Peptic Ulcer

Definition:
Peptic ulcers are open sores that develop on the inside lining of your stomach, upper small intestine or esophagus. The most common symptom of a peptic ulcer is pain.Not long ago, the common belief was that peptic ulcers were a result of lifestyle. Doctors now know that a bacterial infection or some medications — not stress or diet — cause most ulcers of the stomach and upper part of the small intestine (duodenum). Esophageal ulcers also may occur and are typically associated with the reflux of stomach acid.

A peptic ulcer, also known as PUD or peptic ulcer disease, is an ulcer (defined as mucosal erosions equal to or greater than 0.5 cm) of an area of the gastrointestinal tract that is usually acidic and thus extremely painful. As much as 80% of ulcers are associated with Helicobacter pylori, a spiral-shaped bacterium that lives in the acidic environment of the stomach, however only 20% of those cases go to a doctor. Ulcers can also be caused or worsened by drugs such as Aspirin and other NSAIDs. Contrary to general belief, more peptic ulcers arise in the duodenum (first part of the small intestine, just after the stomach) than in the stomach. About 4% of stomach ulcers are caused by a malignant tumor, so multiple biopsies are needed to make sure. Duodenal ulcers are generally benign.

Classification

A peptic ulcer may arise at various locations:

* Stomach (called gastric ulcer)
* Duodenum (called duodenal ulcer)
* Esophagus (called esophageal ulcer)
* Meckel’s Diverticulum (called Meckel’s Diverticulum ulcer)

Signs and symptoms:

Burning pain is the most common peptic ulcer symptom. The pain is caused by the ulcer and is aggravated by stomach acid coming in contact with the ulcerated area. The pain typically may:

* Be felt anywhere from your navel to your breastbone
* Last from a few minutes to many hours
* Be worse when your stomach is empty
* Flare at night
* Often be temporarily relieved by eating certain foods that buffer stomach acid or by taking an acid-reducing medication
* Come and go for a few days or weeks

Less often, ulcers may cause severe signs or symptoms such as:

* The vomiting of blood — which may appear red or black
* Dark blood in stools or stools that are black or tarry
* Nausea or vomiting
* Unexplained weight loss
* Chest pain

A history of heartburn, gastroesophageal reflux disease (GERD) and use of certain forms of medication can raise the suspicion for peptic ulcer. Medicines associated with peptic ulcer include NSAID (non-steroid anti-inflammatory drugs) that inhibit cyclooxygenase, and most glucocorticoids (e.g. dexamethasone and prednisolone).

In patients over 45 with more than 2 weeks of the above symptoms, the odds for peptic ulceration are high enough to warrant rapid investigation by EGD (see below).

The timing of the symptoms in relation to the meal may differentiate between gastric and duodenal ulcers: A gastric ulcer would give epigastric pain during the meal, as gastric acid is secreted, or after the meal, as the alkaline duodenal contents reflux into the stomach. Symptoms of duodenal ulcers would manifest mostly before the meal — when acid (production stimulated by hunger) is passed into the duodenum. However, this is not a reliable sign in clinical practice.
Causes:
Depending on their location, peptic ulcers have different names:

* Gastric ulcer. This is a peptic ulcer that occurs in your stomach.
* Duodenal ulcer. This type of peptic ulcer develops in the first part of the small intestine (duodenum).
* Esophageal ulcer. An esophageal ulcer is usually located in the lower section of your esophagus. It’s often associated with chronic gastroesophageal reflux disease (GERD).

The culprit in most cases
Although stress and spicy foods were once thought to be the main causes of peptic ulcers, doctors now know that the cause of most ulcers is the corkscrew-shaped bacterium Helicobacter pylori (H. pylori).

H. pylori lives and multiplies within the mucous layer that covers and protects tissues that line the stomach and small intestine. Often, H. pylori causes no problems. But sometimes it can disrupt the mucous layer and inflame the lining of the stomach or duodenum, producing an ulcer. One reason may be that people who develop peptic ulcers already have damage to the lining of the stomach or small intestine, making it easier for bacteria to invade and inflame tissues.

