DEFINITION:
Anaphylaxis is an acute multi-system severe type I hypersensitivity reaction. The term comes from the Greek words ava ana (against) and  phylaxis (protection).It is  a life-threatening type of allergic reaction and it can occur within seconds or minutes of exposure to something you’re allergic to, such as the venom from a bee sting or a peanut.

The flood of chemicals released by your immune system during anaphylaxis can cause you to go into shock; your blood pressure drops suddenly and your airways narrow, blocking normal breathing. Signs and symptoms of anaphylaxis include a rapid, weak pulse, a skin rash, and nausea and vomiting. Common triggers of anaphylaxis include certain foods, some medications, insect venom and latex.

Due in part to the variety of definitions, between 1% and 15% of the population of the United States can be considered “at risk” for having an anaphylactic reaction if they are exposed to one or more allergens. Of those people who actually experience anaphylaxis, up to 1% may die as a result. Anaphylaxis results in approximately 1,500 deaths per year in the U.S. In England, mortality rates for anaphylaxis have been reported as up to 0.05 per 100,000 population, or around 10-20 a year. Anaphylactic reactions requiring hospital treatment appear to be increasing, with authorities in England reporting a threefold increase between 1994 and 2004.

Based on the pathophysiology, anaphylaxis can be divided into “true anaphylaxis” and “pseudo-anaphylaxis” or “anaphylactoid reaction.” The symptoms, treatment, and risk of death are the same; however, “true” anaphylaxis is caused by degranulation of mast cells or basophils mediated by immunoglobulin E (IgE), and pseudo-anaphylaxis occurs without IgE mediation.

Classification:
Biphasic anaphylaxis:..CLICK & SEE
Biphasic anaphylaxis is the recurrence of symptoms within 72 hours with no further exposure to the allergen. It occurs in between 1–20% of cases depending on the study examined. It is managed in the same manner as anaphylaxis.

Anaphylactic shock:...CLICK & SEE
Anaphylactic shock is anaphylaxis associated with systemic vasodilation which results in low blood pressure. It is also associated with severe bronchoconstriction to the point where the individual is unable to breathe.

Pseudoanaphylaxis:….CLICK & SEE
The presentation and treatment of pseudoanaphylaxis is similar to that of anaphylaxis. It however does not involve an allergic reaction but is due to direct mast cell degranulation. This can result from morphine, radiocontrast, aspirin and muscle relaxants.[11]

Active anaphylaxis:….CLICK & SEE
Active anaphylaxis is what is naturally observed. Two weeks or so after an animal, including humans, is exposed to certain allergens, active anaphylaxis (which is simply called “anaphylaxis”) would be elicited upon exposure to the same allergens.

Passive anaphylaxis:....CLICK & SEE
Passive anaphylaxis is induced in native animals which receive transfer of the serum experimentally from sensitized animals with certain allergens. Passive anaphylaxis would be provoked in the recipient animals after exposure to the same allergens.

SIGNS & SYMPTOMS :
Anaphylaxis can present with many different symptoms due to the systemic effects of histamine release. These usually develop over minutes to hours.[9] The most common areas affected include: skin (80% to 90%), respiratory (70%), gastrointestinal (30% to 45%), heart and vasculature (10% to 45%), and central nervous system (10% to 15%).

Skin:
Skin involvement may include generalized hives, itchiness, flushing, and swelling of the lips, tongue or throat….

Respiratory:
Respiratory symptoms may include shortness of breath, wheezes or stridor, and low oxygen.

Gastrointestinal:

Gastrointestinal symptoms may include crampy abdominal pain, diarrhea, and vomiting.

Cardiovascular:
Due to the presence of histamine releasing cells in the heart, coronary artery spasm may occur with subsequent myocardial infarction or dysrhythmia.

Nervous sys:

temA drop in blood pressure may result in a feeling of lightheadedness and loss of consciousness. There may be a loss of bladder control and muscle tone, and a feeling of anxiety and “impending doom”.

