — a keratolytic agent that softens and breaks down hard skin. They need to be applied on a dry foot and left in place till the corn softens and falls out. They should not be used on soft corns between the toes.

Wearing loose footwear with low heels and a well-cushioned insole can also relieve the pain. Slippers should be made of soft rubber like MCR (micro cellular rubber). Acupressure slippers are also helpful.

If the corn does not respond to these simple measures, it has to be surgically removed. A qualified dermatologist or surgeon can do it as an outpatient office procedure.

Newer, relatively painless techniques involve freezing the corn with liquid nitrogen or dry ice or removing it with laser technology. It is dangerous to perform “home surgery” — slicing off the corn with a knife or blade. Dangerous debilitating infections can occur due to such amateur attempts.

Diabetics need to take particular care of their corns and calluses and consult their physicians if they have a problem. They may have compromised blood supply to their feet or numbness of the nerves. This may make them insensitive to the pain making them inadvertently ignore the corn. Any self-treatment (especially salicylic corn plasters) is likely to result in dangerous infection.

Some simple rules one should follow:

• Wear proper footwear and socks

• Wash the feet well at night before going to bed

• Moisturise the feet with oil or cream once a day

• Keep the areas between the toes dry, particularly after a bath

• Treat arthritis, blisters, corns and calluses promptly

Appropriate footwear for various sports should be used. One pair of “canvas” shoes should not be used for all activities.

Cat-Scratch Disease

Definition:
Cat-scratch disease is an infection caused by bacteria (germs) carried in cat saliva. The bacteria can be passed from a cat to a human. Doctors and researchers think cats may get the bacteria from fleas, although this hasn’t been proved.

.You can get cat-scratch disease from a cat bite or cat scratch. You can get the infection after a cat scratches you if the cat’s paws have the bacteria on them. (A cat can get the bacteria on its paws when it licks itself.) With a cat bite, the cat can pass the bacteria to you in its saliva. You can also get the bacteria in your eyes if you pet a cat that has the bacteria on its fur and then rub your eyes. Many people who get cat-scratch disease do not remember being scratched or bitten by a cat.

Cat-scratch disease is not a severe illness in people who are healthy. But it can be a problem in people with weak immune systems. People with weak immune systems include those who are receiving chemotherapy for cancer, those who have diabetes or those who have acquired immunodeficiency syndrome (AIDS).

It is a bacterial disease caused by Bartonella henselae. Most people with CSD have been bitten or scratched by a cat and developed a mild infection at the point of injury. Lymph nodes, especially those around the head, neck, and upper limbs, become swollen. Additionally, a person with CSD may experience fever, headache, fatigue, and a poor appetite. Rare complications of B. henselae infection are bacillary angiomatosis and Parinaud’s oculolandular syndrome.

The condition has also been termed Cat-Scratch Adenitis, Cat-Scratch-Oculoglandular Syndrome, Debre’s Syndrome, Debre-Mollaret Syndrome, Foshay-Mollaret Cat-Scratch Fever, Foshay-Mollaret syndrome, Foshay-Mollaret Cat-Scratch Fever Syndrome, Lymphadenitis-Regional Non-bacterial, Lymphoreticulosis-Benign Inoculation, maladie des griffes du chat, Parinaud oculoglandular disease, and Petzetakis’ disease.CSD; Cat scratch fever; Bartonellosis

Transmission
The cat was recognized as the vector of the disease in 1950 by Dr. Robert Debré.

The causative organism was first thought to be Afipia felis, but this was disproved by immunological studies demonstrating that cat scratch fever patients developed antibodies to two other organisms, Bartonella henselae and Bartonella clarridgeiae, which are rod-shaped Gram negative bacteria.

Kittens are more likely to carry the bacteria in their blood, and are therefore more likely to transmit the disease than are adult cats.

Ticks are also a major transmitter of this disease. It is often transmitted at the same time a human may get Lyme Disease.

Causes :

Cat scratch disease is caused by Bartonella henselae. The disease is spread through contact with an infected cat (a bite or scratch), or contact with cat saliva on broken skin or the white of the eye.

About 2 – 3 weeks after becoming infection, lymph nodes swelling (lymphadenopathy) occurs near the site of the scratch or bite.

Occasionally, an infected lymph node may form a tunnel (fistula) through the skin and drain. Cat scratch disease is a common causes of chronic lymph node swelling in children.