H. pylori is a common gastrointestinal infection around the world. In the United States, one in five people younger than 30 and half the people older than 60 are infected. Although it’s not clear exactly how H. pylori spreads, it may be transmitted from person to person by close contact, such as kissing. People may also contract H. pylori through food and water.

H. pylori is the most common, but not the only, cause of peptic ulcers. Besides H. pylori, other causes of peptic ulcers, or factors that may aggravate them, include:

*Regular use of pain relievers. Nonsteroidal anti-inflammatory drugs (NSAIDs) can irritate or inflame the lining of your stomach and small intestine. The medications are available both by prescription and over-the-counter. Nonprescription NSAIDs include aspirin, ibuprofen (Advil, Motrin, others), naproxen (Aleve) and ketoprofen (Orudis KT). To help avoid digestive upset, take NSAIDs with meals.

NSAIDs inhibit production of an enzyme (cyclooxygenase) that produces prostaglandins. These hormone-like substances help protect your stomach lining from chemical and physical injury. Without this protection, stomach acid can erode the lining, causing bleeding and ulcers.
* Smoking. Nicotine in tobacco increases the volume and concentration of stomach acid, increasing your risk of an ulcer. Smoking may also slow healing during ulcer treatment.
* Excessive alcohol consumption. Alcohol can irritate and erode the mucous lining of your stomach and increases the amount of stomach acid that’s produced. It’s uncertain, however, whether this alone can progress into an ulcer or whether other contributing factors must be present, such as H. pylori bacteria or ulcer-causing medications, such as NSAIDs.
* Stress. Although stress per se isn’t a cause of peptic ulcers, it’s a contributing factor. Stress may aggravate symptoms of peptic ulcers and, in some cases, delay healing. You may undergo stress for a number of reasons — an emotionally disturbing circumstance or event, surgery, or a physical trauma, such as a burn or other severe injury.

Complications:

* Gastrointestinal bleeding is the most common complication. Sudden large bleeding can be life threatening[2]. It occurs when the ulcer erodes one of the blood vessels.
* Perforation (a hole in the wall) often leads to catastrophic consequences. Erosion of the gastro-intestinal wall by the ulcer leads to spillage of stomach or intestinal content into abdominal cavity. Perforation at the anterior surface of stomach leads to acute peritonitis, initially chemical and later bacterial peritonitis. Often first sign is sudden intense abdominal pain. Posterior wall perforation leads to pancreatitis; pain in this situation often radiates to back.
* Penetration is when the ulcer continues into adjacent organs such as liver and pancreas[3].
* Scarring and swelling due to ulcers causes narrowing in the duodenum and gastric outlet obstruction. Patient often presents with severe vomiting.
* Pyloric Stenosis

Diagnosis:
n order to detect an ulcer, your doctor may have you undergo the following diagnostic tests:

* Upper gastrointestinal (upper GI) X-ray. Your doctor may begin with this test, which outlines your esophagus, stomach and duodenum. During the X-ray, you swallow a white, metallic liquid (containing barium) that coats your digestive tract and makes an ulcer more visible. An upper GI X-ray can detect some ulcers, but not all.
* Endoscopy. This procedure may follow an upper GI X-ray if the X-ray suggests a possible ulcer, or your doctor may perform endoscopy first. In this more sensitive procedure, a long, narrow tube with an attached camera is threaded down your throat and esophagus into your stomach and duodenum. With this instrument, your doctor can view your upper digestive tract and identify an ulcer.

If your doctor detects an ulcer, he or she may remove small tissue samples (biopsy) near the ulcer. These samples are examined under a microscope to rule out cancer. A biopsy can also identify the presence of H. pylori in your stomach lining. Depending on where the ulcer is found, your doctor may recommend a repeat endoscopy after two to three months to confirm that the ulcer is healing.

