Issues involving pregnancy and the menstrual cycle are centrally important health matters for women. Being pregnant, preparing for pregnancy, and recovering from childbirth are female indispositions. Pain during the menstrual period (dysmenorrhea), premenstrual syndrome (PMS), and chronic pelvic pain are among the distressing conditions which disturb the otherwise healthy lives of many women.

Older women are much more likely than men to develop such serious ailments as Alzheimer’s disease (which burdens the afflicted with near-total memory loss and reduced mental functioning) and osteoporosis (in which the bones become weak, brittle, and porous; the posture stooped with the shoulders rounded). These are just two of the problems of aging that women must be aware of and can take steps to prevent.

Less ominous, but more pervasive, are headaches. Women may get headaches during menstrual periods and pregnancy, as well as under ordinary circumstances. They are more likely to be troubled enough by headache pain to find a way to overcome it. Similarly, women are often victims of osteoarthritis (degenerative joint disease) and have a 200 to 300 percent greater chance than men of suffering rheumatoid arthritis.

Chiropractic offers demonstrated relief for many of women’s health problems. Chiropractic adjustments have been shown to lessen the discomfort of dysmenorrhea and chronic pelvic pain. What It Is
A person with diabetes doesn’t produce enough of the hormone insulin or is unable to use it effectively, which causes high blood sugar (glucose) levels. Over time, this imbalance can lead to heart disease, nerve damage, kidney disease, vision loss, and other complications. There are two types of diabetes. Less common is insulin-dependent diabetes (type 1), which usually develops before age 30. Non-insulin-dependent diabetes (type 2) accounts for 90% of cases; it usually appears after age 40.

What Causes It
Type 1 diabetes occurs when the pancreas stops producing insulin. No one knows exactly why this happens, but some experts believe a virus or an autoimmune response, in which the body attacks its own pancreatic cells, is responsible. People with this type of diabetes must take insulin for life. Type 2 diabetes develops from insulin resistance. Here the pancreas secretes plenty of insulin, but the body’s cells don’t respond to it. Obesity plays a major role in most cases of type 2 diabetes. Genetic factors, however, can contribute to the onset of both types.

How Supplements Can Help
All the supplements can be used along with prescription drugs and by people with both types of diabetes. Taking some supplements may require altering dosages for insulin or the hypoglycemic drugs used for type 2 diabetes. Dosage changes must be supervised by your doctor.

What Else You Can Do
Exercise regularly. Those who burn more than 3,500 calories a week through exercise are half as likely to develop type 2 diabetes as those burning less than 500. People with type 1 can benefit from exercise too.
Lose weight. Being overweight is a major risk factor for type 2 diabetes.
Eat whole grains, fruits, and vegetables to keep blood sugar in check.
People with diabetes may find it beneficial to add soy foods to their diet. These products — including tofu, soy protein, soy milk, and soy flour — may improve glucose control, protect against heart disease, and lessen the stress on the kidneys.
The herb ginkgo biloba is useful for two common side effects of diabetes: nerve damage and poor circulation in the extremities. If you have signs of either complication or if you have trouble controlling your blood sugar levels, try taking ginkgo biloba at a dose of 40 mg three times a day.

Common Names:Bitter melon, bitter gourd, bitter squash or balsam-pear  in English, has many other local names. Goya  from Okinawan and karela from Sanskrit are also used by English-language speakers.

Habitat :  Momordica charantia is a tropical and subtropical vine of the family Cucurbitaceae, widely grown for edible fruit, which is among the most bitter of all vegetables. English names for the plant and its fruit include bitter melon or bitter gourd (translated from Chinese: ??; pinyin: kugua). The original home of the species is not known, other than that it is a native of the tropics. It is widely grown in India, South Asia, The Philippines, Southeast Asia, China, Africa and the Caribbean.

Also known as Ku gua, the herbaceous, tendril-bearing vine grows to 5 m. It bears simple, alternate leaves 4-12 cm across, with 3-7 deeply separated lobes. Each plant bears separate yellow male and female flowers.

The fruit has a distinct warty exterior and an oblong shape. It is hollow in cross-section, with a relatively thin layer of flesh surrounding a central seed cavity filled with large, flat seeds and pith. The fruit is most often eaten green, or as it is beginning to turn yellow. At this stage, the fruit’s flesh is crunchy and watery in texture, similar to cucumber, chayote or green bell pepper, but bitter. The skin is tender and edible. Seeds and pith appear white in unripe fruits; they are not intensely bitter and can be removed before cooking.