Sign & Symptoms:

A person who has had contact with a cat may show common symptoms, including:

* Bump (papule) or blister (pustule) at site of injury (usually the first sign)
* Lymph node swelling near the scratch or bite
* Fever (in some patients)
* Fatigue
* Overall discomfort (malaise)
* Headache

Less common symptoms may include:

* Loss of appetite
* Weight loss
* Enlarged spleen
* Sore throat
* Draining lymph nodes
A sore may develop where a cat has bitten or scratched you. The sore might not happen right away. It may take 3 to 10 days for the sore to appear after the bite or scratch.

The sore may take a long time to heal. An infection of the lymph nodes (also called lymph glands) also develops, most often in the glands that are near the place where you got the cat scratch or cat bite. For example, if the infection is from a cat scratch on your arm, the glands in your armpit may become tender and swollen. The lymph nodes may swell to an inch or more in size.

Cat scratch fever presents with tender regional lymphadenopathy, sterile suppurative papules at the site of inoculation, slight fever, headache, chills, backache, abdominal pain, malaise, alteration of mental status, and convulsions. It may take 7 to 14 days, or as long as two months, before symptoms appear. Most cases are benign and self-limiting, but lymphadenopathy may persist for several months after other symptoms disappear. The prognosis is generally favorable. In temperate climates, most cases occur in fall and winter. The disease usually resolves spontaneously, with or without treatment, in one month. In immunocompromised patients more severe complications sometimes occur.

Exams and Tests :

A scratch or injury and a history of contact with a cat indicates that cat scratch disease is a possible cause of the lymph node swelling. In some cases, physical examination also shows an enlarged spleen (splenomegaly).

The disease often goes unrecognized because of the difficulty in testing. However, the Bartonella henselae IFA test is highly sensitive and specific for the detection of infection caused by this bacteria.

Other tests used in the diagnosis of cat scratch disease:

* Lymph node biopsy to rule out other causes of swollen glands

When to call a Doctor?
Call your family doctor if you notice any of the following problems:

* A cat scratch or bite that does not heal in the usual length of time.
* An area of redness around a cat scratch or bite that continues to get bigger for more than 2 days after the injury.
* Fever that lasts for several days after a cat scratch or bite.
* Painful and swollen lymph nodes for more than 2 or 3 weeks.
* Bone or joint pain, abdominal pain (without fever, vomiting or diarrhea) or an unusual degree of tiredness for more than 2 or 3 weeks.

Treatment:

In AIDS patients and in other people who have suppressed immune systems, cat scratch disease is more serious, and treatment with antibiotics is recommended.

Azithromycin, ciprofloxacin, doxycycline, and multiple other antibiotics have been used successfully.

In most people, cat-scratch disease clears up without treatment. However, antibiotics (medicines that kill bacteria) may be needed when infected lymph nodes stay painful and swollen for more than 2 or 3 months. Antibiotics may also help if you have a fever for a long time or if the infection is in your bones, liver or another organ.

If a lymph node is very large or painful, your doctor may drain it to help relieve the pain. The lymph node is drained by putting a needle through normal skin off to the side of the node and moving the needle to the swollen node. The needle is then inserted into the node and the fluid in the node is drained out.

Treatment for the cat:

Cats require no treatment. The bacteria doesn’t cause cats to get sick. They merely carry the bacteria that causes cat-scratch disease in people.

Prognosis:

In children with normal immune systems, full recovery without treatment is the norm. In immunocompromised people, treatment with antibiotics generally leads to recovery.

Possible Complications:

* Parinaud’s syndrome
* Encephalopathy
* Neuroretinitis
* Osteomyelitis


Prevention:

Avoiding contact with cats prevents the disease. Where this is not reasonable, good hand-washing after playing with a cat, avoiding scratches and bites, and avoiding cat saliva will lessen the risk of infection.

Dizziness is classified into three categories—vertigo, syncope, and nonsyncope nonvertigo. Each category has a characteristic set of symptoms, all related to the sense of balance. In general, syncope is defined by a brief loss of consciousness (fainting) or by dimmed vision and feeling uncoordinated, confused, and lightheaded. Many people experience a sensation like syncope when they stand up too fast. Vertigo is the feeling that either the individual or the surroundings are spinning. Vision and the body’s senses are the most important systems for maintaining balance, but problems in the inner ear are the most frequent cause of dizziness. The inner ear, also called the vestibular system, contains fluid that helps to fine tune the information the brain receives from the eyes and the body. When fluid volume or pressure in the inner ear changes, information about balance is altered. The discrepancy gives conflicting messages to the brain about balance and induces dizziness.