Additional tests
In addition to a biopsy, these other tests can determine if the cause of your ulcer is H. pylori infection:

* Blood test. This test checks for the presence of H. pylori antibodies. A disadvantage of this test is that it sometimes can’t differentiate between past exposure and current infection. After H. pylori bacteria have been eradicated, you may still have a positive result for many months.
* Breath test. This procedure uses a radioactive carbon atom to detect H. pylori. First, you blow into a small plastic bag, which is then sealed. Then, you drink a small glass of clear, tasteless liquid. The liquid contains radioactive carbon as part of a substance (urea) that will be broken down by H. pylori. Thirty minutes later, you blow into a second bag, which also is sealed. If you’re infected with H. pylori, your second breath sample will contain the radioactive carbon in the form of carbon dioxide.

The advantage of the breath test is that it can monitor the effectiveness of treatment used to eradicate H. pylori, detecting when the bacteria have been killed or eradicated. With the blood test, H. pylori antibodies may sometimes still be present a year or more after the infection is gone.
* Stool antigen test. This test checks for H. pylori in stool samples. It’s useful both in helping to diagnose H. pylori infection and in monitoring the success of treatment.

Treatment:
Younger patients with ulcer-like symptoms are often treated with antacids or H2 antagonists before EGD is undertaken. Bismuth compounds may actually reduce or even clear organisms.

Patients who are taking nonsteroidal anti-inflammatories (NSAIDs) may also be prescribed a prostaglandin analogue (Misoprostol) in order to help prevent peptic ulcers, which may be a side-effect of the NSAIDs.

When H. pylori infection is present, the most effective treatments are combinations of 2 antibiotics (e.g. Clarithromycin, Amoxicillin, Tetracycline, Metronidazole) and 1 proton pump inhibitor (PPI), sometimes together with a bismuth compound. In complicated, treatment-resistant cases, 3 antibiotics (e.g. amoxicillin + clarithromycin + metronidazole) may be used together with a PPI and sometimes with bismuth compound. An effective first-line therapy for uncomplicated cases would be Amoxicillin + Metronidazole + Rabeprazole (a PPI). In the absence of H. pylori, long-term higher dose PPIs are often used.

Treatment of H. pylori usually leads to clearing of infection, relief of symptoms and eventual healing of ulcers. Recurrence of infection can occur and retreatment may be required, if necessary with other antibiotics. Since the widespread use of PPI’s in the 1990s, surgical procedures (like “highly selective vagotomy”) for uncomplicated peptic ulcers became obsolete.

Perforated peptic ulcer is a surgical emergency and requires surgical repair of the perforation. Most bleeding ulcers require endoscopy urgently to stop bleeding with cautery or injection.

The word liquorice is derived (via the Old French licoresse) from the Greek    (glukurrhiza), meaning “sweet root”,   from    (glukus), “sweet”  +  (rhiza), “root”,  the name provided by Dioscorides.  It is usually spelled liquorice in British usage, but licorice in the United States and Canada. It is called erk-soos   in Arabic, athimadhuram   in Telugu, jyeshthamadhu   in Kannada,   in Urdu, athimadhuram   in Tamil, irattimadhuram   in Malayalam, yastimadhu   in Sanskrit, mulethi   in Hindi, Vel Mee  in Sinhalese, jethimadh  in Gujarati, and jyeshthamadh  in Marathi.

Habitat: Glycyrrhiza Glabra is  native to southern Europe, India, and parts of Asia. Description:
The liquorice plant is a herbaceous perennial legume. It goows to 1 m in height, with pinnate leaves about 7–15 cm (2.8–5.9 in) long, with 9–17 leaflets. The flowers are 0.8–1.2 cm (1?3–1?2 in) long, purple to pale whitish blue, produced in a loose inflorescence. The fruit is an oblong pod, 2–3 cm (3?4–1 1?6 in) long, containing several seeds. The roots are stoloniferous.