CAUSES:
Anaphylaxis can occur in response to any allergen. Common triggers include insect bites or stings, foods, medication and latex rubber

Tissues in different parts of the body release histamine and other substances. This causes the airways to tighten and leads to other symptoms.

Some drugs (morphine, x-ray dye, and others) may cause an anaphylactic-like reaction (anaphylactoid reaction) when people are first exposed to them. Aspirin may also cause a reaction. These reactions are not the same as the immune system response that occurs with “true” anaphylaxis. However, the symptoms, risk for complications, and treatment are the same for both types of reactions.

Anaphylaxis can occur in response to any allergen. Common causes include:

•Drug allergies :Any medication may potentially trigger anaphylaxis. The most common to do so include antibiotics (?-lactam antibiotics in particular), aspirin, ibuprofen, and other analgesics. Some drugs (polymyxin, morphine, x-ray contrast and others) may cause an “anaphylactoid” reaction (anaphylactic-like reaction) on the first exposure. This is usually due to a toxic reaction, rather than the immune system mechanism that occurs with “true” anaphylaxis. The symptoms, risk for complications without treatment, and treatment are the same, however, for both types of reactions. •Food allergies :The most common are peanut, tree nuts, shellfish, fish, milk, and egg. Severe cases are usually the result of ingesting the allergen…...CLICK & SEE

Less common causes of anaphylaxis include:

*Latex
*Muscle relaxants used during general anesthesia
*Exercise

Anaphylaxis triggered by exercise varies from person to person. In some people, aerobic activity, such as jogging, triggers anaphylaxis. In others, less intense physical activity, such as walking, can trigger a reaction. Eating certain foods before exercise or exercising when the weather is hot, cold or humid has also been linked to anaphylaxis in some people. Talk with your doctor about any precautions you should take when exercising.

Anaphylaxis symptoms are sometimes caused by aspirin, other nonsteroidal anti-inflammatory drugs — such as ibuprofen (Advil, Motrin, others) and naproxen sodium (Aleve, Midol Extended Relief) — and the intravenous (IV) contrast used in some X-ray imaging tests. Although similar to allergy-induced anaphylaxis, this type of reaction isn’t triggered by allergy antibodies.

If you don’t know what triggers your allergy attack, your doctor may do tests to try to identify the offending allergen. In some cases, the cause of anaphylaxis is never identified. This is known as idiopathic anaphylaxis.

Anaphylaxis is life-threatening and can occur at any time. Risks include a history of any type of allergic reaction.

DIAGNOSIS:
Anaphylaxis is diagnosed with high likelihood based on clinical criteria. These criteria are fulfilled when any one of the following three is true:[14]

1.Symptom onset within minutes to several hours of allergen exposure with involvement of the skin or mucosal tissue and any of the following: hives, itchiness, or swelling of the airway; plus either respiratory difficulty or a low blood pressure.

2.Any two or more of the following symptoms within minutes to several hours of allergen exposure: a. Involvement of the skin or mucosa b. Respiratory difficulties c. Low blood pressure d. Gastrointestinal symptoms

3.Low blood pressure within minutes to several hours after exposure to known allergen

Apart from its clinical features, blood tests for tryptase (released from mast cells) might be useful in diagnosing anaphylaxis.

Allergy testing may help in determining what triggered the anaphylaxis. In this setting, skin allergy testing (with or without patch testing) or RAST blood tests can sometimes identify the cause.

TREATMENT :
Anaphylaxis is an emergency condition requiring immediate professional medical attention. Call 911 immediately.

Check the person’s airway, breathing, and circulation (the ABC’s of Basic Life Support). A warning sign of dangerous throat swelling is a very hoarse or whispered voice, or coarse sounds when the person is breathing in air. If necessary, begin rescue breathing and CPR.