Certain medical conditions can cause dizziness because they affect the systems that maintain balance. For example, the inner ear is very sensitive to changes in blood flow. Because such medical conditions as high blood pressure or low blood sugar can affect blood flow, these conditions are frequently accompanied by dizziness. Circulation disorders are the most common causes of dizziness. Other causes are head injuries, ear infections, allergies, and nervous system disorders.

Dizziness often disappears without treatment or with treatment of the underlying problem, but it can be long-term or chronic. According to the National Institutes of Health, 42% of Americans will seek medical help for dizziness at some point in their lives. The costs may exceed a billion dollars and account for five million visits to physicians annually. Episodes of dizziness increase with age. Among people aged 75 or older, dizziness is the most frequent reason for seeing a doctor.

Causes & symptoms
Careful attention to symptoms can help determine the underlying cause of the dizziness. The underlying problems may be benign and easily treated, or they may be dangerous and require intensive therapy. Not all cases of dizziness can be linked to a specific cause. More than one type of dizziness can be experienced at the same time and symptoms may be mixed. Episodes of dizziness may last for a few seconds or for days. The length of an episode is related to the underlying cause.

The symptoms of syncope include dimmed vision, loss of coordination, confusion, lightheadedness, and sweating. These symptoms can lead to a brief loss of consciousness or fainting. They are related to a reduced flow of blood to the brain; they often occur when a person is standing up and can be relieved by sitting or lying down. Vertigo is characterized by a sensation of spinning or turning, accompanied by nausea, vomiting, ringing in the ears, headache, or fatigue. An individual may have trouble walking, remaining coordinated, or keeping balance. Nonsyncope nonvertigo dizziness is characterized by a feeling of being off balance that becomes worse if the individual tries moving or performing detail-intense tasks.

A person may experience dizziness for many reasons. Syncope is associated with low blood pressure, heart problems, and disorders in the autonomic nervous system, which controls such involuntary functions as breathing. Syncope may also arise from emotional distress, pain, and other reactions to outside stressors. Nonsyncope nonvertigo dizziness may be caused by rapid breathing, low blood sugar, or migraine headache, as well as by more serious medical conditions.

Vertigo is often associated with inner ear problems called vestibular disorders. A particularly intense vestibular disorder, Ménière’s disease, interferes with the volume of fluid in the inner ear. This disease, which affects approximately one in every 1,000 people, causes intermittent vertigo over the course of weeks, months, or years. Ménière’s disease is often accompanied by ringing or buzzing in the ear, hearing loss, and a feeling that the ear is blocked. Damage to the nerve that leads from the ear to the brain can also cause vertigo. Such damage can result from head injury or a tumor. An acoustic neuroma, for example, is a benign tumor that wraps around the nerve. Vertigo can also be caused by disorders of the central nervous system and the circulation, such as hardening of the arteries (arteriosclerosis), stroke, or multiple sclerosis.

Some medications cause changes in blood pressure or blood flow. These medications can cause dizziness in some people. Prescription medications carry warnings of such side effects, but common drugs such as caffeine or nicotine can also cause dizziness. Certain antibiotics can damage the inner ear and cause hearing loss and dizziness.

Diet may cause dizziness. The role of diet may be direct, as through alcohol intake. It may be also be indirect, as through arteriosclerosis caused by a high-fat diet. Some people experience a slight dip in blood sugar and mild dizziness if they miss a meal, but this condition is rarely dangerous unless the person is diabetic. Food sensitivities or allergies can also be a cause of dizziness. Such chronic conditions as heart disease and serious acute problems such as seizures and strokes can cause dizziness. These conditions, however, usually exhibit other characteristic symptoms.

Diagnosis
During the initial medical examination, an individual with dizziness should provide a detailed description of the type of dizziness experienced, when it occurs, and how often each episode lasts. A diary of symptoms may help to track this information. The patient should report any symptoms that accompany the dizziness, such as ringing in the ear or nausea, any recent injury or infection, and any medication taken.

The examiner will check the patient’s blood pressure, pulse, respiration, and body temperature as well as the ear, nose, and throat. The sense of balance is assessed by moving the individual’s head to various positions or by tilt-table testing. In tilt-table testing, the person lies on a table that can be shifted into different positions and reports any dizziness that occurs.

Further tests may be indicated by the initial examination. Hearing tests help assess ear damage. X rays, computed tomography scan (CT scan), and magnetic resonance imaging (MRI) can pinpoint evidence of nerve damage, tumors, or other structural problems. If a vestibular disorder is suspected, a technique called electronystagmography (ENG) may be used. ENG measures the electrical impulses generated by eye movements. Blood tests can determine diabetes, high cholesterol, and other diseases. In some cases, a heart evaluation may be useful. Despite thorough testing, however, an underlying cause cannot always be determined.