In Ayurveda Yashtimadhu is known as an aphrodisiac and a rejuvenating tonic. It is an excellent natural herb that is used for treating various ailments like throat congestions, coughs, respiratory disorders and tuberculosis. Yastimadhu also helps inincreasing the appetite by facilitating proper evacuation of stools. This herb has a specialaction on kapha, which helps in expectoration of the accumulated kapha.

Some recent scientific studies have shown that Yashtimadhu also acts as a memory enhancer and mental rejuvenator.

The isoflavene glabrene and the isoflavane glabridin, found in the roots of liquorice, are phytoestrogens.

Liquorice flavour is found in a wide variety of candies or sweets. In most of these candies, the taste is reinforced by aniseed oil so the actual content of liquorice is very low. Liquorice confections are primarily purchased by consumers in the European Union.

In the Netherlands, where liquorice candy (drop) is one of the most popular forms of sweets, only a few of the many forms that are sold contain aniseed, although mixing it with mint, menthol, or with laurel is quite popular. Mixing it with ammonium chloride (salmiak) is also popular. The most popular liquorice, known in the Netherlands as zoute drop (salty liquorice), actually contains very little salt, i.e., sodium chloride. The salty taste is probably due to ammonium chloride and the blood pressure-raising effect is due to glycyrrhizin. Strong, salty sweets are popular in Nordic countries.

Pontefract in Yorkshire was the first place where liquorice mixed with sugar began to be used as a sweet in the same way it is in the modern day.[20] Pontefract cakes were originally made there. In County Durham, Yorkshire, and Lancashire, it is colloquially known as ‘Spanish’, supposedly because Spanish monks grew liquorice root at Rievaulx Abbey near Thirsk.

In Italy (particularly in the south), Spain, and France, liquorice is popular in its natural form. The root of the plant is simply dug up, washed, dried, and chewed as a mouth freshener. Throughout Italy, unsweetened liquorice is consumed in the form of small black pieces made only from 100% pure liquorice extract; the taste is bitter and intense. In Calabria a popular liqueur is made from pure liquorice extract.

Liquorice is also very popular in Syria and Egypt, where it is sold as a drink, in shops as well as street vendors. It is used for its expectorant qualities in folk medicine in Egypt.

Dried liquorice root can be chewed as a sweet. Black liquorice contains about 100 calories[dubious – discuss] per ounce (15 kJ/g).

As an anti-hepatotoxic licorice is effective in the treatment of chronic hepatitis and cirrhosis, for which it is been widely used in Japan. Much of the liver orientated research has focused upon the triterpene glycyrrhizin. This inhibits hepatocyte injury caused by carbon tetrachloride, benzene hexachloride and PCB. Antibody production is enhanced by glycyrrhizin, possibly through the production of interleukin.

Glycyrrhizin inhibits the growth of several DNA and RNA viruses, inactivating Herpes simplex virus particles irreversibly. It has a variety of uses in bronchial problems such as catarrh, bronchitis and coughs in general. Liquorice is used in allopathic medicine as a treatment for peptic ulceration, a similar use to its herbal use in gastritis and ulcers. It can be used in the relief of abdominal colic.

It nourishes the brain-increasing cranial and cerebrospinal fluid. Also improves complexion, hair, and vision.

It is used for:Sore throat with hoarseness of voice and cough.Acid peptic disease.Chronic liver diseases General tonic.