Bitter melon is commonly used as a vegetable in tropical areas such as East Africa, Asia, the Caribbean, and South America. The plant gets its name from the bitter taste of its fruit and juice; however, cooking it with the appropriate spices can reduce the bitterness. In addition to being a food source, bitter melon is employed as an herbal remedy in many parts of the world. While the seeds, leaves and vines of bitter melon may all be used, the fruit is used most often for medicinal purposes.

At lease three types of compounds in bitter melon are believed to lower blood sugar, which can benefit people with diabetes mellitus. It is still unclear whether these compounds work together or individually, but several controlled clinical studies have confirmed that bitter melon is beneficial in controlling the symptoms of diabetes.

Test-tube studies have also shown that two proteins found in bitter melon — alpha-momorcharin and beta-momorcharin — inhibit the AIDS virus. However, these studies have not been conducted in humans.

Culinary uses:
Bitter melons are seldom mixed with other vegetables due to the strong bitter taste, although this can be moderated to some extent by salting and then washing the cut melon before use.

Bitter melon is often used in Chinese cooking for its bitter flavor, typically in stir-fries (often with pork and douchi), soups, and also as tea.

It is also a popular vegetable in Indian and Pakistani cooking, where it is often prepared with potatoes and served with yogurt on the side to offset the bitterness. Bitter melon fried in oil and then stuffed with other spicy ingredients is very popular in Andhra Pradesh, a south Indian state.

Bitter melon is rarely used in mainland Japan, but is a significant component of Okinawan cuisine.

In Vietnam, raw bitter melon slices consumed with dried meat floss and bitter melon soup with shrimp are popular dishes.

It is prepared into various dishes in the Philippines, where it is known as ampalaya. Ampalaya may also be stir-fried with ground beef and oyster sauce, or with eggs and diced tomato. A very popular dish from the Ilocos region of the Philippines, pinakbet, consists mainly of bitter melons, eggplant, okra, string beans, tomatoes, lima beans, and other various regional vegetables stewed with a little bagoong-based stock.

The young shoots and leaves may also be eaten as greens; in the Philippines, where bitter melon leaves are most commonly consumed, they are called dahon (leaves) ng ampalaya. The seeds can also be eaten, and give off a sweet taste, but have been known to cause vomiting and stomach upset.

Medicinal uses:
Bitter melons have been used in various Asian traditional medicine systems for a long time . Like most bitter-tasting foods, bitter melon stimulates digestion. While this can be helpful in people with sluggish digestion, dyspepsia, and constipation, it can sometimes make heartburn and ulcers worse. The fact that bitter melon is also a demulcent and at least mild inflammation modulator, however, means that it rarely does have these negative effects, based on clinical experience and traditional reports.

Perhaps the best substantiated use to date is that of bitter melon for people with diabetes mellitus. Several preliminary (non-randomized, non-blinded) clinical trials suggest this benefit, though controlled trials are necessary for confirmation . In the Philippines, bitter melon tea is used in blood sugar control for poor people as exemplified in the very successful Operation Diabetes . In ayurvedic medicine, bitter melon is popularly seen as a “plant-insulin.” It has been demonstrated that bitter melon contains a protein similar to bovine insulin, sometimes referred to as p-insulin or polypeptide-p (Baldwa, et al. 1977). When purified and injected subcutaneously into patients with insulin-dependent diabetes mellitus (IDDM), it acted very similar to slow-acting animal insulins and was able to sustain patients . One child in this small study who previously had many side effects from bovine insulin was able to use p-insulin exclusively for 5 months with no sign of problems. This represents the potential for a vegetarian alternative to animal insulin for patients with IDDM, as well as a potentially more sustainable source of insulin, and should be further developed. It is not possible to substitute eating bitter melon for taking insulin.

Though it has been claimed that bitter melon’s bitterness comes from quinine, no evidence could be located supporting this claim. Bitter melon is traditionally regarded by Asians, as well as Panamanians and Colombians, as useful for preventing and treating malaria. Laboratory studies have confirmed that various species of bitter melon have anti-malarial activity, though human studies have not yet been published .

Laboratory tests suggest that compounds in bitter melon might be effective for treating HIV infection . As most compounds isolated from bitter melon that impact HIV have either been proteins or glycosproteins (lectins), neither of which are well-absorbed, it is unlikely that oral intake of bitter melon will slow HIV in infected people. It is possible oral ingestion of bitter melon could offest negative effects of anti-HIV drugs, if a test tube study can be shown to be true in people . In one preliminary clinical trial, an enema form of a bitter melon extract showed some benefits in people infected with HIV (Zhang 1992). Clearly more research is necessary before this could be recommended.