Doctors caution that childhood syncope (fainting), although rarely serious, can indicate a serious cardiac. If the fainting is abrupt or happens with exertion, it may indicate a more serious problem.

Treatment:-

Because dizziness may arise from serious conditions, it is advisable to seek medical treatment. Alternative treatments can often be used alongside conventional medicine without conflict. Potentially beneficial therapies include nutritional therapy, herbal remedies, homeopathy, aromatherapy, osteopathy, acupuncture, acupressure, and relaxation techniques.

Nutritional therapy
To prevent dizziness, nutritionists often advise eating smaller but more frequent meals and avoiding caffeine, nicotine, alcohol, foods high in fat or sugar, or any substances that cause allergic reactions. A low-salt diet may also be helpful to some people. Nutritionists may also recommend certain dietary supplements:

*Magnesium citrate, aspartate or maleate: for dizziness caused by magnesium deficiency.
*B-complex vitamins, especially vitamin B12: for dizziness caused by deficiency of these essential vitamins.

Herbal remedies

The following herbs have been used to treat dizziness symptoms:

*Ginger: for treatment of dizziness caused by nausea.
*Ginkgo biloba: may decrease dizziness by increasing blood flow to the brain.

Homeopathy

Homeopathic therapies can work very effectively for dizziness, and are especially applicable when no organic cause can be identified. They are chosen according to the patient’s specific symptom profile:

*Aconite: for feeling light-headed from postural hypotension (getting up too quickly)
*Coccolus: for motion sickness or syncope
*Conium maculatum: for feeling dizzy while looking at rapidly-moving images.
*Gelsemium: for feeling light-headed and out of balance, often associated with influenza or stage fright.
*Petroleum: for dizziness upon standing up too fast and headache before and after a storm.

Aromatherapy:

Aromatherapists recommend a warm bath scented with essential oils of lavender, geranium, and sandalwood as treatment for dizziness. This therapy can have a calming effect on the nervous system.

Osteopathy:

An osteopath or chiropractor may suggest manipulations or adjustments of the head, jaw, neck, and lower back to relieve pressure on the inner ear.

Acupressure:

Acupressure may be able to improve circulation and decrease the symptoms of vertigo. The Neck Release, which involves pressing on five pairs of points on the shoulder blades and neck, is helpful for dizziness associated with migraine headaches.

Relaxation techniques, such as yoga, meditation, and massage therapy for relieving tension, are popularly recommended methods for reducing stress.

Allopathic treatment:
Treatment of dizziness is determined by the underlying cause. If an individual has a cold or influenza, a few days of bed rest is usually adequate to resolve dizziness. Other causes of dizziness, such as mild vestibular system damage, may resolve without medical treatment. If dizziness continues, drug therapy may be required to treat such underlying illnesses as high blood pressure, arteriosclerosis, nervous conditions or diabetes. A physician may also prescribe antibiotics if ear infections are suspected. Selective serotonin reuptake inhibitors (SSRIs) have recently been shown to relieve dizziness in patients who have psychiatric symptoms. When other measures have failed, surgery may be suggested to relieve pressure on the inner ear. If the dizziness is not treatable by drugs, surgery, or other means, physical therapy may be used and the patient may be taught coping mechanisms for the problem.

Expected results
The outcome of treatment depends on the cause of dizziness. Controlling or curing the underlying factors usually relieves the dizziness itself. In some cases, the symptoms disappear without treatment. In a few cases, dizziness can become a permanent disabling condition.

Prevention

Most people learn through experience that certain activities will make them dizzy and they learn to avoid them. Saffron May Ease PMS Symptoms

Saffron, a spice known for flavouring cuisine, might also offer an antidote to premenstrual syndrome, a small study suggests.

It’s thought that the spice might influence depression symptoms via effects on the brain chemical serotonin. Because alterations in serotonin activity are suspected in PMS, a team of Iranian researchers decided to study whether saffron supplements might help relieve these symptoms.

M Agha-Hosseini and colleagues at Tehran University of Medical Sciences randomly assigned 50 women to take either saffron capsules or a placebo twice a day over two menstrual cycles. The women, had all had PMS symptoms for at least six months.

At the end of the treatment period, three-quarters of the women on saffron capsules reported at least a 50% reduction in PMS symptoms.

That compared with only 8% of women in the placebo group, the researchers report in the medical journal
BJOG. In addition, the researchers found, 60% of the saffron group showed a 50% improvement in depression symptoms.