Since Hippocrates’ day licorice has been prescribed for dropsy because it does, indeed, prevent thirst–probably the only sweet thing that does. The chief medicinal action of licorice is as a demulcent and emollient. Its soothing properties make it excellent in throat and chest complaints and it is a very common ingredient in throat pastilles and cough mixtures. It is also widely used in other medicines to counteract bitter tastes and make them more palatable. Recent research has shown that it has a pain-killing effect on stomach ulcers and prolonged use raises the blood pressure. Medicinally the dried peeled root has been decocted to allay coughs, sore throat, laryngitis, and urinary and intestinal irritations. The root is expectorant, diuretic, demulcent, antitussive, anti-inflammatory, and mildly laxative. It has proven helpful in inflammatory upper respiratory disease, Addison’s disease, and gastric and duodenal ulcers. Side effects may develop in ulcer treatment. Licorice may increase venous and systolic arterial pressure causing some people to experience edema, and hypertension. In some countries, licorice has been used to treat cancers. Licorice stick, the sweet earthy flavored stolons, are chewed. Licorice chew sticks blackened Napoleon’s teeth. In the 1940s Dutch physicians tested licorice’s reputation as an aid for indigestion. They came up with a derivative drug, carbenoxolone, that promised to help peptic ulcer patients by either increasing the life span of epithelial cells in the stomach or inhibiting digestive activity in general. Many cures were achieved in the experiments, but negative side effects–the patients’ faces and limbs swelled uncomfortably–outweighed the cures.

Certain agents in licorice have recently been credited with antibacterial and mild antiviral effects; licorice may be useful in treating dermatitis, colds, and infections. It also has been used in a medicinal dandruff shampoo. Other modern-day research found that the herb can reduce arthritic activity.

An extract of licorice is made by crushing the fresh or stored roots, then boiling or passing steam through them and evaporating the liquid, leaving a thick paste or solid black glossy substance with a sharp fracture. The active ingredient Glycyrrhizin may cause hypertension from potassium loss, sodium retention, and in increase of extracellular fluid and plasma volume. It is fifty times sweeter than sugar. Licorice also reportedly contains steroid hormones, but their relation to licorice’s biological activity is yet to be determined, though extracts have been shown to be estrogenic in laboratory animals. Perhaps the most common medicinal use is in cough syrups and cough drops; licorice soothes the chest and helps bring up phlegm. Licorice has also been used to treat ulcers, to relieve rheumatism and arthritis, and to induce menstruation. In this country it was used in powder form as a laxative.

Licorice root is being used today in France and China in eye drops that relieve inflammation. Sodium salts of glycyrrhinic acid are extracted from the root and added to the eye drop formula. The cortisone like action of the licorice root extract is responsible for its healing effects.

Safety:
Caution : There is a small possibility of effecting electrolyte balance with extended use of large doses of licorice. It has an ACTH like effect causing retention of sodium thus raising BP. The whole herb has constituents that counter this but it is best to avoid Licorice if the patient has hypertension, kidney disease or during pregnancy.

It may interfere with the calcium and potassium absorption. Do not use if you are suffering from osteoporosis, hypertension (increases water around heart). Take with boiled milk.

No other information about the safety of this herb is available. Use caution. Ayurvedic herbs are often taken in combination with others to neutralize the toxicity one herb with the opposing effect of other. Do not take except under the supervision of a qualified professional.
Its major dose-limiting toxicities are corticosteroid in nature, due to the inhibitory effect its chief active constituents, glycyrrhizin and enoxolone, have on cortisol degradation and include oedema, hypokalaemia, weight gain or loss, and hypertension.

The United States Food and Drug Administration believes that foods containing liquorice and its derivatives (including glycyrrhizin) are safe if not consumed excessively. Other jurisdictions have suggested no more than 100 mg to 200 mg of glycyrrhizin per day, the equivalent of about 70 to 150 g (2.5 to 5.3 oz) of liquorice.

Bleeding can occur in any part of the digestive tract and should always be investigated because there may be a serious underlying cause. In some cases, only small amounts of blood are lost over a long period of time and go unnoticed. In other cases, severe, sudden bleeding from the digestive tract may result in blood being vomited or passed out of the anus in the feces. You should seek medical help if you notice any bleeding.

Bleeding in the digestive tract is a symptom of a disease rather than a disease itself. Bleeding can occur as the result of a number of different conditions, some of which are life threatening. Most causes of bleeding are related to conditions that can be cured or controlled, such as ulcers or hemorrhoids. The cause of bleeding may not be serious, but locating the source of bleeding is important.