The other realm showing the most promise related to bitter melon is as an immunomodulator. One clinical trial found very limited evidence that bitter melon might improve immune cell function in people with cancer, but this needs to be verified and amplified in other research . If proven correct this is another way bitter melon could help people infected with HIV


How much bitter melon should I take?

For those who can withstand the bitter taste, many herbalists recommend eating one small melon; 100 millileters of a bitter melon decoction; or two ounces of fresh bitter melon juice per day. For people who cannot stand the taste, some practitioners recommend bitter melon tinctures (five millileters, two or three times per day).

What forms of bitter melon are available?

Fresh bitter melon and bitter melon juice can be found at many specialty stores and Asian markets. Bitter melon extracts and tinctures can be found at some health food stores.

What can happen if I take too much bitter melon? Are there any interactions I should be aware of? What precautions should I take?

Excessive amounts of bitter melon juice may cause abdominal pain and diarrhea. In addition, patients with hypoglycemia should avoid bitter melon, because it could theoretically worsen their condition.

At present, there are no well-known drug interactions with better melon. However, make sure to consult with a health care provider before taking bitter melon (or any other dietary supplement).

Obese diabetics at risk of kidney disease

Are you suffering from Type 1 diabetes? You better watch that weight around your waist. Researchers from the   University of Washington, Seattle, have found that adults with Type 1 diabetes who are obese, especially those who carry excess weight around the waist, are at an increased risk of developing kidney disease. In fact, according to the study that was presented at the recently concluded World Diabetes Congress, for every four-inch

increase in waist circumference, the risk of microalbuminuria    small amounts of the protein albumin in urine, the first sign of diabetic kidney disease  increased by 34%. Microalbuminuria is not only an important sign of kidney disease but also a marker of increased risk for cardiovascular disease.

Dr Ian H de Boer from the university’s department of nephrology said,”For patients with Type 1 diabetes, obesity is an important risk factor for the development of kidney disease. Our study suggests that lifestyle interventions, such as exercise and diet, will be useful in preventing kidney and heart disease in this group of people.” The study will appear in
the January 2007 issue of the Journal of American Society of Nephrology.

According to the researchers, the risk of microalbuminuria was significantly higher for patients who were suffering from central obesity fat around the midsection.
The study says,”Weight gain and central obesity are associated with insulin resistance, hypertension and dyslipidemia in Type 1 diabetes.

These metabolic abnormalities are risk factors for kidney disease. Whether waist circumference is associated with microalbuminuria was examined among 1,279 participants who had Type 1 diabetes.”
“Over 93 of 1,105 participants with normal albumin excretion rate developed microalbuminuria over the 5.8 year period.

In conclusion, waist circumference predicts the subsequent development of microalbuminuria in Type 1 diabetes.”

The diet myth in diabetes and heart diseases has been exploded.

Studies by city doctors have revealed that rice and food cooked in mustard oil, in right proportion, do not harm diabetics and heart patients.

A diabetic can have carbohydrate and fat, but in the right quantity,   stated endocrinology head of SSKM Hospital Subhankar Chowdhury.   The belief that mustard and groundnut oil should be replaced with sunflower seed, sunflower and other oils with low saturated fat is also erroneous.

According to doctors, mustard and groundnut oil do not contain saturated fat, which is harmful for diabetics and those with heart diseases. Unsaturated fat is of two types   poly and mono. The two main ingredients of polyunsaturated fat are n3 and n6. A balanced quantity of each is required in a diet.The ideal n6-n3 ratio is 10:1. In sunflower seed, sunflower and other oils containing unsaturated fat, the ratio is 70:1.

This ratio causes abnormal blood lipid levels and blood clot,   explained Chowdhury.
Oil containing monounsaturated fat is also good for diabetics and heart patients, he added. “Mustard oil has a good proportion of monounsaturated fat and n3 type in polyunsaturated fat. Ideally there should be a mixture of both.

Diabetics should not avoid carbohydrates altogether, said doctors.   Carbohydrate is usually substituted with protein. But too much protein can damage the kidney in the long run. For a diabetic, the risk is higher as the organs are affected by the disease,    said a doctor.

A balanced and sensible diet with complex carbohydrates and high fibre content like whole grain, fruits and salads, vegetables and other items low in fat and cholesterol is the key to managing diabetes,  opined a city-based endocrinologist.

Nutritionists feel food with low glycemic index (those that are absorbed slowly and, therefore, maintain the blood sugar level) are good for diabetics.
Oatmeal, whole wheat flour, whole pulses, fenugreek seeds, flax seeds and leafy vegetables have low glycemic index,  said Vijaya Agarwal, consultant nutritionist at AMRI Hospitals.

Frequent meals are advisable for maintaining a proper blood sugar level, signed off Agarwal.