1.Call 911.
2.Calm and reassure the person.
3.If the allergic reaction is from a bee sting, scrape the stinger off the skin with something firm (such as a fingernail or plastic credit card). Do not use tweezers — squeezing the stinger will release more venom.
4.If the person has emergency allergy medication on hand, help the person take or inject the medication. Avoid oral medication if the person is having difficulty breathing.
5.Take steps to prevent shock. Have the person lie flat, raise the person’s feet about 12 inches, and cover him or her with a coat or blanket. Do NOT place the person in this position if a head, neck, back, or leg injury is suspected, or if it causes discomfort.

PROVIDING FIRST AID:
Although emergency medical help is essential, there are things that must be done to improve survival chances. If the person affected is conscious and having breathing difficulties, help them sit up. If they’re shocked with low blood pressure, they’re better off lying flat with their legs raised.

If the person is unconscious, check their airways and breathing, and put them in the recovery position.

If you know that the person is susceptible to anaphylaxis, ask if they carry a preloaded adrenaline syringe. If necessary, help the person inject it into their thigh muscle.  If available, antihistamines and steroids should also be given.

DO NOT:
•Do NOT assume that any allergy shots the person has already received will provide complete protection.
•Do NOT place a pillow under the person’s head if he or she is having trouble breathing. This can block the airways.
•Do NOT give the person anything by mouth if the person is having trouble breathing.
Paramedics or physicians may place a tube through the nose or mouth into the airways (endotracheal intubation) or perform emergency surgery to place a tube directly into the trachea (tracheostomy or cricothyrotomy).

The person may receive antihistamines, such as diphenhydramine, and corticosteroids, such as prednisone, to further reduce symptoms (after lifesaving measures and epinephrine are given).

PROGNOSIS:
Anaphylaxis is a severe disorder that can be life-threatening without prompt treatment. However, symptoms usually get better with the right therapy, so it is important to act right away.

Possible Complications:
•Airway blockage
•Cardiac arrest (no effective heartbeat)
•Respiratory arrest (no breathing)
•Shock

RISK FACTORS:

There aren’t many known risk factors for anaphylaxis, but some things that may increase your risk include:

*A personal history of anaphylaxis. If you’ve experienced anaphylaxis once, your risk of having this serious reaction is increased. Future reactions may be more severe than the first reaction.

*Allergies or asthma. People who have either condition are at increased risk of having anaphylaxis.

*A family history.
If you have family members who have experienced exercised-induced anaphylaxis, your risk of developing this type of anaphylaxis is higher than it is for someone without a family history.

PREVENTION:
Immunotherapy with Hymenoptera venoms is effective against allergies to bees, wasps, hornets, yellow jackets, white faced hornets, and fire ants.

The greatest success with prevention of anaphylaxis has been the use of allergy injections to prevent recurrence of sting allergy. The risk to an individual from a particular species of insect depends on complex interactions between likelihood of human contact, insect aggression, efficiency of the venom delivery apparatus, and venom allergenicity. Venom immunotherapy reduces risk of systemic reactions below 3%.[citation needed] One simple method of venom extraction has been electrical stimulation to obtain venom, instead of dissecting the venom sac.

A potential vaccine has been developed to prevent anaphylaxis due to peanut and tree nut allergies if they are exposed to a small amount of peanuts or nuts. Although it shows some promise to reduce the likelihood of anaphylaxis in affected individuals, the vaccine has not yet been approved for marketing and distribution. Desensitization techniques are also being studied for peanut allergies.

•Avoid triggers such as foods and medications that have caused an allergic reaction (even a mild one) in the past. Ask detailed questions about ingredients when you are eating away from home. Also carefully examine ingredient labels.

•If you have a child who is allergic to certain foods, introduce one new food at a time in small amounts so you can recognize an allergic reaction.

•People who know that they have had serious allergic reactions should wear a medical ID tag.

•If you have a history of serious allergic reactions, carry emergency medications (such as a chewable form of diphenhydramine and injectable epinephrine or a bee sting kit) according to your health care provider’s instructions.

•Do not use your injectable epinephrine on anyone else. They may have a condition (such as a heart problem) that could be negatively affected by this drug.