The digestive or gastrointestinal (GI) tract includes the esophagus, stomach, small intestine, large intestine or colon, rectum, and anus. Bleeding can come from one or more of these areas, that is, from a small area such as an ulcer on the lining of the stomach or from a large surface such as an inflammation of the colon. Bleeding can sometimes occur without the person noticing it. This type of bleeding is called occult or hidden. Fortunately, simple tests can detect occult blood in the stool.

Possible causes:

The causes of bleeding in the digestive tract include inflammation of or damage to the tract’s lining and tumors.

Bleeding from the upper tract, which includes the esophagus, stomach, and duodenum, may occur when stomach acid damages the lining of these organs. This is a common complication of the gastroesophageal reflux and peptic ulcers. Severe bleeding is sometimes due to enlargement of veins in the esophagus, which may be a complication of chronic liver diseases.

Most cases of bleeding from the lower digestive tract, which includes the colon, rectum, and anus, are due to minor disorders, such as hemorrhoids or a fissure caused by straining to defecate. However, bleeding may be a sign of colorectal cancer. Diverticulosis and other disorders of the colon can also lead to the presence of blood in the feces.

Symptoms:
The symptoms vary according to the site and the severity of the bleeding. if the bleeding is mild, blood loss may go unnoticed, but it may eventually cause symptoms of anemia, such as pale skin and shortness of breath. Severe bleeding from the esophagus, stomach, or duodenum may cause:

· vomit containing bright red blood or resembling coffee grounds.
· light-headedness.
· black, tarry stools.

If there is a heavy loss of blood from the lower part of the tract, there will probably be visible blood in the stools. When there is severe blood loss from any part of the tract, shock may develop. Shock causes symptoms that include fainting, sweating, and confusion and requires immediate hospital treatment.

What might be done?
Minor bleeding may be detected only during an investigation for anemia or screening to detect colorectal cancer. If the bleeding is severe, you may need intravenous fluids and a blood transfusion to replace loss blood. You will be examined to detect the location of the bleeding, usually by endoscopy through the mouth.

Treatment for bleeding depends on the underlying cause. For example, peptic ulcers are treated with antibiotics and ulcer-healing drugs, but colorectal cancer needs surgery. It may be possible to stop bleeding by a treatment done during endoscopy, such as laser surgery, making open surgery unnecessary. Treatment is usually successful if the cause is identified and treated early.

Salt intake linked to stomach ulcer

Eating too much salt is known to cause high blood pressure, heart attacks and stroke. Now US researchers say it can also give you an ulcer.

Presenting the results of their study at the American Society for Microbiology conference, researchers said their findings show salt and Helicobacter pylori (H pylori) bacterium interact to cause ulcers, which affect the digestive track……..click & see

Found in stomach and duodenum, the H pylori bacterium accounts for up to 90 per cent of duodenal ulcers and up to 80 per cent of gastric ulcers.

Many people carry the bacterium without experiencing any symptoms. And some of those who are affected suffer far less severe symptoms than others.

Researchers at the Uniformed Services University of the Health Sciences in the USA found high levels of salt cause genetic changes in the bacterium and make it more powerful, reported online edition of Daily Mail.

The team of scientists headed by Hanan Gancz said the bacterium might also increase the risk of gastric cancer.

“It was known that people who ate a high-salt diet had an increased risk of gastric cancer but no one had looked specifically at the effects of salt on H pylori itself,” the researchers added.

Yashtimadhu or Licorice is one of the greatest herbs known to mankind. Egyptian hieroglyphics record the use of Licorice in a popular beverage. Alexander the Great, the Scything armies, Roman Emperor Caesar, and even India’s great prophet, Brahma, are on record endorsing the beneficial properties contained in Licorice. Warriors used it for its ability to quench thirst while on the march, while others recognized Licorice’s valuable healing properties. A very important quality of licorice continues to be its use as a flavoring agent. Glycyrrhiza is Greek meaning ‘sweet root’.