Welcome to the world of medical palmistry, a branch of science based on documented and proven scientific observations.

A detailed examination of the palm does provide valuable clues to a person’s medical history, lifestyle, diseases and life expectancy. Palms and fingers have characteristic creases, whorls, arches and loops. These are unique in each individual and never identical, even in twins. One of the oldest biometric methods of establishing positive identity is by using fingerprints.

“Palmar creases” form in an unborn baby as it holds its hands tightly clenched during the 12th week. Normally this forms three palmar creases or lines. Any physical, medical or drug-induced injury to the foetus during the first three months is reflected permanently as abnormal palmar creases. This can be picked up on ultrasound examination after the 12th week. If the creases are abnormal, the foetus should be closely monitored for associated abnormalities in the kidney, heart and other organ

Sometimes the upper two lines fuse to form a single palmar crease or simian line that stretches across the open palm. A single palmar crease can be present in one out of 30 apparently normal people. It is more common in males and is usually present only on one hand. One or both parents of these children may have the abnormal crease on one hand. This is a minor aberration and warrants monitoring as these children may reveal mild abnormalities in other organs in later life. It is also associated with certain chromosomal anomalies, the most common of which is Down’s Syndrome (Trisomy 21).

Not all abnormal palmar creases are hereditary or genetic. Alcoholic women who continue to drink during pregnancy can produce children with “foetal alcohol syndrome” and a single palmar crease.

People with mental illnesses have more open loops and fewer whorls on their finger tips. Those prone to chronic diseases like leprosy and tuberculosis also tend to have only two lines on the palm, with the abnormal line just above the thumb.

Normally, a person has 10 fingers and toes. In one in 1,000 births, there may be extra digits, separate, complete, incomplete or fused. These defects can be associated with other internal congenital malformations, and so a detailed examination must be done for any affected newborn.

Marfans syndrome is a genetic disorder in which the person has “arachnodactyly” or abnormally long fingers like spider legs. This can be diagnosed before birth through ultrasound.

Congenital hypothyroidism, certain renal diseases and some forms of dwarfism are linked with a “tripartiate” hand — where the index, middle and ring fingers are the same length.

Cigarette smokers, people suffering from chronic respiratory ailments, and those with congenital heart disease may have blue nails. Some lung diseases like bronchiectasis, and chronic intestinal diseases may bend the nail like a convex parrot beak, a condition called “clubbing”. Jaundice causes the skin of the palms to turn yellow. Carotenemia produces a similar appearance. It is a harmless condition and is caused by an excess consumption of yellow carotene containing fruits and vegetables.

Hormone levels in the uterus also influences finger length. A person (irrespective of sex) with the index finger shorter than the ring finger will have had more testosterone (male hormone) while in the womb, and a person with an index finger longer than the ring finger will have had more eostrogen (female hormone). Professional women, especially women scientists, tend to have higher levels of testosterone vis-a-vis their oestrogen level, making their brains closer to those of men in general. The converse is true with men working in the fine arts and social sciences.

The position in which we hold our palms is a reflection of the body mass index or BMI (weight in kilogram divided by height in metre squared). A BMI more than 30 is diagnostic of obesity. Such people tend to hold their hands with the thumbs facing backwards as they stand. Overweight people with a BMI between 25 and 30 hold their arms with the thumb facing sideways. People of normal weight with a BMI between 20 and 25 stand with their palms facing forwards.

So, remember, your palms will reveal a lot about your health, but only if you go to a medical palmist.

Sex, Chemicals and Longevity

A substance released during intercourse can extend the lifespan of an organism:

Intimate bedroom moments have given researchers new clues about combating ageing. A team of European researchers, led by a biologist in Austria, has found that a substance released in copious amounts during sexual intercourse can actually extend the lifespan of a range of organisms — from flies to worms and even mammals.

Spermidine, an important constituent of seminal fluid which helps neutralise the acidic pH of the vagina — the first step in the sperm’s journey towards fertilising an egg — is known to be necessary for cell growth and maturation. Scientists have previously found that when ageing sets in, its concentration dips in living cells. However, it was unclear if its decrease was the cause or consequence of ageing.

But in an interesting study reported online in the journal Nature Cell Biology, scientists led by Frank Madeo of the Institute of Molecular Biosciences, University of Graz, Austria, have shown that the levels of spermidine can be propped up in cells through the oral route. Experimenting with laboratory mice, the scientists found that an additional dose of spermidine rejuvenated the cells, as a result of which the animals lived longer.

“Our studies have shown that spermidine-treated fruit flies and worms can live up to 30 per cent longer than their average lifespan,” Madeo told KnowHow.

Madeo thinks the study is important for a variety of reasons. First, spermidine is an intrinsically natural compound abundantly found in every type of cell or eukaryotic organism and hence part of normal human nutrition. It is found in abundant quantities in soybeans as well as grapefruit, a subtropical citrus fruit known for its bitter taste.

“Intriguingly, spermidine is (released)… in huge concentrations during sexual intercourse. This means that it is already approved by nature,” says Madeo. The second fascinating thing, according to him, is the way it does cellular “clean up”. Spermidine is capable of inducing a process called “autophagy” by which a cell itself destroys its own degraded components. Scientists have known for long that autophagy plays a critical role in halting the progression of cancers and certain neurodegenerative disorders.

A similar study about three months ago by a team of French scientists found that melatonin, a naturally occurring hormone, when supplied externally, could delay the onset of ageing.

“It is too early to speculate if and to which extent these two substances (melatonin and spermidine) might interfere with ageing through similar mechanisms. It is worthwhile, however, to note that both the studies addressed the effect of a single compound that is naturally produced in the human body,” says Madeo.

Altering the concentrations of these compounds through external supply seems to be efficient in retarding the signs of ageing, he adds.

The advantage, particularly in the case of spermidine, says Madeo, is that its application is easy. Madeo and his team received positive results when they used spermidine simply as a nutritional supplement to food or drinking water in their experiments.

The scientists are also hopeful that spermidine will help them formulate therapies for several neurodegenerative diseases such as Alzheimer’s and Parkinson’s disease which are normally associated with longevity.

In fact, Madeo along with his associate Tobias Eisenberg is currently engaged in a series of experiments on mice to see whether spermidine could be effective against such neurodegenerative diseases.

Milind Gajnan Watve, professor of biology at the Indian Institute of Science Education and Research, Pune, feels the work is interesting. It indicates that the biology of ageing is very similar in a range of organisms, from simple single-cell creatures to humans. “The study is promising, but has a long way to go,” he adds.

Definition:
Ear infections are one of the most common reasons parents take their children to the doctor. While there are different types of ear infections, the most common is called otitis media, which means an inflammation and infection of the middle ear. The middle ear is located just behind the eardrum.

Symptoms

*The child may have symptoms of a cold, or the ear infection may start shortly after having a cold.

All acute ear infections include fluid behind the eardrum. You can use an electronic ear monitor, such as EarCheck, to detect this fluid at home. The device is available at pharmacies.

Possible Causes:
Ear infections are common in infants and children in part because their eustachian tubes become clogged easily. For each ear, a eustachian tube runs from the middle ear to the back of the throat. Its purpose is to drain fluid and bacteria that normally occurs in the middle ear. If the eustachian tube becomes blocked, fluid can build up and become infected.
Anything that causes the eustachian tubes and upper airways to become inflamed or irritated, or cause more fluids to be produced, can lead to a blocked eustachian tube. These include:

*Colds and sinus infections
*Allergies
*Tobacco smoke or other irritants
*Infected or overgrown adenoids
*Excess mucus and saliva produced during teething

Ear infections are also more likely if a child spends a lot of time drinking from a sippy cup or bottle while lying on his or her back. Contrary to popular opinion, getting water in the ears will not cause an acute ear infection, unless the eardrum has a hole from a previous episode.