Liquorice/ Licorice, a perennial herb of the genus Glycyrrhiza, in the family Leguminosae is a tall shrub (4  to  5 feet). This tender, twining plant, woody at the base is native of Asia and Mediterranean region and grows in subtropical climates. Glycyrrhiza glabra is its scientific name
A perennial shrub up to 1m high, violet flowers in racemes, dried roots are the source of liquorice. It is a herb or a small shrub up to 1m high with pinnate leaves having 9-17 leaflets. The leaflets ovate and obtuse. flowers pale blue, arranged in a raceme. Caylx glandular, pubescent. The pods glabrous, red to brown having 3-4 seeds. The root light brown, sweet in taste.

CHEMICAL CONSTITUENTS:
Principle constituent of liquorice is the sweet tasting Triterpenoid saponin glyccrrhizin
(2-9%), a mixture of potassium and calcium salts of glycyrrhizinic acid. Include other
triterpenoid saponins like glabranin A&B, glycyrrhetol, glabrolide, isoglabrolide, isoflavones, coumarins, triterpene sterols.

MEDICINAL USES:
The medicinally active sweet juice contained in its root, abounds with a constituent, much used in demulcent compositions. The inspissated juice is used as a confection and for medicinal purposes. Acrid resins, however, render the root irritant and poisonous. The word licorice derives from Greek glykeia rhiza “sweet root” – glykys the modern Greek name means sweet and rhiza means root. This herb known in Malayalam as At(t)i madhuram, Iratti madhuram, can be purchased from angadikkada, the shop in the street Ati madhuram/ Iratti madhuram which means excessively sweet (or extremely charming/ beautiful) is a sugar ally.

Eratti/ iratti means doubling. But Eratti/ iratti madhuram, doesn’t  mean doubling of the sweetness. Iratti is transformed into eratti. The characteristic sweet taste of liquorice is also reflected in the Indian names. In Sanskrit, madhu means sweet, pleasant. This element is found in names for licorice not only in Sanskrit (madhuka and yashtimadhu from yashti “stem, stalk”, but also in modern names of both South and North India, e.g., jestamadha (Marathi), yashthimodhu (Bengali), yashti madhukam, madhu yashti(ka), madhukam, yashti madhuram, yashti, yashtee madhu, madhusrava, yashteekam, kleethakam (Sanskrit), jathi-madh, jethi-madh, mulathi (Hindi), ati-madhura, yashti-madhuka (Kannada), ati-maduram (Tamil), ati-madhuramu, yashti-madhukam (Telugu).

The drug posses potent demulcent, expectorant and anti-inflamattory properties, attributed to the presence of glycyrrhizin, which is about 50 times sweeter than sucrose. Besides these, glyrrhizin is also credited with anti-hepatotoxic, anti-viral and anti-bacterial activites. The drug is also beneficial to peptic ulcer.

In India, the crude as well as its dried aqueous extract is mainly used in bronchial
troubles along with Viola pilosa, Adiantum lunulatum and Justicia adhatoda in the form of
decoction or in lozenges, but in Allopathy, its additional use along with anise oil is as
mild laxative and for masking the bad taste of some herbal preparations of senna aloe,
hyoscyamus, etc.

In Ayurveda, Glycyrrhiza glabra is used in “Yastyadi Churna”, “Yastyadi Kwath” and “Yastimadhavadi Taila”. In Unani system, it is an ingredient of “Banadiq-ul-bazur” used as a diuretic in urinary troubles, of lozenges “Hab Awaz Kusha” and “Hab Maqhas Badam” of “Dawa-i-sandal” a cooling agent for such diseases as syphilis, of “Sufuflodh” for threatened abortion, of “Sharbat Aijaz” a cough syrup and of “Laooq Bihdana” and “Laooq Badama” used as cough linctus.