Ear infections occur most frequently in the winter. An ear infection is not itself contagious, but a cold may spread among children and cause some of them to get ear infections.

Risk factors:

*Not being breast-fed
*Recent ear infection
*Recent illness of any type (lowers resistance of the body to infection)
*Day care (especially with more than 6 children)
*Pacifier use
*Genetic factors (susceptibility to infection may run in families)
*Changes in altitude or climate
*Cold climate
*Sudden change of weather

Diagnosis:

Signs and tests
The doctor will ask questions about whether your child (or you) have had ear infections in the past and will want you to describe the current symptoms, including whether your child has had any symptoms of a cold or allergies recently. Your doctor will examine your child’s throat, sinuses, head, neck, and lungs.

Using an instrument called an otoscope, the doctor will look inside your child’s ears. If infected, there may be areas of dullness or redness or there may be air bubbles or fluid behind the eardrum. The fluid may be bloody or purulent (filled with pus). The physician will also check for any sign of perforation (hole or holes) in the eardrum.

A hearing test may be recommended if your child has had persistent (chronic and recurrent) ear infections

Modern  Treatment
The goals for treating ear infections include relieving pain, curing the infection, preventing complications, and preventing recurrent ear infections. Most ear infections will safely clear up on their own without antibiotics. Often, treating the pain and allowing the body time to heal itself is all that is needed:

*Apply a warm cloth or warm water bottle.
*Use over-the-counter pain relief drops for ears.
*Take over-the counter medications for pain or fever, like ibuprofen or acetaminophen. DO NOT give aspirin to children.
*Use prescription ear drops to relieve pain.

ANTIBIOTICS
Some ear infections require antibiotics to clear the infection and to prevent them from becoming worse. This is more likely if the child is under age 2, has a fever, is acting sick (beyond just the ear), or is not improving over 24 to 48 hours.

However, for several years there was a tendency to over-prescribe antibiotics, leading to the increasing numbers of bacteria that are resistant to these drugs. Joint guidelines from the American Academy of Pediatrics and the American Academy of Family Physicians are aimed at using antibiotics for ear infections when they are most needed. If the antibiotics do not seem to be working within 48 to 72 hours, contact your doctor to consider switching to a stronger antibiotic. Usually there is no benefit to more than two, or at the most three, rounds of appropriate antibiotics.

SURGERY
If there is fluid in the middle ear and the condition persists, even with antibiotic treatment, a healthcare provider may recommend myringotomy (surgical opening of the eardrum) to relieve pressure and allow drainage of the fluid. This may or may not involve the insertion of tympanostomy tubes (often referred to as ear tubes). In this procedure, a tiny tube is inserted into the eardrum, keeping open a small hole that allows air to get in so fluids can drain more easily down the eustachian tube. Tympanostomy tube insertion is done under general anesthesia. Usually the tubes fall out by themselves. Those that don’t may be removed in your doctor’s office.

If the adenoids are enlarged, surgical removal may be considered, especially if you have chronic, recurrent ear infections. Removing tonsils does not seem to help with ear infections.

Prognosis:
Ear infections are curable with treatment but may recur. They are not life threatening but may be quite painful.

Prevention:
What can kids do to prevent ear infections? You can avoid places where people are smoking, for one. Cigarette smoke can keep your eustachian tubes from working properly.
You can reduce your child’s risk of ear infections with the following practices:

*Wash hands and toys frequently. Also, day care with 6 or fewer children can lessen your child’s chances of getting a cold or similar infection. This leads to fewer ear infections.
*Avoid pacifiers, especially at daycare.
*Breastfeed — this makes a child much less prone to ear infections. But, if bottle feeding, hold your infant in an upright, seated position.
*Don’t expose your child to secondhand smoke.
*The pneumococcal vaccine prevents infections from the organism that most commonly causes acute ear infections and many respiratory infections.
*Some evidence suggests that xylitol, a natural sweetener, may reduce ear infections.
*Avoid overusing antibiotics.