It is madhura, slightly tikta, sheetala, used in opthalmia, deranged pitta, anorexia, emaciation, allays thirst and cures ulcer.

Therapeutic Uses:
Root (powder) : prescribed in coughs, hoarseness and in respiratory troubles; mixed with citrus juice efficacious in catarrhal affections and with honey in jaundice; in combination with ginger and milk, acts as a good tonic during convalescence; infusion,

Decoction or extract is laxative and an useful medicine in urinary diseases, bronchial and gastric troubles. alterative, galactagogue; good for the eyes, in incipient loss of sight, in diseases of the eyelid; removes biliousness, ear diseases due to biliousness; improves taste; lessens thirst, hiccough, vomiting, fatigue; heals ulcers, wounds; improves the voice; cures” vata “, inflammation, consumption, purifies the blood; useful in leprosy, anremia; hemicrania, haemoptysis, abdominal pains, epilepsy
The root is hot, dry, sweet; diuretic, emmenagogue, demulcent; relieves thirst, cough, vomiting, asthma, bronchitis, abdominal colic, headache; good in eye troubles; cures unhealthy humours, ulcers.-

The branches are bitter.-
The leaves are used for scalds of the head, and in foul perspiration of the armpits
The root is demulcent, pectoral, and emollient.used for coughs, consumption, and chest complaints..
The root is. said to be good for sore throats

CONTENTS:
Licorice root contains triterpenoid saponins (4-24%), mostly glycrrhizin, a mixture of potassium and calcium salts of glycyrrhizic acid; falvonoids (1%), mainly liquiritin and liquiritigenin, chalcones isoliquiritin, isoliquiritigenin, and isoflavonoids (formononetin) ; amines (1-2%) asparagine, betaine, and choline; amino acids; 3-15% glucose and sucrose; starch (2-30%); polysaccharides (arabinogalactans); sterols (beta-sitosterol); coumarins (glycerin); resin; and volatile oils (0.047%). Also Vitamins E, B-complex, phosphorous, biotin, niacin, pantothenic acid, lecithin, manganese, iodine, chromium, and zinc have been found.

BIOCHEMICAL ACTIONS:
Liquorice is used both in the Western and Oriental medicines. In western medicine, liquorice has been used since the ancient Grecian age as an expectorant and antitussive agent and as ad additive for sweetening. In old Chinese Materia Medica “Shin Nung Pen T Sao Chung”, liquorice was described as a drug for strengthening muscle and bone, and curing wounds.

Since liquorice extracts were effective clinically for treatment of gastric ulcer, but
caused oedema and hypertension in nearly 20% patients treated. The physiological and
pharmacolgical studies on glycyrrhizin, the main saponin of liquorice have advanced
remarkably. The side effect of liquorice extracts was regarded as a mineral corticoid like
action of glycyrrhizin. It has been reported that glycyrrhizin caused retention of Na+and Cl and excretion of K+.

Glycyrrhizetic acid has been shown to possess anti-inflammatory activity, in 1/8th potency of hydrocortisone by the cotton-pellet method. The activity is potentiated to 1/5th of hydrocortisone when carbenoxolone (Sodium salt of hemisuccinate of glycyrrhetic acid) is used. Glycyrrhizin ointment is employed clinically for aphtha and other inflammatory skin diseases.

In these cases, glycyrrhizin or its sapogenin glycyrrhetic acid potentiates the
action of glucocorticoid. Glycyrrhizin has been shown to inhibit ulcers in rats and cures
experimental gastric ulcers caused by acetic acid administration. To avoid side effects in
glycyrrhizin in liquorice preparation, such as oedema and hypertension, a glycyrrhizin free
fraction was studied. Later it has been whon that the fraction named FM 100 was found
effective for gastric ulcers and it contains several iso-flavones and chalcones. De-glycyrrhized liquorice extract is now an important substance for treatment of gastric and duodenal ulcers.
